Abstract

Background: Tuberculous (TB) meningitis is the most severe manifestation of extrapulmonary TB and contributes to a high mortality rate. The presence of extraneural TB may raise suspicion of TB meningitis (TBM). This study aimed to explore the characteristics of extraneural TB in TBM patients.Methods: This was a cross-sectional descriptive study using secondary data from medical records of TBM patients admitted at Dr. Hasan Sadikin General Hospital Bandung, Indonesia from 2017 to 2021. Demographic and clinical data were collected, including HIV status. TBM cases were classified into grades I, II, and III using Medical Research Counsil (MRC) criteria that were based on Glasgow Coma Scale (GCS) score and the presence of focal neurological deficits. The clinical classification diagnosis of TBM was made based on the Marais diagnostic criteria which included several diagnostic items and corresponding scoring which further divides TBM into three classes. Extraneural TB is defined as the finding of TB outside the nervous system. Disseminated TB was diagnosed based on the finding of ≥2 infected locations.Results: During the study period, 497 medical records were analyzed. Most TBM patients experienced Grade II (76.9%) and extraneural TB site was found in 65.4%, with pulmonary TB as the common site (77%). The highest mortality rate was in disseminated TB (50%). The finding of extraneural TB did not differ between HIV-negative and HIV-positive patients (67.8% vs. 67.9%; p=0.101).Conclusion:  The presence of extraneural TB is common in patients with TBM. Therefore, extraneural TB evaluation is important to ensure TBM diagnosis. Further studies are needed to explore factors related to TBM diagnosis to ensure TBM patient’s wellbeing.

Abstract

Objective: To evaluate the correlation of histological subtype, stage, age, and prognosis in cervical cancer patients treated at Prof. Dr. Margono Soekarjo Hospital, Purwokerto.Methods: This is a retrospective descriptive cohort study conducted using medical records. Data on cervical cancer staging, age, and histopathological subtype were collected from all patients meeting the inclusion criteria. Statistical analysis was performed, and associations were examined using chi-square tests. The data used in the study were obtained from the medical records of patients at the research site from January to December 2024.Result: Of 582 patients identified, 245 met the inclusion criteria. Squamous cell carcinoma (SCC) accounted for 147 cases, and adenocarcinoma (AC) for 98. Most patients were aged >50 years old and diagnosed at stage II. SCC was most common at stage II, while AC was most frequently found at stage III. Statistical analysis revealed a significant but weak association between age, stage, prognosis, and histological subtype.Conclusion: There was a significant weak correlation of histological subtype and age, stage, and prognosis in cervical cancer.Korelasi Tipe Histologis, Stadium, Usia, dan Prognosis pada Kanker Uterin Serviks di Rumah Sakit Margono Soekardjo pada Januari – Desember 2024AbstrakTujuan: Penelitian ini bertujuan untuk mengetahui korelasi antara subtipe histologis, stadium, usia, dan prognosis kanker serviks uteri pada pasien yang diobati di RSUD Prof. Dr. Soekarjo Margono, Purwokerto, dalam rentang waktu Januari – Desember 2024Metode: Studi ini merupakan studi retrospektif deskriptif kohort dengan data dari rekam medis. Data yang diambil adalah stadium kanker, usia, dan subtipe histopatologis dan didapatkan dari semua pasien yang memenuhi kriteria inklusi. Analisis statistik dilakukan serta asosiasi diuji dengan uji Chi-Square. Data yang digunakan dalam penelitian diperoleh dari rekam medis pasien di RSUD Margono Soekardjo, Purwokerto pada bulan Januari hingga Desember 2024Hasil: Dari 582 pasien, sebanyak 245 pasien memenuhi kriteria inklusi. Karsinoma sel skuamosa (SCC) ditemukan pada 147 kasus, sedangkan adenokarsinoma (AC) pada 98 kasus. Sebagian besar pasien berusia >50 tahun dan didiagnosis pada stadium II. SCC paling banyak ditemukan pada stadium II, sementara AC didapatkan paling banyak pada stadium III. Analisis statistik menunjukkan adanya hubungan bermakna dengan korelasi lemah antara usia, stadium, dan prognosis dengan tipe histologis.Kesimpulan: Terdapat korelasi signifikan antara subtipe histologis dan usia, stadium, serta prognosis kanker serviks.Kata kunci: Kanker serviks; prognosis; subtipe histologis; stadium; usia

Abstract

Objective: This study aims to investigate the correlation between maternal lactate dehydrogenase (LDH) levels and neonatal APGAR scores in preeclamptic patients.Methods: This is an observational analytic study with a cross-sectional design conducted at Margono Soekarjo Hospital from January 2022 to May 2024, involving 78 women with preeclampsia who underwent cesarean delivery. Participants were categorized into early-onset (<34 weeks) and late-onset (≥34 weeks) preeclampsia groups. LDH levels and APGAR scores were analyzed using Spearman’s correlation and ROC analysis.Results: This study showed that elevated LDH levels were significantly associated with lower one-minute APGAR scores, showing moderate correlations in both groups, and were related to five-minute APGAR scores. The LDH cut-off points predicting poor one-minute APGAR outcomes were 284 IU/L (sensitivity 82.1%, specificity 66.7%) for early-onset and 491.5 IU/L (sensitivity 44.4%, specificity 96.2%) for late-onset preeclampsia.Conclusion: Elevated maternal LDH levels are associated with lower one-minute APGAR scores in preeclamptic patients, especially in early-onset cases, indicating they may serve as an early marker for fetal hypoxia and asphyxia risk. AbstrakTujuan: Penelitian ini bertujuan untuk meneliti hubungan antara kadar laktat dehidrogenase (LDH) maternal dan skor APGAR neonatus pada pasien preeklampsia.Metode: Penelitian ini merupakan studi analitik observasional dengan desain potong lintang yang dilakukan di RSUD Margono Soekarjo pada Januari 2022 hingga Mei 2024, melibatkan 78 wanita dengan preeklampsia yang menjalani persalinan melalui seksio sesarea. Peserta dibagi menjadi dua kelompok, yaitu preeklampsia onset dini (<34 minggu) dan onset lambat (≥34 minggu). Kadar LDH dan skor APGAR dianalisis menggunakan korelasi Spearman dan analisis ROC.Hasil: Hasil penelitian ini menunjukkan bahwa peningkatan kadar LDH berhubungan signifikan dengan skor APGAR satu menit yang lebih rendah, menunjukkan korelasi sedang pada kedua kelompok, serta menandai adanya kaitan dengan skor APGAR lima menit. Titik potong kadar LDH yang memprediksi hasil APGAR satu menit yang buruk adalah 284 IU/L (sensitivitas 82,1%, spesifisitas 66,7%) untuk preeklampsia onset dini dan 491,5 IU/L (sensitivitas 44,4%, spesifisitas 96,2%) untuk onset lambat.Kesimpulan: Kadar LDH maternal yang meningkat berhubungan dengan penurunan skor APGAR satu menit pada pasien preeklampsia, terutama pada kasus onset dini. Hal ini menunjukkan bahwa LDH dapat berfungsi sebagai penanda dini risiko hipoksia dan asfiksia janin.
