Abstract

Advanced Glycation End Products (AGE) are proteins that can cause cell destruction by increasing oxidative stress and inflammation. This product resulted from a series of chemical reactions after an initial glycation reaction. A Leydig cell is one of the types of cells affected by AGE. This cell is located in the interstitial of the testes and stimulated by the luteinizing hormone. This study aimed to compare the luteinizing hormone receptor levels in Leydig cell culture of Sprague-Dawley rat induced by AGE only and the one that administered gamma-mangosteen. We conducted an experimental laboratory study on luteinizing hormone receptor levels in Leydig cell culture of Sprague-Dawley rats induced by advanced glycation end products 200 μg/mL and given gamma-mangostin 5 μM compared to the one that was not given gamma-mangostin. The highest mean of LHR level was in group 3 given gamma-mangostin 5 μM (8.06 pg/ cells/24h), and the lowest mean was in group 1 (control) (7.78 pg/ cells/24h). The LHR levels in the rats' Leydig cell culture given 5μM gamma-mangostin were higher than the other groups indicate the inhibition capacity on the oxidation process caused by AGE in aging rats Leydig cells culture.

Abstract

Testicular damage is a serious complication of diabetes mellitus resulting in male infertility, which is associated with caspase-3 and IGF-1 mRNA expression. Platelet-rich plasma (PRP), with its rich growth factor composition, has proven beneficial in regenerative therapy. It is believed that PRP has not been studied in testes for diabetes mellitus and there are no studies in the literature concerning the influence of PRP on expressions of growth factors in testes.The aim of this study was to investigate the efficacy of adjunctive PRP in insulin treatment for repair of testicular damage in a diabetic rat model. Diabetes was induced by administering single dose 60 mg/kg streptozotocin. Twenty Wistar male rats were divided into four groups: group 1, control group; group 2, diabetes without treatment; group 3, diabetes with treated insulin; and group 4, diabetes with treated insulin and PRP. Rats were euthanized after two weeks of treatment, and testes were taken for caspase-3 and IGF-1 mRNA expression measurements.Diabetes mellitus induction caused a significant increase in caspase-3 mRNA expression with p=0.049 and significant decrease in IGF-1 mRNA expression with p=0.004. There was no difference in caspase-3 and IGF-1 mRNA expression of the diabetic rat testis given insulin and PRP compared to without PRP.

Abstract

Diabetes melitus (DM) adalah penyakit degeneratif yang banyak diderita olehmasyarakat dan jumlah penderitanya cenderung semakin meningkat. DM mengakibatkanberbagai hal yang merugikan terutama dapat menyebabkan penurunan atau kerusakanfungsi organ maupun sistem tubuh. Gangguan fungsi seksual adalah salah satupermasalahan kesehatan yang ditimbulkan oleh penyakit ini. Studi literatur dilakukanuntuk mengetahui beberapa gangguan fungsi seksual yang diakibatkan oleh penyakitdiabetes melitus. Penelusuran literatur dilakukan melalui basis data Pubmed dan NCBI.Jumlah penderita penyakit DM cenderung meningkat. WHO memperkirakan sekitar 330juta penduduk dunia yang menderita DM di tahun 2030. Pada tahun 2005, terdapatsekitar 24 juta penderita DM di Indonesia. Gangguan fungsi seksual yang di timbulkanoleh DM bisa di alami oleh pria maupun wanita, yaitu gangguan keinginan seksual,gangguan bangkitan seksual dan gangguan ejakulasi pada pria. Gangguan keinginanseksual terutama pada pria DM dikaitkan dengan penurunan kadar hormon testosteron.Gangguan bangkitan seksual pada pria DM muncul berupa disfungsi ereksi dan berkaitandengan kerusakan endotel pembuluh darah dan kerusakan saraf perifer sedangkangangguan bangkitan seksual pada wanita berupa gangguan lubrikasi vagina. Gangguanejakulasi pada pria DM dapat muncul karena gangguan saraf otonom. Hal ini dapatditimbulkan oleh berbagai macam faktor antara lain metabolic insult terhadap serabutsaraf, insufisiensi neurovaskular, kerusakan autoimun, dan defisiensi neurohormonalfaktor pertumbuhan. Abstract

Background: Menopause is an event that marks the end of the reproductive phase of a woman. Vasomotor symptoms are one of the primary symptoms of menopausal women and affect about 40-60% of perimenopausal women. This research aims to investigate the relationship of physical activity with vasomotor symptoms in perimenopausal women.Methods: The population in this study were 173 women aged 45-54 years who are still experiencing menstruation in the Wukirsari village working area of Puskesmas Cangkringan Sleman-Yogyakarta. The instrument for measuring vasomotor symptoms adopted two questions from the menopause rating scale (hot flushes and night sweats) questionnaire while measuring physical activity using a questionnaire from Riskesdas. Data were analyzed using SPSS version 23 for Windows.Results: Most of the participants were in the 45-50 years age group (75.73%), followed by severe vasomotor symptoms (95.95%), sufficient physical activity (91.91%), medium level education (49.71%), and informal occupation (94.80%). In addition, this study found that most of the participants were normal Body Mass Index (BMI) (54.34%), passive smoking (64.74%), and pre-hypertension (46.20%). There was a significant relationship between physical activities and BMI to the vasomotor symptoms among women of perimenopausal age (p<0.05).Conclusion: There was a significant relationship between physical activities and BMI the vasomotor symptoms among women of perimenopausal age.

