Abstract

Keluarga Berencana (KB) merupakan suatu upaya yang dilakukan secara sadar dan terencana oleh individu atau pasangan suami istri untuk mengatur kehamilan, demi mewujudkan keluarga yang sehat dan sejahtera. Salah satu faktor penting dalam keberhasilan program KB adalah konseling, yaitu pendekatan komunikasi interpersonal dalam pendidikan kesehatan yang bertujuan membantu individu memahami informasi, mengeksplorasi perasaan dan kebutuhan, serta mengambil keputusan yang tepat. Melalui konseling, petugas kesehatan membantu klien memahami fakta-fakta tentang kontrasepsi, mengidentifikasi harapan dan kebutuhan mereka, serta mendukung mereka dalam membuat pilihan yang sesuai dengan kondisi dan keinginannya. Penelitian ini bertujuan untuk mengetahui hubungan konseling dengan pengambilan keputusan menggunakan alat kontrasepsi di Praktek Mandiri Bidan Maria Silalahi. Penelitian ini menggunakan desain analitik cross sectional dengan 40 responden. Analisa data dilakukan menggunakan uji chi square. Hasil menunjukkan ada hubungan antara konseling tentang kontrasepsi dengan pengambilan keputusan menggunakan kontrasepsi. Penelitian ini merekomendasikan perlunya peningkatan pengetahuan pasangan ibu usia subur tentang alat kontrasepi sehingga dapat menentukan kontrasepsi apa yang cocok dipakainya. Bagi petugas kesehatan agar memberikan konseling bagi ibu (wanita usia subur) sebelum menggunakan kontrasepsi dan membantu ibu dalam menentukan kontrasepsi apa yang cocok.

Abstract

Program pembatasan jumlah anak bertujuan membatasi kelahiran hingga dua anak per keluarga dan mengurangi angka kesakitan serta kematian ibu akibat kehamilan dan persalinan yang berulang. Salah satu alat kontrasepsi yang paling sering digunakan adalah suntikan, karena sederhana, efektif, cocok untuk wanita usia subur, dan aman digunakan saat menyusui. Amenorea adalah kondisi tidak menstruasi selama tiga bulan berturut-turut dan sering terjadi pada penggunaan kontrasepsi jangka panjang. Hal ini disebabkan oleh atrofi endometrium, karena penurunan pengaruh estrogen terhadap endometrium akibat pemberian kontrasepsi yang rutin setiap 3 bulan. Peneliti memakai metode survei analitik dengan pendekatan cross sectional. Penelitian ini dilakukan di PMB R Girsang Rambung Merah, Kota Pematang Siantar, yaitu sebanyak pada bulan Maret sampai Juni 2023. Hasil penelitian adalah distribusi frekuensi lama penggunaan kontrasepsi suntikan KB 3 bulan mayoritas lebih dari 1 tahun yaitu 46 responden (66,7%), distribusi frekuensi mayoritas responden mengalami gangguan menstruasi yaitu 45 responden (65,2%). Hasil penelitian menunjukkan bahwa ada hubungan antara lama penggunaan kontrasepsi suntikan KB 3 bulan dengan gangguan menstruasi dengan nilai p value 0,000 (p<0,05). Pada responden disarankan untuk memperhatikan lama penggunaan suntikan KB dengan adanya efek samping selama penggunaan kontrasepsi tersebut.

Abstract

The community service program, "Health education and physical examinations, blood pressure, blood sugar, and free medical treatment at the Pratama Hibas Clinic in Simalungun Regency," was successfully implemented. Health is a human right and an investment for the success of national development. Hypertension, or high blood pressure, is a leading cause of heart failure, stroke, and kidney failure. High blood pressure is called the "silent killer" because people with it often don't show symptoms. The National Heart, Lung, and Blood Institute estimates that half of people with high blood pressure are unaware of their condition. Once diagnosed, patients' blood pressure should be monitored at regular intervals, as high blood pressure is a lifelong condition. Therefore, comprehensive health development is necessary to create a healthy society. The community has received positive feedback, and they hope similar activities will be held regularly. This kind of health education should be ongoing for both the community and their families, as increasing age or apathy among the community can endanger their health. Furthermore, children and siblings in the community also share the responsibility to maintain the health of their fathers, mothers, and siblings with adequate knowledge

Abstract

High-risk pregnancy requires intensive monitoring, multiple interventions, and complex care coordination to ensure optimal outcomes for both mother and fetus. In this context, case management has emerged as a crucial strategy for integrating health, social, and psychological services. This literature review examines the definition and principles of case management, the specific role of case managers in the care of high-risk pregnant women, and the benefits and challenges of its implementation. The goal is to highlight how case management can improve care coordination, service accessibility, and patient satisfaction, ultimately contributing to reduced maternal and perinatal morbidity and mortality.

Abstract

In primary healthcare facilities, quality management of ANC services is crucial to ensure that every pregnant woman receives comprehensive, standardized care. This literature review aims to identify, analyze, and synthesize various quality management models that have been implemented or proposed for ANC services in primary healthcare facilities. The review indicates that existing models often incorporate a systems approach, continuous improvement cycles, and multidisciplinary participation. Key challenges include resource availability, health worker capacity, and adherence to standards. An ideal model should consider the local context, be patient-centered, and supported by a robust monitoring and evaluation system.

Abstract

Midwives are the backbone of maternal and child health services, yet they often work in challenging conditions with limited support. Clinical supervision is a globally recognized key intervention for supporting and developing frontline health workers. This literature review aims to explore the role and effectiveness of clinical supervision in improving midwives' technical and non-technical competencies and work motivation. The review results indicate that structured, supportive, and ongoing clinical supervision is significantly positively correlated with improvements in clinical skills, adherence to practice standards, decision-making ability, self-confidence, job satisfaction, and intrinsic motivation. Effective supervision also helps reduce burnout and professional isolation, particularly in remote areas. Implementation challenges include lack of supervisor capacity, time constraints, and resistance to a culture of blame-based supervision. Optimizing clinical supervision requires investment in supervisor training, the development of a clear framework, and the creation of an organizational culture that supports learning and development.
