Abstract

Pada perusahaan manufaktur baut yang memproduksi baut tipe flange terdapat permasalahan produk reject pada proses pembentukan kepala baut. Produk reject yang terjadi akibat cacat flange berupa cacat diameter flange yang berbentuk oval. Berdasarkan data perusahaan, cacat diameter flange oval terjadi sebanyak 5% dari total output yang telah berjalan, dimana dalam 1 bulan, kebutuhan pengiriman yang dibutuhkan adalah 1.400.000 pcs. Oleh karena itu, perlu dilakukan penerapan metode Plan-Do-Check-Action (PDCA) untuk mengetahui penyebab cacat diameter flange yang berbentuk oval, sehingga proses manufaktur baut tipe flange dapat dioptimalkan. Oleh karenanya penelitian ini bertujuan untuk mengoptimalkan proses manufaktur baut tipe flange dengan menerapkan metode PDCA dalam mengatasi cacat flange. Berdasarkan analisa yang dilakukan, didapatkan hasil cacat flange disebabkan oleh pencucian material yang kurang bersih. Pencucian material kurang bersih menyebabkan material berkerak sehingga proses tidak stabil, dan hasil diameter flange menjadi oval. Oleh karena itu dilakukan standarisasi peningkatan waktu pencucian dari standar awal 10 menit menjadi 15 menit, sehingga cacat flange tidak terjadi, dan proses manufaktur menjadi optimal. Selain itu, penyebab lain cacat flange adalah keauasan dari komponen punch, sehingga perlu dilakukan pergantian secara berkala oleh departemen maintenance.

Abstract

The Hospital Facility Maintenance Installation (IPSRS) at Petala Bumi Regional General Hospital, Pekanbaru, functions under the direct supervision of the Hospital Director and is responsible for maintaining hospital facilities, infrastructure, and medical equipment to ensure safety and service quality. This descriptive-analytic qualitative study (2023) aimed to analyze the medical equipment maintenance system at IPSRS, involving four informants from hospital management and technical staff. Data were obtained through interviews, observations, and document reviews, analyzed using content analysis with triangulation techniques. The findings show that IPSRS faces human resource shortages, with limited staff qualifications and multitasking duties affecting performance. The maintenance budget is available but requires better allocation. Materials and spare parts are mostly sufficient but partly depend on third-party suppliers. SOPs exist but are not fully implemented due to HR constraints and lack of evaluation. The organizational structure functions adequately but needs better HR placement. It is recommended that hospital management add skilled technical staff, update SOPs, and enhance staff competence through training and certification. Strengthening HR, budget management, and maintenance procedures will improve IPSRS efficiency and support high-quality, sustainable hospital services.

Abstract

To enhance the quality of services within the Medical Record Installation, it is necessary to implement a patient registration system that is simple, efficient, and easily accessible. One of the essential components of this system is the patient medical card, which is issued by the medical record unit to facilitate the identification and management of patient data. The main objective of this program is to optimize the hospital’s information management system, particularly within the Medical Record Installation of RSUD Teluk Kuantan, to support accurate, timely, and integrated health data management. The activity was carried out through three main stages: Preparation Phase – including the introduction and orientation at RSUD Teluk Kuantan. Implementation Phase – involving interviews, direct observations, and document reviews within selected medical record units. Reporting Phase – consisting of the preparation of the final report (Chapters I–VIII), followed by consultations with the field supervisor and academic advisor, and concluded with a residency result seminar. The priority problem was identified using the USG (Urgency, Seriousness, and Growth) method, which determined that the main issue was the Patient Medical Card Management System at RSUD Teluk Kuantan. As part of the Action Plan, the proposed solution involved establishing a Memorandum of Understanding (MoU) between the Head of the Medical Record Installation and the Hospital Director, concerning the development and implementation of an improved patient medical card system at RSUD Teluk Kuantan. Additionally, another strategic solution was to recruit new human resources who meet the professional qualifications required for the medical record department. This step aims to strengthen the unit’s operational capacity and ensure that medical record services are conducted effectively and in accordance with hospital standards.
