Abstract

Background: Depression disorder among the elderly population is a serious issue that is often underdetected and undertreated. Depression can lead to functional impairments, which are comparable to or even worse than those in individuals with chronic conditions such as heart and lung diseases. Bereavement is believed to be one of the risk factors for depression in the elderly. This literature search aims to explore bereavement as an etiology of depression in the elderly. Method: A structured search was conducted using the PubMed, Cochrane, and Science Direct databases, using tailored keywords aligned with the clinical question to obtain relevant results. Results: The search and selection process yielded one selected article that was deemed relevant to the case. This article provided a summary of evidence concerning the relationship between bereavement and the emergence of depression diagnosis in the elderly. Conclusion: The results of this systematic review and meta-analysis indicate that bereavement increases the risk of depression in the elderly. However, further studies are still needed to elucidate the mechanisms underlying depression in the elderly.

Abstract

Background: Attention Deficit Hyperactivity Disorder (ADHD) often emerges in childhood and can persist into adolescence and adulthood. Methylphenidate is the primary medication for managing ADHD in youth, but its long-term effects remain uncertain. Evaluating the evidence on the long-term effects of methylphenidate in children with ADHD is crucial. Case: A 7-year-old with school difficulties, hyperactivity, and distractibility (Conner's score 22) was diagnosed with ADHD and treated with methylphenidate. Significant behavioral improvement was observed (Conner's score 8), but parental concerns regarding long-term medication use persisted. Metode: We conducted a structured search in PubMed, EBSCOhost, Cochrane, and SAGE databases, focusing on the past 5 years of English-language human studies with full-text availability. We selected randomized control trials (RCTs), meta-analyses, and systematic reviews and screened them based on relevance to our clinical question. The search yielded results from PubMed (43 articles), EBSCOhost (46 articles), Cochrane (41 articles), and SAGE (125 articles). After rigorous screening, some articles were identified as relevant. Conclusion: Our case analysis based on evidence revealed that methylphenidate treatment is associated with serious and non-serious side effects, irrespective of treatment duration. Primary care clinicians should exercise caution when prescribing methylphenidate for ADHD management in childhood.

Abstract

Background: Preeclampsia is a common medical complication during pregnancy, affecting approximately 5-15% of all pregnancies. This research aims to identify factors influencing the occurrence of preeclampsia at a Primary care clinic in Bandar Selamat, Tembung, Medan, in the year 2023. Method: This study adopts a cross-sectional approach, involving pregnant women with a gestational age of ≥20 weeks who visited the primary care clinic in Bandar Selamat, Tembung, Medan, from July to December 2023. A total of 37 respondents were sampled using convenience sampling. The dependent variable was the occurrence of preeclampsia, while independent variables included age, parity, medical history, calcium supplementation, and employment history. Multivariate analysis was conducted using SPSS version 23. Result: 16.8% of the respondents experienced preeclampsia. Additionally, 21.6% were in the unhealthy reproductive age group, 72.9% were primigravida, 13.5% had a medical history, 43.2% had poor calcium supplementation, and 40.5% were employed. Multivariate analysis revealed that Age (P-value: 0.044, OR: 24.5, 95% CI: 1.082 - 556.407), Parity (P-value: 0.252, OR: 4.926, 95% CI: 0.32 - 75.4), History of Chronic Disease (P-value: 0.32, OR: 40.484, 95% CI: 1.37 - 1196.2), Calcium Supplementation (P-value: 0.167, OR: 11.582, 95% CI: 0.3 - 374.8), and Employment Status (P-value: 0.679, OR: 1.849, 95% CI: 0.1 - 33.87) are associated with preeclampsia. Conclusion: There is a significant relationship between age, parity, history of chronic disease, and the occurrence of preeclampsia in pregnant women at the primary care clinic in Bandar Selamat, Tembung, Medan. History of Chronic Disease is the most influential variable (OR: 40.484).
