Abstract

ABSTRACT Indonesia is one of the countries with the highest burden of TB in the world. The DOTS strategy has been applied to control TB disease, but the implementation of the DOTS strategy has not been optimal due to the obstacles that have arisen. To analyze the implementation of DOTS strategy as tuberculosis control. This research is a systematic literature review using indexed articles on Google Scholar, Science Direct, Pub Med, and Garuda. Articles were selected using PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses). Analysis of article quality using the Critical Appraisal Skills Program (CASP). Eight articles were obtained that were in accordance with the research, indicating that the implementation of the DOTS strategy as tuberculosis control on political commitment in terms of funding, case detection, drug taking supervisor (PMO), as well as recording and reporting has not run optimally, while the drug distribution has been going well. Implementation of the DOTS strategy can run optimally by increasing cross-sectoral collaboration in tuberculosis control. Keywords: Directly Observed Treatment Short-Course Strategy, Qualitative, Tuberculosis Control.  ABSTRAK Indonesia menjadi salah satu negara yang memiliki beban tertinggi TB di Dunia. Telah diterapkan strategi DOTS untuk penanggulangan penyakit TB, akan tetapi implementasi strategi DOTS belum optimal disebabkan oleh adanya hambatan yang muncul. Untuk menganalisis implementasi strategi DOTS sebagai pengendalian tuberkulosis. Penelitian ini merupakan kajian pustaka sistematik dengan menggunakan artikel terindeks pada Google Scholar, Science Direct, Pub Med, dan Garuda. Artikel yang dipilih menggunakan PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses). Analisis kualitas artikel menggunakan Critical Appraisal Skills Program (CASP). Diperoleh delapan artikel yang sesuai dengan penelitian, menunjukkan bahwa implementasi strategi DOTS sebagai pengendalian tuberkulosis pada komitmen politik dalam hal pendanaan, deteksi kasus, pengawas minum obat (PMO), serta pencatatan dan pelaporan belum berjalan optimal, sedangkan pada pendistribusian obat sudah berjalan baik. Implementasi strategi DOTS dapat berjalan optimal dengan meningkatkan kerjasama di lintas sektor dalam pengendalian tuberkulosis. Kata Kunci: Directly Observed Treatment Short-Course Strategy, Kualitatif, Pengendalian Tuberkulosis.

Abstract

ABSTRACT The minimum service standard is a benchmark for the type and quality of services provided to the community at a minimum. The coverage of TB suspects receiving services in Pesawaran District has not reached the national target (100%). To analyze the implementation of minimum service standard policies in the health sector for people suspected of having TB. This research is a qualitative research with a case study approach. The research locations were carried out at the Pesawaran District Health Office, Kedondong Health Center, Kota Dalam Health Center, Kalirejo Health Center, and Roworejo Health Center. Primary data was obtained from in-depth interviews and observations, secondary data was obtained from document review. There were 11 informants in this study consisting of 3 main informants and 9 triangulation informants. The implementation of the policy on minimum service standards in the health sector for people suspected of having TB has not run optimally. There are variables that are not maximized so that they hinder policy implementation, namely resources, communication between organizations and implementing activities, characteristics of implementing agencies, dispositions of implementing agencies, and environmental, social and economic conditions. Implementation of minimum service standard policies in the health sector for people suspected of having TB can be carried out optimally by increasing cooperation and coordination in both government and private circles. Keywords: Qualitative, Minimum Service Standards, Tuberculosis  ABSTRAK Standar pelayanan minimal merupakan tolak ukur jenis dan mutu pelayanan yang diberikan kepada masyarakat secara minimal. Cakupan terduga TB yang mendapatkan pelayanan Kabupaten Pesawaran belum mencapai target nasional (100%). Untuk menganalisis implementasi kebijakan standar pelayanan minimal bidang kesehatan (SPM-BK) orang terduga TB. Penelitian ini merupakan penelitian kualitatif dengan pendekatan studi kasus (case study). Lokasi penelitian dilakukan di Dinas Kesehatan Kabupaten Pesawaran, Puskesmas Kedondong, Puskesmas Kota Dalam, Puskesmas Kalirejo, dan Puskesmas Roworejo. Data primer diperoleh dari hasil wawancara mendalam dan observasi, data sekunder diperoleh dari telaah dokumen. Informan penelitian ini berjumlah 11 orang yang terdiri dari 3 informan utama dan 9 informan triangulasi. Implementasi kebijakan SPM-BK orang terduga TB belum berjalan maksimal. Terdapat variabel yang belum maksimal sehingga menghambat implementasi kebijakan, yaitu sumber daya, komunikasi antar organisasi dan kegiatan pelaksana, karakteristik badan pelaksana, disposisi pelaksana, serta kondisi lingkungan, sosial, dan ekonomi.  Implementasi kebijakan SPM-BK orang terduga TB dapat dilakukan secara optimal dengan meningkatkan kerjasama dan koordinasi baik dilingkungan pemerintah maupun swasta. Kata Kunci: Kualitatif, Standar Pelayanan Minimal, Tuberkulosis.

