Abstract

Stroke was the highest cause of death in the world in 2015. Stroke was the highest cause of death in Indonesia in 2019. DIY Province was ranked 2nd in most stroke cases in Indonesia in 2018. The severity level of the clinical manifestation of ischemic stroke due to vertebrobasilar artery insufficiency is the worst compared to other areas. This research uses a quantitative descriptive method with a retrospective design. The subjects of this research were ischemic stroke patients at Bethesda Hospital Yogyakarta for the period January 2023. This research used a total sampling technique. Vertebrobasilar artery insufficiency data was obtained from non-contrast CT-scan readings in an umbrella study by dr. Sudharmaji, Sp.Rad. Data on clinical manifestations were obtained from medical records. Data were analyzed using univariate tests with Microsoft Excel. There were 30 research samples with distribution of infarct locations in the cerebellum (6), medulla oblongata (1), occipital lobe (8), pons (15), thalamus (3), and patients whose focal parenchymal lesions were not clearly visualized (3). The distribution of clinical manifestations found was right ipsilateral hemiparesis (11), dysarthria (11), vertigo (11), left ipsilateral hemiparesis (8), alternans hemiparesis (1), dysphagia (2), diplegia (1), drop attack (1), polyuria (1), and others (1). The distribution of the most clinical manifestations is ipsilateral right hemiparesis, dysarthria, and vertigo.

Abstract

Pendahuluan. Lanjut usia (lansia) dengan disabilitas mempunyai keluhan kesehatan paling tinggi. Meskipun keluhan kesehatannya paling tinggi, lansia dengan disabilitas merupakan populasi yang memiliki persentase paling tinggi yang tidak melakukan pengobatan. Minimnya pengobatan kesehatan ini juga menjadi risiko munculnya masalah kesehatan lain. Badan Perencanaan Pembangunan Daerah (Bappeda) Kota Yogyakarta berkolaborasi dengan Dinas Sosial Tenaga Kerja dan Transmigrasi (Dinsosnakertrans) Kota Yogyakarta melakukan kegiatan bertajuk “Jogja Sapa Lansia” berfokus pada lansia dengan disabilitas. Tujuan. Sebagai perwujudan Kota Yogyakarta ramah lansia. Metode. Program service learning bersama mahasiswa. Kegiatan yang dilakukan adalah dalam bentuk kunjungan ke lansia tirah baring. Dalam kunjungan mahasiswa melakukan observasi langsung dan eksplorasi masalah mengenai pemenuhan kebutuhan lansia. Asesmen menggunakan kuesioner short CANE (Camberwell Assessment Of Need For The Elderly). Hasil. Dilakukan kunjungan kepada sembilan lansia yang tirah baring dan didapatkan kebutuhan dominan yang belum terpenuhi adalah kebutuhan pendengaran/penglihatan, mobilitas dan dukungan sosial. Dari sisi caregiver kebutuhan lansia yang belum terpenuhi adalah kebutuhan informasi kondisi kesehatan. Mahasiswa kemudian melakukan edukasi sesuai dengan kondisi lansia. Kesimpulan. Lansia dengan tirah baring perlu dukungan lebih pada kondisi penglihatan/pendengaran, mobilitas, sosial dan informasi kondisinya.

Abstract

Purpose: The purpose of this study is to evaluate the association between high blood osmolality and neurological status in acute ischemic stroke. Methodology: This is a cross-sectional study. Subjects of this study are acute ischemic stroke patients admitted at Dr. Sardjito Hospital. Subjects who met the inclusion and exclusion criteria were analysed mainly the blood osmolality and NIHSS score at the time of admission. Results: The study involved 58 participants, with 29 (50%) exhibiting blood osmolality exceeding 300 mOsm/kg. A bivariate analysis revealed several factors significantly linked to poor neurological condition upon admission. These included elevated blood osmolality (OR=5,296, p=0,014, 95% CI, 1,29-21,7), presence of infection (OR=11,67, p=0,007, 95% CI, 1,95-6,99), increased white blood cell count (OR=4,750, p=0,028, 95% CI, 1,21-18,5), and an ASPECT Score below 7 (OR=14, p<0,001, 95%CI, 3,34-59). Multivariate analysis revealed that high blood osmolality (p=0,031) significantly associated with poor neurological status at the time of hospital admission. Applications: This study highlighted the importance of evaluating blood osmolality in acute ischemic stroke patients as it is associated with neurological deficit.
