Abstract

ABSTRAK  Pada tahun 2022, jumlah kematian ibu tercatat sebanyak 4.005 kasus dan meningkat menjadi 4.129 kasus pada tahun 2023. Tingginya angka kehamilan berisiko di Padang Pariaman, khususnya di Klinik Amanah, menunjukkan bahwa pengetahuan ibu hamil mengenai deteksi penyakit yang dapat menyebabkan kematian masih rendah. Salah satu langkah untuk mengatasi hal ini adalah dengan meningkatkan pemahaman ibu hamil tentang pentingnya pencatatan skrining dan deteksi dini. Kegiatan ini bertujuan untuk memberikan edukasi kepada ibu hamil terkait pencatatan skrining dan deteksi dini. Metode yang digunakan adalah penyuluhan dengan bantuan media audiovisual dan leaflet. Hasilnya, kegiatan edukasi berlangsung lancar, peserta aktif dalam bertanya dan menjawab, serta mengikuti acara hingga selesai. Selain itu, pengetahuan ibu hamil meningkat hingga 8,50 poin. Kegiatan ini bertujuan untuk mengantisipasi terjadinya komplikasi selama kehamilan. Kata Kunci: Deteksi Dini, Ibu Hamil, Pencatatan, Skrining ABSTRACT  In 2022, the number of maternal deaths was recorded at 4,005 cases and increased to 4,129 cases in 2023. The high rate of risky pregnancies in Padang Pariaman, especially at the Amanah Clinic, indicates that pregnant women's knowledge of disease detection that can cause death is still low. One of the steps to overcome this is to increase the understanding of pregnant women about the importance of recording screening and early detection. This activity aims to provide education to pregnant women regarding recording screening and early detection. The method used was counseling with the help of audiovisual media and leaflets. As a result, the educational activities went smoothly, the participants were active in asking and answering questions, and followed the event until it was over. In addition, the knowledge of pregnant women increased by 8.50 points. This activity aims to anticipate the occurrence of complications during pregnancy. Keywords: Early Detection, Pregnant Women, Recording, Screening

Abstract

Hospital medical record management continues to encounter numerous operational challenges, including difficulty in locating files, delays in returning documents, inaccurate manual entries, and the potential for misplacement or permanent loss of records. Smart Tracer is a web-based solution developed to enable digital, real-time monitoring of medical record file circulation. This study was conducted to design and build a web-based monitoring system for medical record documents employing the Waterfall software development methodology. Functional verification was performed through Black Box Testing to confirm that each system component operates correctly. The findings demonstrate that Smart Tracer delivers eight integrated capabilities: user authentication, medical record data administration, borrowing and return transaction recording, real-time document status and location tracking, rapid file retrieval, circulation history logging, an interactive monitoring dashboard, and automated activity reporting. Each tested function yielded valid outcomes that met the defined system specifications. Smart Tracer is intended to enhance the overall effectiveness of medical record administration, streamline document tracking workflows, and advance digital transformation in health information management.Keywords: Medical Record, Monitoring System, Web-Based Application, Waterfall Method, Black Box Testing
