Diabetes mellitus type 2 is a chronic metabolic disease characterized by high blood sugar levels, generally occurring in adults. This condition is caused by the body's resistance to insulin or insufficient insulin production. The study aims to describe the management of impaired skin integrity in patients with diabetes mellitus type 2 with modern moist wound healing wound care. The methode of research is a descriptive case study with a nursing care approach. Management for three days of management, two ulcer wounds were found in the patient, namely in the left gluteal and right leg. The wound on the left gluteal area measures 7 cm in length and 3 cm in width, while the wound on the right foot is 5 cm deep and 3 cm wide. Both wounds showed signs of infection characterized by pus, unpleasant odor, and redness at the edge of the wound. Based on these conditions, both wounds were categorized as grade 3 wounds. The nursing diagnosis that was established was impaired skin integrity related to changes in circulation (D.0129). Interventions in the form of wound care with the implementation of modern moist wound healing with the implementation of modern moist wound healing wound care with a duration of 30 minutes for 3 days. Evaluation showed an improvement in the condition of the ulcer wound. Major and minor signs of the wound decreased, such as decreased signs of infection, reduced necrotic tissue, and the disappearance of unpleasant odor from the wound. This indicates that the wound healing process is going well. Management analysis, namely the problem is partially resolved. It is necessary to add a combination of other interventions in the care of moist wound healing in patients with type 2 diabetes mellitus. ABSTRAK Diabetes melitus tipe 2 adalah penyakit metabolik kronis yang ditandai dengan kadar gula darah tinggi, umumnya terjadi pada orang dewasa. Kondisi ini disebabkan oleh resistensi tubuh terhadap insulin atau produksi insulin yang tidak mencukupi. Penelitian bertujuan untuk menggambarkan pengelolaan gangguan integritas kulit pada pasien diabetes melitus tipe 2 dengan perawatan luka modern moist wound healing. Jenis penelitian adalah deskriptif studi kasus dengan pendekatan asuhan keperawata. Pengelolaan selama tiga hari pengelolaan, ditemukan dua luka ulkus pada pasien, yaitu di bagian gluteal kiri dan kaki kanan. Luka di gluteal kiri berukuran 7 cm dan lebar 3 cm, sedangkan luka di kaki kanan kedalaman 5 cmdan lebar 3 cm. Kedua luka menunjukkan tanda infeksi yang ditandai dengan adanya nanah, bau tidak sedap, dan kemerahan di tepi luka. Berdasarkan kondisi tersebut, kedua luka dikategorikan sebagai luka derajat 3. Diagnosa keperawatan yang ditegakkan gangguan integritas kulit berhubungan dengan perubahan sirkulasi (D.0129). Intervensi berupa perawatan luka dengan implementasi modern moist wound healing dengan implementasi perawatan luka modern moist wound healing dengan durasi 30 menit selama 3 hari. Evaluasi menunjukkan adanya perbaikan kondisi luka ulkus. Tanda-tanda mayor dan minor luka mengalami penurunan, seperti menurunnya tanda-tanda infeksi, berkurangnya jaringan nekrotik, serta hilangnya bau tidak sedap dari luka. Hal ini mengindikasikan bahwa proses penyembuhan luka berjalan dengan baik. Analisis pengelolaan yaitu masalah teratasi sebagian. Perlu penambahan kombinasi intervensi lain dalam perawatan luka moist wound healing pasien diabetes melitus tipe 2.
