Abstract

Salah satu kondisi krisis yang memerlukan tindakan segera yaitu syok. Syok hipovolemik adalah suatu kondisi kegawatdaruratan yang ditandai dengan hilangnya kemampuan intavaskuler untuk mempertahankan volume secara adekuat yang disebabkan oleh intake yang tidak adekuat, kehilangan cairan berlebih atau gabungan dari kedua kondisi tersebut. Manifestasi klinis pada syok hipovolemik diantaranya hipotensi, oliguria, takikardi, pernafasan cepat dan dangkal, gelisah dan agitasi, penurunan kognitif, kulit lembab pucat dan dingin. Tujuan dari penulisan ini adalah untuk memberikan gambaran asuhan keperawatan pada pasien dengan syok hipovolemik di ruang Instalasi Gawat Darurat Rumah Sakit Daerah Gunungjati Kota Cirebon. Metode penelitian pada kasus ini adalah study kasus deskriptif. Subjek dalam penelitian ini adalah pasien dengan syok hipovolemik di Instalasi Gawat Darurat Rumah Sakit Daerah Gunungjati Kota Cirebon. Berdasarkan pengkajian yang dilakukan pada pasien didapatkan masalah keperawatan pola nafas tidak efektif, perfusi perifer tidak efektif dan hipovolemia. Intervensi yang dilakukan selama 1 x 7 jam di ruang Instalasi Gawat Darurat dengan tujuan dan kriteria hasil masing masing diagnosa. Setelah dilakukan implementasi keperawatan penulis melakukan evaluasi. Hasil evaluasi pada diagnose pola nafas tidak efektif didapatkan masalah teratasi sebagian dengan hasil ferkuensi nafas menurun menjadi 25 x/menit. Begitupula dengan diagnosa perfusi perifer tidak efektif dan hipovolemia didapatkan masalah teratasi sebagian sesuai dengan kriteria hasil tekanan darah sistolik dan diastolic membaik 136/80 mmHg, SPO2 100% dengan Non Rebreathing Mask 10 lpm.

Abstract

The basic need for oxygenation is one of the most vital basic needs in human life. This is related to the cellular metabolic process, where this process requires oxygen in the body. The impact that occurs if the lack of oxygen in the body is not immediately resolved can cause a lack of oxygen in the body's cells. Body cells that lack oxygen will have difficulty concentrating because metabolism is disrupted due to a lack of oxygen supply in the blood. This case study aims to explain nursing care for patients with a diagnosis of Bronchopneumonia medicine using oxygenation therapy interventions, nebulizer inhalation therapy, and semi-fowler position or head up 30o. This research uses a qualitative approach with case studies as the main method, using observation sheets, interviews, and documentation studies. The subject of this research was one patient, An. K patient with bronchopneumonia at Sumber Kasih Hospital, Cirebon City. The results of the case study showed clinical success that the nursing problem of ineffective airway clearance and ineffective breathing patterns was resolved by increasing airway clearance, improving breathing patterns, and increasing oxygen saturation.

Abstract

Hemorrhagic stroke is a type of stroke caused by rupture of blood vessels in the brain, resulting in bleeding in the brain. The main risk factor that can influence a stroke is hypertension. This study aims to provide a picture of the implementation of nursing care for patients with hemorrhagic stroke using a nursing process approach. This research uses qualitative methods. The type of case study used in this research is a prospective case study. This research was carried out at RSD Gunung Jati, Cirebon City, precisely in the ICU room on March 1 2024. Data collection techniques were through observation, interviews and Further documentation studies use data analysis. After providing nursing care to the patient with management of hemodynamic stability for 3 x 24 hours, given therapy, the results showed that the patient’s blood pressure was stable 150/95mmhg, the patient’s consciousness was sopor GCS 6 and the muscle strenght of the right upper lower extremity was 0 in the patient’s.

Abstract

All pregnant women have the potential to experience complications during pregnancy and the postpartum period. To prevent complications such as bleeding and infection in the postpartum period, comprehensive nursing care is essential, including counseling on postpartum care. This counseling aims to meet the mother's need for knowledge and skills related to self-care, newborn care, family adaptation, and health recovery.  This study aims to describe the implementation of nursing care in postpartum mothers through the nursing process approach. A case study design was used on primiparous postpartum mothers with spontaneous delivery in the Delima room, Waled Hospital, Cirebon Regency. The research was conducted on February 12, 2024.  The results showed that breast milk production can be accelerated by nonpharmacological measures such as oxytocin massage, which involves massaging the area around the back (vertebrae pars thoracica) to stimulate the release of breast milk. The mother reported feeling satisfied, happy, and confident in her ability to provide breast milk to her baby. Although the problem of ineffective breastfeeding related to inadequate milk supply was not fully resolved, patients reported feeling comfortable after oxytocin massage. The implication of the study suggests that oxytocin massage can be an effective intervention in accelerating milk release and improving the well-being of postpartum mothers. Appropriate counseling and nursing care are crucial in helping postpartum mothers overcome challenges and improve the quality of self-care and newborn care.
