Abstract

Oral lichen planus (OLP) is an autoimmune disease that commonly affects the mucocutaneous area. The etiology of OLP remains unclear, but several factors are considered risk factors, such as chronic liver disease (hepatitis C infection), stress, genetics, hypertension, diabetes, smoking, and tobacco chewing. OLP often causes pain, especially during exacerbation periods. OLP management aims to reduce symptoms, improve clinical conditions, reduce the riskof oral cancer, and maintain oral health. This case report presents a case of OLP in a 54-year-old Javanese female patient with complaints of pain in her oral cavity that persisted for three weeks. The same condition occurred three months earlier, but it resolved without treatment. Clinical examination of pathognomonic features of OLP in the form of white, mesh-shaped lesions (Wickham striae) on the buccal and gingival mucosa is the basis for determining thediagnosis of OLP. The ulcerative type of OLP is established based on the appearance of ulcerated lesions in the tongue area and complaints of pain. The patient had a history of hypertension with regular consumption of captopril for the past three years., but there was no documented history of allergies. The results of the psychological assessment with DASS-42 revealed that the patient experienced very severe anxiety, moderate depression, and mild stress. Management in this case was done by prescribing topical corticosteroid, which is dexamethasone mouthwash, whichwas gargled by the patient twice a day. One month after therapy, the ulcerative lesions on the tongue resolved entirely, and pain complaints disappeared. In this case, topical corticosteroids effectively reduced symptoms and improved the clinical condition. However, long-term follow-ups are necessary to ensure that the lesion does not transform into a malignant lesion.

Abstract

Infeksi herpes simpleks labialis rekuren merupakan bentuk sekunder atau rekuren dari infeksi herpes simpleks primer. Pada manusia, virus herpes simpleks bersifat laten dan dapat mengalami reaktivasi. Rekurensi akibat reaktivasi virus tinggi, paparan sinar ultraviolet, trauma jaringan mukosa rongga mulut atau jaringan saraf, kondisi imunosupresi, dan gangguan hormon. Laporan kasus ini bertujuan untuk melaporkan dan membahas tatalaksana dari infeksi herpes simpleks labialis rekuren pada seorang perempuan berusia 23 tahun yang datangke RSGM UGM Prof. Soedomo Yogyakarta dengan keluhan rasa nyeri dan panas akibat sariawan di lidah disertai rasa menebal pada bibir atas yang didahului demam selama empat hari. Kondisi serupa telah beberapa kali terjadi sebelumnya dan diobati dengan krim asiklovir, namun kali ini ditambahkan penggunaan penguat imun. Hasil pengisian kuesioner Depression Anxiety Stress Scale (DASS) oleh pasien menunjukkan nilai stres ringan, dan kecemasan serta stres sedang. Pasien diterapi dengan asiklovir sistemik sebagai terapi kausatif. Pasien sembuh sepenuhnya 14 hari setelah kunjungan pertama. Disimpulkan bahwa kasus ini didiagnosis klinis sebagai infeksi herpes simpleks labialis rekuren. Terapi kausatif dengan pemberian asiklovir sistemik menunjukkan respon yang sangat baik. Penting untuk memberikan edukasi mengenai pengelolaan masalah mental guna mencegah kekambuhan dalam waktu singkat.ne boosters were used. The patient completed the DASS questionnaire, which revealed mild stress, anxiety, and moderate stress values. The patient was treated with 400 mg of oral acyclovir, taken five times daily, accompanied by instructions to manage her mental stress. The patient recovered completely within 14 days after the initial visit. The diagnosis was confirmed through clinical examination, characteristic of herpes labialis. Causative therapy with oral acyclovir had excellent results. It is important to give education on the management of mental issues to prevent recurrences in a short period.
