Abstract

Tetra-amelia syndrome is a rare congenital disorder characterized by the absence of all four extremities. This extremity disorder occurs in 1 in 1,300 to 2,000 births. This condition is caused by genetic mutations in the WNT3 and RSPO2 genes. Specific risk factors for tetra-amelia syndrome remain unclear. This is also due to the lack of studies that discuss specific risk factors for tetra-amelia syndrome. In this case report, we discuss a stillborn baby with tetra-amelia syndrome to a mother with various pregnancy risk factors in Jember. This case report aims to examine the presence of several risk factors in the mother and their relationship with congenital disorders, especially tetra-amelia syndrome.

Abstract

Introduction: Endometriosis is an estrogen-dependent chronic inflammatory disease characterized by the growth of endometrial tissues outside the endometrium or in the pelvic area. This condition is usually found in reproductive women, 15 – 49 years old. Chronic pelvic pain related to endometriosis is one of the major symptoms that makes women come to the clinic, besides dysmenorrhea and dyspareunia. This condition also affects the fertility of women, which, in some reports, shows that 30 – 50% women with endometriosis are also infertile. Vitamin D is a type of vitamin that is popular for treating reproductive problems. Vitamin D (1,25 (OH)2 D3) serum is important in inflammation and implantation, which supports overcoming endometriosis symptoms. Material and Methods: The Present study performed a cross-sectional study. A total of ninety-one women were included in this study, which was divided into three groups: normal group, the pelvic pain group, and the infertility pain. In this study, from ANOVA and confirmed with LSD 95% test. Results: There is a difference in Vitamin D (1,25 (OH)2 D3) serum level in the chronic pelvic pain group and/or infertility group. This significantly shows that there is a correlation between those variables. Conclusion: Sexual health is an important concern for patients with coronary artery disease. Some patients are symptomatic and have difficulty having sex. This study identifies specific issues that need to be addressed, especially convenient time, medication safety, and effective communication when providing sexual education.

Abstract

Introduction: Perioperative strategies to reduce postoperative pain are important for enhancing patient satisfaction. However, further research and trials has sparked ongoing debates of various strategies regarding efficacy and safety. Objective: This study aims to improve evidence-based strategies regarding the effect of paracervical anaesthetic blocks in patients undergoing laparoscopic hysterectomy. Methods: A systematic literature search was conducted through PubMed, Google Scholar, and ScienceDirect for RCTs in laparoscopic hysterectomy patients administered paracervical blocks and those given placebos. The quantitative analysis of pooled relative risk and mean difference with a 95% confidence interval were performed using the Review Manager 5.4 software in the random-effects model or fixed-effects model forest plot. Results: Based on four RCTs included in the analysis, there were significant differences in overall postoperative pain scores assessed by VAS (Visual Analogue Scale) [MD = -0.82, 95%CI (-1.47 to -1.06), p = 0.01]. The subgroup analysis also showed significant differences in VAS pain scores at 30 min and 1 hour post-operation [MD = -2.13, 95% CI (-3.09 to -1.16), p = 0.0001] and [MD = -2.55, 95% CI (-4.29 to -0.81), p = 0.004]. However, there were insignificant results in adequate pain control [RR = 7.90, 95%CI (0.39 to 158.67), p = 0.18], length of hospital stay [MD = 0.01, 95%CI (-0.52 to 0.54), p = 0.96], additional analgesics requirement at 24 hours [RR = 0.88, 95%CI (0.55 to 1.39), p=0.58], and perioperative complications [RR = 0.90, 95%CI (0.56 to 1.47), p = 0.68].Conclusion: This meta-analysis provides evidence that the administration of paracervical block in patients undergoing laparoscopic hysterectomy is associated with a reduction of postoperative VAS pain score but not associated with the length of hospital stay, adequate pain control, additional analgesics requirement at 24 hours, and perioperative complications.

Abstract

This systematic literature review evaluates the relationship between CA-125 immunohistochemical expression in ovarian epithelial tumors and serum CA-125 levels as diagnostic indicators. Five studies meeting inclusion criteria from 20,500 initially identified articles consistently demonstrated significant positive correlations between tissue expression intensity and serum concentrations. Key findings reveal that elevated preoperative serum levels correspond directly with increased tissue expression intensity, while combined assessment improves diagnostic accuracy. Both parameters effectively predict early recurrence in advanced-stage (III/IV) disease and show distinct variation across histological subtypes, with mucinous carcinomas exhibiting the lowest expression levels. However, methodological limitations, including heterogeneous study designs, varying immunohistochemical techniques, and restricted sample sizes, affect generalizability. Clinical implications support integrating tissue immunohistochemistry with serum measurements and complementary biomarkers like HE4 for enhanced subtype differentiation. Future multicenter prospective studies with standardized protocols are recommended to establish definitive cut-off values and long-term prognostic significance. These findings underscore CA-125’s enduring clinical value while emphasizing that optimal diagnostic performance requires contextual interpretation alongside other markers, histological classification, and comprehensive patient assessment. The systematic PRISMA-guided approach ensured rigorous evidence synthesis, highlighting CA-125’s crucial role in addressing ovarian cancer’s significant global burden through improved early detection strategies.
