Abstract

Postoperative acute pain is an unavoidable problem in postoperative conditions. Effective relief of pain is of the most importance to treating patients undergoing surgery. Effective pain management has significant physiological benefits. Manyways for pain management, althought opioid is general pain management and Regional block is a popular techniques in paediatric pain management recently, wether for surgery or need of adequate analgesic condition.

Abstract

Background: High dose opioid has been clinically proven to provide effective blockage of stimuli during head incision but it has several unwanted side effects. Scalp block is an alternative modality which has been used as an adjunct to general anesthesia aimed to blunt stress response throughout craniotomy while maintaining more stable condition in terms of less hemodynamic fuctuation. Objective: The study is aimed to compare fentanyl comsumption in general anesthesia combined with scalp block using saline and general anesthesia combined with scalp block using levobupivacaine 0.5% in patient which underwent craniotomy. Method: Twenty patients suffered from intracranial tumor with age ranging from 18 - 65 years who underwent craniotomy tumor removal at RSUP Dr. Sardjito were divided into 2 treatment groups in which the Levobupivacaine 0.5% block group received general anesthesia with a combination of scalp blocks using Levobupivacaine 0.5% while the control group received general anesthesia with a combination of scalp blocks using saline. Patients with local anesthetic allergy and uncontrolled bleeding were excluded from data collection. Result: The mean of fentanyl addition in patients with Levobupivacaine block was 513.0 mcg with a standard deviation of 164.2 mcg. Whereas in patients with Levobupivacaine block the average fentanyl addition was 1149.5 mcg with a standard deviation of 318.4 mcg. The mean difference in the addition of fentanyl between the two groups of 636.5 mcg showed a signifcant difference p = 0.001 (p <0.05). Conclusion: Combination of scalp blocks using Levobupivacaine 0,5% with general anesthesia showed reduction of fentanyl consumption alongside with statistically insignifcant hemodynamic fuctuation compared to general anesthesia and saline block in supratentorial brain tumor patient undergoing craniotomy. Hemodynamic profle in Levobupivacaine 0,5% group was maintained better than control group.

Abstract

Kegiatan petugas ambulans di tingkat desa memiliki peranan krusial dalam mendukung sistem rujukan kesehatan primer, terutama dalam penanganan kasus kegawatdaruratan di wilayah pedesaan. Namun, penguatan kapasitas dan jejaring antar petugas ambulans desa masih minim perhatian. Pengabdian Masyarakat ini bertujuan mendeskripsikan kegiatan Gathering Ambulans Desa yang diselenggarakan oleh RSU PKU Muhammadiyah Amanah Sumpiuh sebagai bentuk intervensi non-formal dalam meningkatkan kompetensi dan kolaborasi petugas ambulans. Metode yang digunakan adalah deskriptif kualitatif dengan pendekatan studi kasus. Kegiatan yang melibatkan 99 peserta dari 52 desa ini terdiri dari pelatihan BHD, sesi diskusi interaktif, dan sosialisasi layanan RS. Hasil kegiatan menunjukkan peningkatan pemahaman peserta terhadap penanganan kegawatdaruratan serta terbentuknya jejaring lintas desa. Kegiatan ini bisa dijadikan contoh untuk diterapkan kembali sebagai model penguatan sistem kesehatan komunitas. Kegiaitan pengabdian ini menyarankan integrasi pelatihan komunitas dalam perencanaan kebijakan kesehatan daerah.

Abstract

This study aims to analyze the influence of Hospital Syariah Compliance, Patient Satisfaction, and Service Quality on Patient Loyalty, with Patient Trust as a mediating variable. The background of this study is based on the importance of integrating Islamic values in healthcare services to build long-term relationships with patients, especially in sharia-based hospitals. The study was conducted at Muhammadiyah Amanah Sumpiuh Hospital involving 263 patient respondents who had received services at least twice. The method used is a quantitative approach with Partial Least Squares Structural Equation Modeling (PLS-SEM) analysis using SmartPLS-4 software. The results show that Hospital Syariah Compliance, Patient Satisfaction, and Service Quality have a significant effect on Patient Loyalty, both directly and indirectly through Patient Trust. Trust is proven to mediate the relationship between the three exogenous variables on loyalty, with full mediation on the relationship between service quality and sharia compliance on loyalty, and partial mediation on the path of satisfaction on loyalty. The conclusion of this study confirms that trust is a key element in shaping patient loyalty, especially in the context of hospitals based on Islamic values.
