Abstract

Background: Digital transformation in public health services has led to the development of the Mobile JKN application by BPJS Kesehatan, aiming to improve access, efficiency, and quality of outpatient services. However, the implementation still faces challenges related to system reliability, user readiness, and service performance, especially in regional hospitals. Objectives: This study aims to evaluate the relationship between service quality dimensionsm (efficiency, fulfillment, and system availability and patient satisfaction with the use of the Mobile JKN application among outpatients at RSUD Otanaha, Gorontalo. Methods: A quantitative analytic study with a cross-sectional design was conducted in April 2025. A total of 97 respondents were selected using accidental sampling. Data were collected using a structured questionnaire based on the E-S-QUAL model, tested for validity and reliability. Data analysis included descriptive statistics and Spearman correlation using SPSS. Results: The majority of respondents rated the Mobile JKN service as “less good” in efficiency (35.1%) and system availability (41.2%), while 48.5% expressed being “less satisfied” overall. Correlation results showed a strong, positive, and significant relationship between efficiency (r = 0.748), fulfillment (r = 0.624), system availability (r = 0.739), and patient satisfaction (p = 0.000). The results also indicated that elderly patients with lower digital literacy experienced more difficulties using the application. Conclusion: The quality of digital health services, particularly in efficiency and system availability, significantly influences patient satisfaction. Targeted educational interventions and user support are crucial for ensuring equitable access to health technology among vulnerable groups such as the elderly.

Abstract

Background: The Mobile JKN application is a digital innovation by BPJS Kesehatan designed to simplify public access to the National Health Insurance system. However, its adoption remains limited, partly due to patients’ lack of knowledge regarding the application’s functions and use. Objectives: This study aims to examine the relationship between the knowledge level of outpatients and their use of the Mobile JKN application at RSUD Otanaha in 2025. Methods: This research used a quantitative method with a cross-sectional design. A total of 97 outpatients were selected using the Slovin formula and accidental sampling technique. Data were collected using a structured questionnaire consisting of five indicators: know, comprehension, application, effort expectancy, and performance expectancy. Statistical analysis included univariate description and Pearson correlation to assess the relationship between knowledge and Mobile JKN usage. Results: The study revealed that most respondents rated the Mobile JKN application's efficiency (35.1%) and system availability (41.2%) as “less good,” while 48.5% expressed overall dissatisfaction. Pearson correlation analysis showed a strong and statistically significant relationship between patients’ knowledge and their perception of the application's usability and performance. Specifically, knowledge was strongly correlated with effort expectancy (r = 0.733) and performance expectancy (r = 0.819). Comprehension and application dimensions also demonstrated strong correlations with both effort and performance expectancy (r values ranging from 0.770 to 0.830, p < 0.001). Elderly users experienced more difficulties due to lower digital literacy. Conclusion: There is a significant relationship between patient knowledge and the use of the Mobile JKN application. Knowledge plays a key role in optimizing the application’s utilization. More intensive educational efforts are needed to empower patients to use the app independently as part of improved health service delivery.

Abstract

Background: The Health Workload Analysis (ABK-Kes) method is a standardized approach used to calculate the need for healthcare personnel based on actual workloads in each functional unit. Following the implementation of the Electronic Medical Record (EMR) system at RSUD Toto Kabila, several disparities in workload distribution were observed, with some staff, particularly in the analysis unit, assigned overlapping tasks. This condition led to inefficiencies and an imbalance in human resource deployment. Objectives: This study aimed to analyze the ideal number of medical record personnel required after the implementation of the Electronic Medical Record (EMR) system at RSUD Toto Kabila in 2025 using the Health Workload Analysis (ABK-Kes) method, in order to ensure optimal human resource distribution and workload efficiency. Methods: This research employed a quantitative descriptive design. All medical record officers at RSUD Toto Kabila were included using total sampling. Data were collected through observation, structured interviews, stopwatch timing, and document review. The ABK-Kes method was applied to calculate workload volume, effective working time, and standard time per task in each unit. Results: The results indicated that the ideal staffing needs include 3 officers in outpatient registration, 2 in inpatient/ER registration, 4 in analysis, 5 in coding, 2 in filing, and 1 in reporting. Staffing shortages were identified in the inpatient registration and analysis units, while other units met or exceeded requirements. An imbalance in task distribution was found, especially in cases where one officer performed duties across multiple units. Conclusion: The implementation of the EMR system has created changes in workload distribution among medical record officers. The ABK-Kes method provides a structured basis for evaluating staffing needs. Periodic evaluation and strategic workforce planning are necessary to ensure balanced workloads and improved data accuracy in EMR management.

Abstract

The implementation of electronic medical records (EMR) as a computerized health information system has become a strategic focus in modernizing hospitals in Indonesia. RSUD Toto Kabila Bone Bolango has adopted this system for outpatient services since early 2025. However, the implementation process still faces several challenges, including limited network infrastructure, uneven staff training, high dependence on IT teams, data input errors, and suboptimal system utilization by medical personnel. This study aims to explore the implementation of EMR at RSUD Toto Kabila and analyze its impact on service efficiency and healthcare worker readiness. The research used a qualitative approach with phenomenological methods and triangulation techniques, including interviews, observation, and documentation. Conducted from April to June 2025, the study involved seven informants: the head of the medical record unit, IT staff, and outpatient medical personnel. The findings show that EMR implementation has had a positive impact on service efficiency, workflow acceleration, and reduced reliance on paper-based records. Nevertheless, structural issues such as unstable connectivity and competency gaps among professions hinder optimal system use. Strategic improvements in cross-professional training and digital culture reinforcement are needed to ensure the sustainability of electronic health information systems in healthcare facilities.

Abstract

The Integrated Referral System (SISRUTE) is a technological innovation in health services designed to accelerate, simplify, and clarify the patient referral process between healthcare facilities electronically. This system plays a crucial role, especially in emergency services that demand rapid and accurate coordination. However, its implementation at RSUD Toto Kabila remains suboptimal. Key issues include the absence of written Standard Operating Procedures (SOP), limited technical training for health personnel, and unstable internet connectivity. This study aims to describe the implementation of SISRUTE at the Emergency Department of RSUD Toto Kabila Bone Bolango and identify obstacles encountered during the process. This is a qualitative study using a phenomenological approach and source triangulation technique. The research was conducted from May to June 2025 involving five informants from medical and administrative personnel. The findings reveal that SISRUTE implementation is still partial and heavily reliant on admission staff, with minimal involvement from doctors and nurses. The application tends to be slow in emergency situations, and the referral flow is not yet standardized. The study concludes that managerial improvements, cross-professional training, and reinforcement of a technology-oriented work culture are essential to ensure effective and sustainable implementation of SISRUTE.
