Abstract

Introduction: Pleuritis Tuberculosis (TB) sebagai salah satu manifestasi TB ekstraparu seringkali didiagnosis bukan secara bakteriologis. Penggunaan Xpert MTB/RIF Ultra dalam mendeteksi kuman Mycobacterium Tuberculosis (MTB) pada cairan pleura menjadi salah satu alternatif penegakan diagnosis. Case: Seorang pria berusia 39 tahun tanpa riwayat TB dengan antibodi HIV reaktif mengeluhkan sesak nafas, batuk non-produktif, dan nyeri dada kanan, serta ditemukan efusi pleura kanan berdasarkan rontgen toraks. Pemeriksaan bakteriologis sputum menggunakan Xpert MTB/RIF Ultra tidak menemukan MTB, namun pada sampel cairan pleura mendeteksi gen MTB. Pasien tersebut kemudian diberikan obat antituberkulosis (OAT) dan antiretroviral (ARV) dua minggu setelahnya. Discusion: Penegakan diagnosis TB secara bakteriologis pada pasien imunodefisiensi masih menjadi tantangan, karena infeksi TB pada kondisi imunodefisiensi sering bersifat pauci-basiler dan sering kali bermnifestasi sebagi TB ekstraparu. Pemeriksaan Xpert MTB/RIF Ultra yang memiliki sensivitas tinggi ternyata dapat mendeteksi gen Mycobacterium Tuberculosis (MTB) pada cairan pleura. Conclusion: Penggunaan Xpert MTB/RIF Ultra pada cairan pleura menjadi salah satu alternatif penegakan diagnosis bakteriologis, sehingga diagnosis pleuritis tuberculosis (TB) pada pasien imunodefisiensi dapat dilakukan lebih cepat dengan sensitivitas yang mendekati kultur mikrobiologi.

Abstract

Antiphospholipid syndrome (APS) is a blood clotting condition characterized by thrombosis and obstetric disorders, more common in women. This case is rarely found in men due to vague symptoms. We reported a 34-year-old male patient who was admitted to Ulin Hospital on April 6, 2022 with complaints of headache, joint pain, and a history of autoimmune disorders. The patient develops cerebrovascular conditions at a young age. The diagnosis is made based on an anamnesis, physical examination, laboratory, and CT Scan, which shows Systemic Lupus Erytematous with APS. Laboratory results showed increased leukocytes, cholesterol, and coagulation, as well as high anticoagulant lupus. The therapies provided include methyl prednisolone, hyloquin, Myfortic, and the anticoagulant warfarin with a target of INR 2-3. The manifestations of APS and Lupus Erytematous vary, so a proper clinical approach is required. The diagnosis of APS in men is often overlooked due to the lack of obvious obstetric symptoms. Without supporting examinations, treatment cannot be carried out effectively, which can worsen the patient's condition. Patients are required to routinely monitor INR for anticoagulants, and evaluations are required for laboratory examinations to prevent economic losses.

Abstract

BACKGROUND Metabolic dysfunction-associated fatty liver disease (MAFLD) is a common chronic liver condition often associated with obesity and diabetes. Empagliflozin, a sodium-glucose cotransporter 2 inhibitor, is an antidiabetic medication that improves glycemic control, insulin resistance, and body weight. This study aimed to examine the efficacy of empagliflozin in adults with MAFLD. METHODS A comprehensive literature search was performed using the PubMed, ScienceDirect, Cochrane Library, Scopus, and Wiley Online Library databases. Randomized controlled trials assessing liver function, lipid profile, metabolic profile, and body composition were included. Weighted mean differences (WMDs) and 95% confidence intervals (CIs) were calculated using random-effects models, and study quality was assessed using the Cochrane Risk of Bias Tool for Randomized Trials. RESULTS 6 RCTs with a total of 636 participants were analyzed. Empagliflozin significantly reduced alanine aminotransferase levels (WMD: –6.65 IU/l, 95% CI: –13.02 to –0.28; p = 0.04) and gamma-glutamyl transferase levels (WMD: −10.60 IU/l, 95% CI: −29.05 to −7.68; p<0.00001). A non-significant reduction in aspartate aminotransferase was observed (WMD: –4.69 IU/l, 95% CI: –9.89 to 0.51; p = 0.08). Empagliflozin significantly improved low-density lipoprotein cholesterol (p = 0.02) and total cholesterol (p = 0.05) levels but did not significantly affect triglycerides, high-density lipoprotein cholesterol, metabolic profiles, or body composition. CONCLUSIONS This meta-analysis highlights the beneficial effects of empagliflozin on liver function and indicates the need for further research on its metabolic effects and long-term outcomes in managing MAFLD.
