Abstract

Program pembatasan jumlah anak bertujuan membatasi kelahiran hingga dua anak per keluarga dan mengurangi angka kesakitan serta kematian ibu akibat kehamilan dan persalinan yang berulang. Salah satu alat kontrasepsi yang paling sering digunakan adalah suntikan, karena sederhana, efektif, cocok untuk wanita usia subur, dan aman digunakan saat menyusui. Amenorea adalah kondisi tidak menstruasi selama tiga bulan berturut-turut dan sering terjadi pada penggunaan kontrasepsi jangka panjang. Hal ini disebabkan oleh atrofi endometrium, karena penurunan pengaruh estrogen terhadap endometrium akibat pemberian kontrasepsi yang rutin setiap 3 bulan. Peneliti memakai metode survei analitik dengan pendekatan cross sectional. Penelitian ini dilakukan di PMB R Girsang Rambung Merah, Kota Pematang Siantar, yaitu sebanyak pada bulan Maret sampai Juni 2023. Hasil penelitian adalah distribusi frekuensi lama penggunaan kontrasepsi suntikan KB 3 bulan mayoritas lebih dari 1 tahun yaitu 46 responden (66,7%), distribusi frekuensi mayoritas responden mengalami gangguan menstruasi yaitu 45 responden (65,2%). Hasil penelitian menunjukkan bahwa ada hubungan antara lama penggunaan kontrasepsi suntikan KB 3 bulan dengan gangguan menstruasi dengan nilai p value 0,000 (p<0,05). Pada responden disarankan untuk memperhatikan lama penggunaan suntikan KB dengan adanya efek samping selama penggunaan kontrasepsi tersebut.

Abstract

Communication is at the heart of quality midwifery care. Effective interactions between midwives, patients (mothers), and family members are the foundation for building trust, ensuring understanding of health information, facilitating shared decision-making, and ultimately improving satisfaction and health outcomes. However, various barriers can hinder effective communication in the clinical setting. This literature review examines the importance of effective communication, the challenges faced, and strategies for improving communication management between midwives, patients, and families. The aim is to provide insight into how communication skills can be improved to support patient- and family-centered midwifery care and promote positive pregnancy, labor, and postpartum experiences.

Abstract

Midwifery practice, although focused on physiological events, is not free from clinical risks that have the potential to cause injury or harm to mothers and babies. Incidents such as postpartum hemorrhage, neonatal asphyxia, or infection can have serious consequences. Clinical risk management is a systematic approach to identifying, assessing, and mitigating potential hazards in healthcare to improve patient safety. This literature review aims to explore the application of a clinical risk management framework in midwifery practice, encompassing specific risk identification, risk assessment methods, and effective mitigation strategies. The review results indicate that proactive integration of clinical risk management can reduce adverse incidents, increase adherence to clinical standards, improve team communication, and ultimately, improve maternal and infant health outcomes. Implementation challenges include a lack of a reporting culture, limited resources, and the need for ongoing training. Comprehensive implementation requires leadership commitment and the active participation of all midwifery staff.