Abstract

Background: Sperm analysis is an initial detection procedure of male infertility recommended by the WHO. Sperm motility as one of the important parameters in sperm analysis has a close correlation with various determinants of male infertility and the success of Assissted Reproductive Technology (ART). Several laboratory assays were carried out as a complement to sperm analysis to describe the sperm ability for fertilization. One of the assays was the modified sperm stress test (MOST) to predict oocyte fertilization in In-Vitro Fertilization (IVF).Objective: This study aimed to assess the correlation between MOST ratio and sperm parameters in infertile men.Methods: This study is a descriptive-analytic study by taking sperm analysis and MOST data from 864 males with infertility problems who went to an infertility clinic, Sadewa IVF, Yogyakarta, Indonesia from May 2015 to November 2020.Results: The mean sperm volume and concentration of the 864 patients were 3.04 mL and 35.74 x 106 / mL respectively. The mean forward motility and normal morphology were 47.93% and 2.07% respectively. The mean leukocyte cell number was 0.84%, and the mean MOST ratio was 0.51. The results showed a positive correlation between sperm concentration (r = 0.235) and motility (r = 0.302) with the MOST ratio. Meanwhile, the leukocyte parameter negatively correlated with the MOST ratio (r = -0.076) and no correlation between other sperm parameters to the MOST ratio.Conclusion:There is a positive correlation between sperm concentration and motility with the MOST ratio, and a negative correlation between the number of leukocytes and the MOST ratio. This result supports the findings of previous studies regarding the importance of sperm motility examination in the success of ART. To the best of our knowledge, this is the first study to assess the MOST in male infertility patients with the big sample size.

Abstract

The prevalence of varicocele is estimated to be around 15-20% of the male population, and of these, about 35-40% are patients requiring infertility treatment. A varicocele is a dilation of the veins in the spermatic cord diagnosed by physical examination of the male genital area and assisted by scrotal ultrasound. The development of electronic nose technology provides an opportunity to detect disease characteristics of volatile organic compounds produced by biological materials. This study aims to utilize the metabolomic gas produced from the odor of the seminal fluid by using an electronic nose. The identification of the pattern of volatile organic compounds formed was labeled as unilateral varicocele, bilateral varicocele and clinical non varicocele as the basis for classification with supervised machine learning. In this study, the accuracy values were quite good for several algorithms, both in training accuracy and testing accuracy with an average accuracy value above 80%.

Abstract

Total motile sperm count and DNA fragmentation Index are two parameters in sperm analysis that have recently been used to determine the outcome of the management of cases of male infertility. Total Motile Sperm Count is one of the values considered better than the 2010 World Health Organization standard sperm analysis in terms of predictive value for the success of the spontaneous ongoing pregnancy rate. High DNA Fragmentation Index values were associated with lower pregnancy success and an increased risk of low fertilization rate or total fertilization failure. In this study, we developed a method to classify sperm analysis based on total motility sperm count and DNA Fragmentation Index values by using an electronic nose. In the total motility sperm count (TMSC) study, we use four algorithms with the result of accuracy values 95% and in the DNA fragmentation Index study, we get a fairly good accuracy value for two algorithms with the accuracy values 70%.

Abstract

Puberty is a series of physical development and sexual maturation that aims to achieved reproductive capacity. The hypothalamic-pituitary-gonadal (HPG) axis has an important role in the normal process of puberty. Disruption in the HPG axis causes hormonal disturbances, one of which is hypogonadotropic hypogonadism, which is associated with delayed puberty. Hormonal disorders can also cause various problems besides the reproductive system, such as muscle weakness, lack of energy, and an increased risk of bone fractures. We report the case of a male patient who complained of predominant muscle weakness, did not show signs and symptoms of puberty, and had an idiopathic hypogonadotropic hypogonadism who unresponsive to human chorionic gonadotropin (hCG) therapy. We decided to administered Testosterone Undecanoate (TU) therapy to overcome the problem of delayed puberty and complaints of muscle weakness experienced by the patient. TU therapy has shown beneficial effects in these patients by increasing testosterone levels, increasing penis length, and eliminating complaints of muscle weakness.
