Abstract

Diabetes Mellitus is one of the most common metabolic disorders in the world and its percentage tends to increase from year to year. One of the factors to promote a better quality of life is self-care management. Social support is one of the most important factors for improving self-care management. This study aims to identify the description of social support and self-care management in the elderly with type 2 diabetes mellitus in the Mauk Community Health Center, Tangerang Regency. The research design used is a descriptive survey. Data collection used the SDSCA (Summary of Diabetes Self-Care Activities) and MSPSS (Multidimensional Perceived of Social Support) questionnaires which had been tested for validity and reliability. A sample of 167 respondents was taken by purposive sampling technique. The analysis technique used in this study is Univariate. The results of this study indicate that the elderly have social support with an average value of 33.97 and the elderly have self-care management with an average value of 39.11. Conclusions and suggestions, namely family support and self-management of the elderly with Type 2 DM in the moderate range, so it needs to be increased to achieve optimal blood glucose. Abstract

Background: Hypertension, also known as high blood pressure, is a chronic condition characterized by blood pressure exceeding the standard blood pressure limit of 140/90 mmHg. The prevalence of hypertension is predominantly among the elderly due to factors such as age, genetics, lifestyle, and knowledge, making them vulnerable to complications. Given the large elderly population and high prevalence of hypertension, attention to the health of the elderly is crucial. Purpose: To determine the relationship between knowledge and self-care management in the elderly with hypertension. Method: This quantitative descriptive correlation study used a cross-sectional approach and cluster sampling. The study was conducted in various villages within the Kartasura Community Health Center working area in December 2024, at seven of the 15 Integrated Health Posts across four villages. The study population comprised all elderly (2,438 people), and 120 respondents were elderly with hypertension. Univariate and bivariate data analysis was performed using the chi-square correlation test with a p-value of 0.05. Results: The chi-square analysis showed a relationship between knowledge and self-care management, with a correlation value of 0.704 for X2 and a p-value of 0.0001 (<0.05). Therefore, hypothesis (Ho) was rejected and hypothesis (Ha) was accepted, indicating a significant relationship between knowledge and self-care management in elderly with hypertension. Conclusion: There was a correlation between hypertension knowledge and self-care management in elderly hypertensive patients, with a p-value of 0.0001. Suggestion: Future researchers can monitor long-term changes in self-care management. Furthermore, the development of technological interventions, such as health apps, can improve adherence to self-care management in older adults.   Keywords: Blood Pressure; Elderly; Hypertension; Knowledge Level; Self-Care.   Pendahuluan: Hipertensi atau disebut dengan tekanan darah tinggi merupakan penyakit kronis yang dapat dilihat dengan adanya tekanan dalam darah yang melebihi batas standar tekanan darah yaitu lebih dari 140/90 mmHg. Prevalensi penyakit hipertensi ini mayoritas diderita lansia karena faktor usia, genetik,gaya hidup, dan tingkat pengetahuan, sehingga rentan terjadi komplikasi. Dengan adanya jumlah populasi lansia dan prevalensi hipertensi yang tinggi, maka perlunya perhatian terhadap kesehatan lansia. Tujuan: Untuk mengetahui hubungan antara tingkat pengetahuan dengan self care management terhadap lansia penderita hipertensi. Metode: Penelitian kuantitatif deskriptif correlation menggunakan pendekatan cross-sectional dan pemilihan sampel menggunakan teknik cluster sampling. Studi ini dilaksanakan di berbagai desa yang terdapat di lingkup UPTD Puskesmas Kartasura pada Bulan Desember 2024, di 7 posyandu dari 15 posyandu yang diadakan di 4 desa. Populasi pada penelitian ini semua masyarakat yang masuk ke dalam kategori usia lanjut sejumlah 2,438 orang dan sampel dari lansia dengan hipertensi sejumlah 120 responden. Analisis data yang digunakan adalah univariat dan bivariat menggunakan tes statistik korelasi chi square pada ketetapan p-value ˂ 0.05. Hasil: Analisis chi square menunjukkan adanya hubungan antara pengetahuan dan self care dengan nilai korelasi X2 sebesar 0.704, sedangkan p-value yang diperoleh 0.0001 (<0.05), sehingga hipotesis (Ho) ditolak dan (Ha) diterima, menunjukkan korelasi yang signifikan antara tingkat pengetahuan dan self care manajemen terhadap lansia penderita hipertensi. Simpulan: Terdapat korelasi antara tingkat pengetahuan hipertensi terhadap self care manajemen pada lansia penderita hipertensi dengan perolehan p-value 0.0001. Saran: Peneliti selanjutnya dapat memantau perubahan dalam pengelolaan self-care pada jangka panjang. Selain itu, pengembangan intervensi teknologi, seperti aplikasi kesehatan dapat meningkatkan kepatuhan lansia terhadap pengelolaan self-care.   Kata Kunci: Hipertensi; Lansia; Self Care; Tekanan Darah; Tingkat Pengetahuan.
