Abstract

Mitral valve (MV) disease are the second most common clinically significant type of valvular defect in adults. The annual incidence of degenerative MV disease in developed countries is estimated to be between 2% and 3%. MV diseases can be treated through MV replacement or repair. Despite the fact that MV replacement is sometimes the best option for treating MV diseases, it is fraught with complications. Surgical MV repair is the gold standard for treating primary degenerative MR, with approximately 95% of patients treated in designated centers, while the remaining 5% will be considered for one of three transcatheter MV repair, transcatheter MV replacement, or percutaneous transluminal valvuloplasty. Despite the fact that MV replacement is sometimes the best option for treating MV diseases, it is fraught with complications. One of the most common postoperative complication is thromboembolism. Other complications related to MV replacement are the higher risk of re-operation compared to MV repair, endocarditis, paravalvular leaks, prosthetic valve degeneration, valve embolism or migration, and left ventricular outflow tract obstruction (LVOTO). This literature review will explore the various complications related to MV replacement.

Abstract

Acinetobacter baumannii is a gram-negative, non-motile bacterium that causes infections, particularly in wounds and burns. MDR A. baumannii strains are resistant to multiple types of antimicrobials, making them difficult to treat. Phage therapy is a potential strategy to combat MDR A. baumannii infections, as bacteriophages can specifically target and kill bacterial cells without harming human cells. This study aims to review the efficacy of bacteriophages as an alternative treatment for MDR A. baumannii and explore their synthesis methods. The article presents a method for isolating and purifying bacteriophages from waste samples using MDR A. baumannii as an indicator. Bacteriophage therapy has no detectable side effects and can reduce the impact on human gut microflora compared to antibiotics. However, more clinical research is needed to establish the safety and effectiveness of using bacteriophages as an antibiotic-resistant therapy. In conclusion, bacteriophages have the potential to become a therapeutic agent to control nosocomial infections caused by MDR A. baumannii. The specificity of bacteriophages for the infected pathogen reduces their impact on human gut microflora compared to antibiotics. However, more research is required to establish the safety and effectiveness of bacteriophages as an antibiotic-resistant therapy, including studies on potential side effects and administration methods. Overall, this study highlights the potential of bacteriophages as an alternative to antibiotics and the need for further research in this area

Abstract

Evaluasi penggunaan antibiotik penting untuk memastikan bahwa pengobatan yang diberikan sesuai dengan ajaran Islam.Kesehatan dalam Islam dianggap sebagai nikmat dan karunia dari Allah. Oleh karena itu, penggunaan antibiotik dalam pengobatan ISK harus memperhatikan aspek kehalalan, keharaman, etika, dan moralitas. Tulisan ini membahas evaluasi penggunaan antibiotik dalam pengobatan Infeksi Saluran Kemih (ISK) dengan pendekatan keislaman berdasarkan prinsip-prinsip Maqashid Asy-Syari’ah dengan metode narrative literature review. Evaluasi pemberian antibiotik pada pasien ISK dapat dijalankan dengan mengacu pada prinsip Hifzh An-Nafs (menjaga jiwa) dan Hifzh Al-Maal (menjaga harta), yang mengajarkan pentingnya menjaga dosis, indikasi, efek samping minimal, dan harga obat. Dalam pemberian antibiotik, terdapat kategori yang rasional dan tidak rasional. Penerapan prinsip-prinsip Maqashid Asy-Syari’ah dalam pengobatan ISK di RSUD dr. Drajat Prawiranegara menunjukkan upaya untuk memberikan pelayanan medis yang efektif, efisien, dan sesuai dengan ajaran agama. Dengan mengintegrasikan pengetahuan medis dan nilai-nilai Islam, diharapkan pengobatan antibiotik pada pasien ISK dapat menjaga kesehatan jiwa dan raga sesuai dengan ajaran agama.