Abstract

Introduction: The Social Security Agency on Health (BPJS) is one of the health insurance providers that has been implemented in Indonesia, but so far the implementation of BPJS Health in Hospitals has still experienced obstacles in implementing the program. This study aims to analyze the implementation of BPJS Health policies in hospitals in more depth. The research aims to analyze the implementation of BPJS Health policies in hospitals in more depth. Method: This research is a systematic review. The article search using Google Scholar and PubMed databases used the keywords "Policy" AND " Social Security Agency on Health " AND "Hospital". Article searches were carried out from April to May 2024. Articles were selected using Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Inclusion criteria are full-text articles, open access, using Indonesian health insurance policy variables, published from 2020 to 2024, using Indonesian and English. Results: Four articles were obtained which were by the research showing that the implementation of Indonesian health insurance policy in hospitals in terms of the number of human resources, financing, health facility infrastructure, and communication between policy implementers and the community had not been optimal, which had an impact on the health service process. At disposition, the policy implementer already commits to implementing the policy. The bureaucratic structure is running well, as is evident by the existence of Standard Operating Procedures (SOP) for implementing Indonesian health policies. Conclusion: The implementation of Indonesian health insurance policy in hospitals on the resource and communication variables has not been optimal, while on the disposition and bureaucratic structure variables, it has been optimal.   Pendahuluan: Badan Penyelenggara Jaminan Sosial (BPJS) Kesehatan merupakan salah satu penyelenggara jaminan kesehatan yang telah dilaksanakan di Indonesia, namun sejauh ini pelaksanaan BPJS Kesehatan di Rumah Sakit masih mengalami kendala dalam pelaksanaan programnya. Penelitian ini bertujuan untuk menganalisis pelaksanaan kebijakan BPJS Kesehatan di rumah sakit secara lebih mendalam. Penelitian ini bertujuan untuk menganalisis pelaksanaan kebijakan BPJS Kesehatan di rumah sakit secara lebih mendalam. Metode: Penelitian ini merupakan telaah sistematis. Pencarian artikel menggunakan basis data Google Scholar dan PubMed menggunakan kata kunci “Kebijakan” DAN “Badan Penyelenggara Jaminan Sosial Kesehatan” DAN “Rumah Sakit”. Pencarian artikel dilakukan pada bulan April sampai dengan Mei 2024. Artikel dipilih menggunakan Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Kriteria inklusi adalah artikel teks lengkap, akses terbuka, menggunakan variabel polis asuransi kesehatan Indonesia, diterbitkan tahun 2020 sampai dengan 2024, menggunakan bahasa Indonesia dan bahasa Inggris. Hasil: Diperoleh empat artikel yang merupakan hasil penelitian yang menunjukkan bahwa implementasi kebijakan jaminan kesehatan Indonesia di rumah sakit dari segi jumlah sumber daya manusia, pembiayaan, prasarana fasilitas kesehatan, dan komunikasi antara pelaksana kebijakan dengan masyarakat belum optimal yang berdampak pada proses pelayanan kesehatan. Pada disposisi, pelaksana kebijakan sudah berkomitmen untuk melaksanakan kebijakan. Struktur birokrasi sudah berjalan dengan baik, terbukti dengan adanya Standar Operasional Prosedur (SOP) pelaksanaan kebijakan kesehatan Indonesia. Kesimpulan: Implementasi kebijakan jaminan kesehatan Indonesia di rumah sakit pada variabel sumber daya dan komunikasi belum optimal, sedangkan pada variabel disposisi dan struktur birokrasi sudah optimal.

Abstract

Social isolation can increase mortality and most likely have an impact on health. The impact of social isolation and loneliness on health and well-being is recognised globally as a public health problem. Social isolation can be experienced at any age, especially in the elderly. The purpose of this study was to determine the effect of social isolation on the occurrence of cardiovascular disease in the elderly. This study is a systematic review. Article searches are conducted through databases: PubMed, Science Direct, and Google Scholar with the keywords used in article searches are as follows "Social isolation" AND "cardiovascular disease" AND "elderly". Article selection is done using a PRISMA flow chart. Results: From the results of searching articles in databases obtained as many as 1460 articles that have a relationship with the research theme. Then the article's results were adjusted to the study system used and obtained 565 results that were included in the study system criteria. Then from the 565 articles were re-selected by considering the predetermined inclusion criteria and obtained the results of 10 articles. Conclusion: Social isolation has been shown to be associated with increased risk of cardiovascular disease in older adults. Despite limitations in study design and measurement, these findings underscore the importance of addressing social factors in maintaining the health of older adults.

Abstract

The imbalance in bed utilization across several inpatient units at Hospital X poses challenges to service efficiency, such as prolonged waiting times and an increased risk of nosocomial infections. This study aims to evaluate the efficiency of bed management at Hospital X in 2024 using the Barber Johnson Graph method. This research employed a quantitative descriptive design using secondary data obtained from daily inpatient census reports. The analyzed indicators included Bed Occupancy Rate (BOR), Average Length of Stay (AvLOS), Turn Over Interval (TOI), and Bed Turn Over (BTO). The results showed a BOR of 75.91%, an AvLOS of 2.91 days, a TOI of 0.92 days, and a BTO of 95.56 times per year. Based on the Barber Johnson Graph, only BOR and BTO met the efficiency standards, while AvLOS and TOI were below the standard. These findings indicate that although bed occupancy levels and usage frequency were optimal, the relatively short length of stay and rapid bed vacancy intervals require further evaluation. This study provides valuable insights for hospital management in formulating policies to sustainably optimize inpatient service efficiency.
