Abstract

Introduction The surgical site infection in Orthopaedic surgery is a serious problem for both the patient and surgeonMethods This study was conducted using retrospective descriptive method using medical record data of the patients that underwent internal fixation surgery of the lower extremities in RSUP H. Adam Malik Medan within the period of January 2012 – June 2014. The characteristics assessed in this study include stage, gender, site of the infection, history of internal fixation surgery (implants) and risk factors.Results There were 25 patients (5.9%) out of 420 sample that developed infection with characteristics, 23 were male (92%) and 2 were female (8%). There were 11 patients (42%) aged less than 30 years old, 11 patients (42%) aged 30-60 years old, and 3 patients (12%) aged above than 60 years old. According to the site of the infection it is found that there were 13 patients has infections originating from the femur (52%), tibial area 8 patients (32%), ankle area 1 patient (4%) and the combination of multiple sites are 3 patients all together. From the data of 25 patients that developed infection it is found that there were 4 smokers and 2 patients suffering from diabetes and 3 elderly patients (above 60 years old).Conclusion: Patients developing infection post internal fixation surgery of the lower extremities in RSUP H. Adam Malik are still considered high. In this study, the risk factor relating to the surgical site infection is unclear although it is known that the risk factors such as elderly age, duration of operation, smoker, diabetes mellitus, length of stay in the ward and mucosal damage around the fracture site causes one to develop infection.Keywords : : internal fixation purgery, post operation, infection, risk factor

Abstract

Objectives : To know the charachteristic of femoral fracture patients in RSUP Haji Adam Malik Medan period of Januari 2009- Desember 2010.Background : Femoral fracture has a high incidence among other kinds of fracture that can caused the serious physical suffering, mentally affected, and loss time of daily working of patients. Thus femoral fracture have low mortality rate but with high morbidity rate. The method of prevention can be done by identifying the related variables through the epidemiology studies comprehensively.Method : Collecting the secunder data that recorded on medical record of patients that diagnosed femoral fracture in RSUP. Haji Adam Malik Medan (RSUP HAM) period of Januari 2009 – Desember 2010.Results : Found that femoral fracture in RSUP HAM most cases is male with age 21-30 years old that cause 93.92 % by traffic accident. Most region of fracture is mid-diaphysis about 54.7%. Also found about 179 samples (72.46%) from outside Medan with most of education level is elementary school about 178 subjects (72.06 %). And they were brought to the RSUP HAM below than 72 hours is about 163 subjects (66.0%). For the subjects with age 0-50 years, most often region of femoral fracture is mid diaphysis, meanwhile for age above 50 years old is neck femur. For the fracture classification, found that closed fracture is more often than open fracture femoral fracture.Conclusion : Most of femoral fracture patient in RSUP HAM is male that classified as productive age caused by road traffic accident. And most of them is in mid diaphysis region of femoral. The preventive method by focusing to decrease incidence of road traffic accident is the main priority followed by improving the tools and skills for treating the femoral fracture cases. Abstract

Background: Total knee arthroplasty (TKA) is the most common surgical option for end-stage knee OA with knee deformity and persistent pain. This procedure has already been increased in national insurance era. There are many conditions may affect the outcome of TKA include: the complexity or difficulty of the surgery or specific states of the affected limb. The difficult conditions in primary TKA are extra-articular deformity, severe coronal deformity, stiffness knee, neglected dislocation or patella subluxation, post-osteotomy (HTO or DFO), previous incision surgery. The paper aims to review the conditions which may adversely affect the usually excellent outcome of TKA.Methods: This is a descriptive study. The study describes the difficult or complex situation in primary TKA. The data for this study was obtained from the 9 Government Hospital in Indonesia. The patients receive surgery between January and December 2019.Results: Totally, there were 881 patients receive TKA in 9 government hospitals in Indonesia during 2019. From those, there were 221 difficult TKA cases 31,38%, and 600 were the simple case (68,11%). Severe coronal deformity are 183 cases. TKA with the stiff knee are 52 cases. Cases with wound problems or previous incision are 12 cases. Ekstra articular deformity are 15 cases. TKA post osteotomy are 9 cases. TKA with genu recurvatum are 6 case. TKA with patella dislocation or subluxation is 4 case.Conclusions: Almost one-third of primary TKA are difficult cases that need much more advanced technique and advance implant (more expensive) for more extended durability (avoid failure and costly-revision). National insurance coverage should recognize primary-difficult TKA.

Abstract

Background: Anterior Cruciate Ligament (ACL) injury is a significant orthopedic problem, particularly among young, active individuals. Anatomical factors, such as a narrow intercondylar notch, are considered intrinsic risks. However, findings on the association between the Notch Width Index (NWI) and femoral notch shape with ACL tears vary across populations. This study aimed to describe these anatomical characteristics in patients diagnosed with ACL injury at H. Adam Malik General Hospital, Medan. Methods: A descriptive observational study was conducted using medical records and Magnetic Resonance Imaging (MRI) data of patients treated between January 2023 and December 2024. A total of 64 patients with confirmed ACL injuries who met the inclusion criteria were included. Data on demographics, mechanism of injury, NWI, and femoral notch shape (classified as Type A, U, or W) were collected and analyzed descriptively. Results: The majority of patients were male (71.9%), with a mean age of 27.75 ± 8.35 years. The most common occupations were student (35.9%) and private employee (32.8%). The primary mechanism of injury was sports-related activities (64.1%), with the left knee being more commonly affected (64.1%). The mean NWI for the cohort was 0.28 ± 0.032. The most prevalent femoral notch morphologies were Type W (46.9%) and Type U (42.2%), while Type A was the least common (10.9%). Conclusion: In this cohort, ACL injuries predominantly affect young adult males involved in sports. The average NWI suggests a tendency towards a narrower intercondylar notch. Contrary to some studies, wider notch shapes (Type W and U) were more frequent than the stenotic Type A, indicating potential population-specific anatomical variations.
