Abstract

Nausea and vomiting after cesarean section with spinal anesthesia is a common problem that can interfere with patient recovery. The use of the Antigua acupressure technique (P6) as a non-pharmacological therapy shows the potential to reduce these symptoms. This study aims to analyze the effect of the neiguan acupressure technique on nausea and vomiting in postoperative sectio caesarea patients with spinal anesthesia. The method used was a systematic review by analyzing three relevant primary studies from PubMed and Google Scholar databases during the period 2019-2024. Data were collected and evaluated to assess the effectiveness of acupressure in relieving nausea and vomiting. The results showed that the neiguan acupressure technique significantly reduced nausea and vomiting, with measurements taken at hour 6 postoperatively showing p=0.003 and hour 12 p=0.005. All studies analyzed showed positive results regarding the effect of acupressure. The Neiguan acupressure technique is effective in overcoming nausea and vomiting in postoperative cesarean section patients with spinal anesthesia. However, other factors also contribute to changes in nausea and vomiting response, so this technique should be considered as part of a comprehensive therapeutic approach.

Abstract

Penatalaksanaan nyeri pascaoperasi pada pasien bedah geriatri merupakan tantangan klinis karena meningkatnya risiko efek samping opioid dan komplikasi pascaoperasi. Multimodal analgesia dilaporkan mampu mengurangi kebutuhan opioid dan mempercepat pemulihan, namun bukti pada populasi geriatri masih terbatas dan heterogen. Meta-analisis ini bertujuan mengevaluasi efektivitas multimodal analgesia dibandingkan analgesia standar dalam menurunkan kebutuhan opioid dan lama rawat inap pada pasien bedah geriatri. Pencarian sistematis dilakukan pada PubMed, Cochrane Central, Web of Science, dan Scopus untuk publikasi periode Januari 2015–Desember 2024. Kriteria inklusi meliputi uji acak terkontrol (RCT) pada pasien berusia ≥65 tahun yang menjalani pembedahan elektif dengan intervensi multimodal analgesia. Outcome primer adalah kebutuhan opioid dan lama rawat inap, dengan penilaian risiko bias menggunakan Cochrane Risk of Bias 2 dan analisis menggunakan model efek acak. Dari 847 artikel yang diidentifikasi, lima RCT dengan total 494 partisipan memenuhi kriteria inklusi. Hasil meta-analisis menunjukkan bahwa multimodal analgesia secara signifikan menurunkan kebutuhan opioid (MD = −3,5 mg), memperpendek lama rawat inap (MD = −0,8 hari), menurunkan skor nyeri 24 jam, serta mengurangi insiden delirium dan postoperative nausea and vomiting (PONV). Kualitas bukti dinilai moderat untuk kebutuhan opioid dan rendah untuk lama rawat inap. Disimpulkan bahwa multimodal analgesia efektif dalam mengurangi kebutuhan opioid, mempercepat pemulihan, dan menurunkan komplikasi pascaoperasi pada pasien bedah geriatri. Pendekatan ini layak dipertimbangkan sebagai standar perawatan pada populasi usia lanjut.
