Abstract

Background: Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disorder characterized by hyperglycemia resulting from impaired insulin secretion or function. The prevalence of T2DM is increasing globally, including in Indonesia. However, patients’ knowledge regarding disease management remains relatively low. Continuous education is considered an essential strategy to improve disease control.  Objective: This study aimed to analyze the effect of education using booklet media on improving knowledge among T2DM patients enrolled in the Chronic Disease Management Program (Prolanis) at Lima Kaum I Public Health Center, Tanah Datar Regency. Methods: The study employed a pretest-posttest control group design involving a total of 48 respondents, divided into an intervention group (received the booklet) and a control group. The research instrument used was the SDKS INA 1 questionnaire to assess patients’ knowledge. Data were analyzed using the Chi-Square test and the Wilcoxon test. Results: showed a significant increase in patients' knowledge after the educational intervention using the booklet (p=0.000; p<0.05). Conclusion: Therefore, booklet media is proven to be an effective educational tool for enhancing knowledge among patients with Type 2 Diabetes Mellitus.

Abstract

Integrated Patient Progress Notes (IPPNs) is an interprofessional documentation of integrated patient development in the form of patient medical records that serves to improve the quality of interprofessional communication in hospitals. Documentation in the form of a complete IPPNs and the accuracy of writing assessments and plans will help establish smooth collaboration between health workers so that good results can be achieved in healing patients. Fracture is a condition of discontinuity of bone structure caused by trauma or pathological conditions. Femur fracture cases are the most common cases and have a high risk if adequate management is not carried out. This study aims to analyze the accuracy of writing assessment and plan on the Pharmacist's IPPNs for femur fractures in Padang X Hospital. This research is a descriptive qualitative study with prospective data collection from purposive sampling. The results of this study showed that there was no assessment on the Pharmacist's IPPNs (0%) which was written correctly from the 39 Pharmacist's IPPNs analyzed and there was no plan on the Pharmacist's IPPNs (0%) which was written correctly from the 39 Pharmacist's IPPNs analyzed. This study shows that there is no accuracy of pharmacists in writing assessments and plans on IPPNs.

Abstract

The documentation of patient condition progress performed by healthcare professionals through integrated care in patient medical records is known as Integrated Patient Progress Notes (IPPN), This research aims to assess the accuracy of pharmacists' IPPN writing for ovarian cancer patients at X Padang Hospital in October–November 2023. This research is a type of qualitatively descriptive case study. The data collection for this study is prospective. Data collection in this research is done in primary and secondary. Primary data collection is done by means of observation; secondary data acquisition is taken through medical records in the hospital. After conducting an analysis, it was determined that the results of the analysis plan of IPPNs for 36 patients were as follows: 0 CPPTs (0%) were accurately written, and all 36 IPPNs (100%) were written incorrectly. The analysis plan resulted in 36 IPPNs among the 36 patients. The accuracy rate for correctly identified IPPNs was 0% (0 IPPNs), while the rate for incorrectly identified IPPNs was 100% (36 IPPNs).
