Abstract

Background: Safety climate concerns individual perceptions of safety policies, procedures, practices and behavior in the work environment. The low safety climate increases the risk of work accidents. Improvement safety climate, one of which can be done by increasing safety leadership, which is safety leadership bring policy direction and commitment regarding the importance of occupational safety and health. This research aims to determine the effect safety leadership to safety climate in process industry workers. Method: This research uses the "PRISMA” or preferred reporting items for systematic reviews and meta-analyses. In searching for relevant research using database google scholar and Sciendirect, with keywords “safety leadership”, “safety climate" And “process industry worker”.The total number of journals that meet the inclusion and exclusion criteria is 4 journals indexed Q1. Results: The results of this study show that there is a direct influence between safety leadership to safety climate in a positive direction. The influence is in a positive direction indicating that there is an increase safety leadership influence on safety climate which is better. There is an indirect influence between safety leadership to safety perfomance with through safety climate. Conclusion: Safety leadership influences improving the safety climate for process industry workers. These findings can be used as reference material for improving the safety climate as well as safety performance in process industry, to reducing the possibility of work accidents.

Abstract

Introduction: This study aimed to assess the maturity of safety culture implementation across petrochemical companies in Indonesia. By using a mixed-methods to provide a comprehensive understanding of occupational health and safety risks in high-hazard industries. The petrochemical sector being prone to workplace accidents and process related to incidents. The objective was to identify the maturity level of safety practices and highlight priority areas for improvement, addressing gaps in existing literature on safety culture measurement in industrial settings. Methods: This cross-sectional mixed-methods study involved surveys and observations conducted across 10 petrochemical companies in 2024. A total of 513 participants were enrolled, and data were collected through validated questionnaires, plant visits, and document reviews. Ethical approval was obtained from the Faculty of Public Health Universitas Indonesia and participants provided informed consent. Results: The primary outcome of the study was the maturity level of safety culture, which most respondents rated as “Generative.” Based on company type significant differences (p < 0.05) were found in commitment. While other variables such as information, organizational learning, employee participation, and communication showed no significant differences. There were no significant differences based on industry type and role type for each variable. The findings from qualitative method indicate that most companies have implemented both national safety standards namely SMK3 (Occupational Health and Safety Management System) and international systems (including ISO 45001, OSHA PSM, ILO PSM, and Responsible Care). Conclusion: In conclusion, the importance of a mature safety culture in supporting occupational health and safety practices and highlights the need for continuous improvement in workplace safety systems. Future studies should explore interventions that strengthen these dimensions, helping reduce occupational risks in the petrochemical industry.

Abstract

Background: The workplace risks faced at PT X oil and gas company, located offshore Natuna, are relatively high evidenced by fluctuations in accidents from 2018 – 2022. In 2018, recordable injury attributable to accidents was recorded, then in 2019-2020, there were no accidents, and the prevalence increased in 2021-2022. The company analyzed the main causes responsible for work accidents during this period and carried out intervention measures in Occupational Health and Safety (OHS) program to reverse the trend in 2023. By implementing OHS program, the number of accidents in recordable injury category fell from 4 (2021- 2022) to 1 (2023). This shows that the program has been successful in managing work accidents. Considering that the investigation and analysis identified unsafe acts as the direct causes, Human Factor Analysis and Classification System (HFACS) method was used to analyze other factors associated with accidents. Objective: This study aimed to investigate OHS program actions at PT X in responding to increased work accidents in 2021-2022, and compare with recommendations for improvements from HFACS method. Method: A qualitative design was used with secondary data, comprising 10 work accidents records and investigation reports for recordable injury cases from 2018 – 2022 available in PT X information system. Subsequently, the secondary data were then classified according to four failure stages in HFACS, namely unsafe acts, the precondition of unsafe acts, unsafe supervision, and organizational influence. This classification produced recommendations for improvement, which were then compared with the prevention intervention program for accidents at PT X. Results: The results showed that OHS program intervention to reduce the number of accidents in 2023 was in accordance with the recommendations of HFACS analysis. Conclusion: The application of HFACS in analyzing accidents records and investigation reports produced relatively comprehensive recommendations by focusing on the component stages. Several additional recommendations from the analysis of latent conditions will further improve OHS performance at PT X.

Abstract

Latar Belakang: Implementasi teknologi pada kegiatan operasional perusahaan adalah sebuah keniscayaan, jika perusahaan ingin tetap exist dan bergerak maju seiring dengan kebutuhan zaman. Termasuk dalam kegiatan operasional penambangan batubara yang ada di PT Beraucoal, saat ini berusaha untuk mengimplementasikan teknologi sebagai tools untuk kendali bahaya yang muncul. Salah satunya adalah penggunaan DMS (Driving Monitoring System) yang bertujuan untuk melakukan pengawasan langsung terhadap prilaku pengopersian unit/kendaraan (aktifitas operator di dalam kabin dan kondisi di luar kabin) untuk mencegah kecelakaan yang disebabkan oleh un safe act dari operator saat mengoperasikan unit. Dalam tahap awal, implementasi DMS diterapkan batu hanya pada kegiatan hauling coal, karena selain tingkat incident pada kegiatan pengoperasian unit di lokasi ini cukup tinggi, lokasi ini juga dari segi infrastruktur lebih menudukung, yaitu berupa keberadaan jaringan cellular sebagai salah satu elemn untuk dapat menjalankan DMS secara ideal dan maksimal. Fokus kendali DMS pada tiga Tindakan Tidak Aman yang diketahui sebagai penyebab terbesar kecelakaan pada kegiatan pengoperasian unit di jalan hauling, yaitu: 1. Prilaku berkendara operator (distraction, smoking, phoning, coalision), 2. Fatigue (yawning, head down, closed eyes), 3. Kecepatan Berkendara. Adapun subyek penelitian adalah perusahaan pertambangan yang merupakan mitra kerja PT. Berau Coal, yaitu: PT. PAMA, PT. BUMA, PT. MADHANI, PT. RICOBANA dan PT. MTL. Tujuan: Penelitian ini bertujuan untuk mengetahui implementasi penggunaan driving monitoring system (dms) sebagai kendali bahaya dari aktivitas pengoperasian unit pengangkutan batubara PT. Berau Coal Metode: Penelitian ini adalah kuantitatif deskriptif yaitu dengan menggunakan pendekatan yang disebut dengan analisis data sekunder. Hasil: Dari data yang dikumpulkan, didapat bahwa implementasi DMS telah memberikan dampak terhadap penurunan jumlah incident pada aktifitas pengoperasian unit di jalan hauling, dengan total penurunan incident rata-rata dalam dua tahun setelah implementasi DMS yaitu sebesar 77%dari total rata-rata incident pada kegiatan pengoperasian unit di jalan jauling selama dua tahun sebelum implementasi DMS. Kesimpulan: Penelitian ini menyimpulkan bahwa upaya implementasi teknologi dalam kegiatan operasional pertambangan di PT. Berau Coal terus dilakukan. Salah satu yang dijalankan saat ini adalah implementasi Driving Monitoring System (DMS), yang berfungsi untuk mengcapture atau menangkap Tindakan Tidak Aman yang dilakukan oleh operator saat mengoperasikan unit
