Abstract

Background: Stroke is still becoming one of the most common non-communicable diseases in Indonesia. Sample Registration System survey in 2016 showed that in Indonesia stroke has become the most common cause of death at all age, about 19.9%. 4 Research conducted by Novbakht found that age, sex, BMI, type of stroke, family history of stroke, hyperlipidaemia, diabetes, and severity of stroke was related and increased the mortality rate. 6 Academic Hospital Universitas Gadjah Mada had not conducted this type of research, so that researcher wanted to know the one-year mortality rate after stroke, factors that related to mortality, and to improve the quality of stroke treatment at Academic Hospital Universitas Gadjah Mada.Objective: To know one-year mortality rate in acute stroke patients in Academic Hospital Universitas Gadjah Mada by looking for the acute stroke characteristic, outcome of stroke patients, and risk factors that cause mortality in stroke patients.Method: This is an analytic research using retrospective cohort study through data collection of acute stroke patients (patients with <5 days stroke onset) during July 2019 until June 2022 then we followed up the condition in the first year after stroke by using medical records, direct examination, or via telephone or video call. Mortality rates were assessed one year after stroke and the predictors of death were evaluated using the cox proportional hazard model.Result: One-year mortality after stroke in our study were related to age (>67 years old) (OR=2,29; 95% CI 1,18-4,46; p=0,015) and the severity of stroke was related to higher NIHSS score (NIHSS score during admission 16-42, (OR=38,49; 95% CI 8,92-166,0; p<0,001)) and ICU admission ((OR=8,01; 95% CI 4,03-15,90; p<0,001)).  Conclusion: An association was found between stroke severity and one-year outcomes of acute stroke patients with old age (>67 years), high NIHSS scores, and ICU admissions

Abstract

BACKGROUND : Trauma becomes the main health problem worldwide due to its high mortality rate. Traffic accidents are a trauma with a high incidence and death rate in Indonesia, specifically in Yogyakarta. Factors causing trauma-related death are the lack of hospital infrastructure, the lack of quality human resources, the lack of standardization of trauma management, and the limited number of hospitals providing comprehensive trauma services. This research aims to identify the characteristics of trauma patients who died while receiving medical treatment at UGM Academic Hospital and to analyze the mortality of patients according to the trimodal distribution to prepare this hospital to become a trauma center hospital.METHOD : This quantitative descriptive research used a cross-sectional method. Data were taken from medical records of patients admitted to the ER of UGM Academic Hospital in 2018-2023 based on predetermined inclusion and exclusion criteria. The research was conducted from March to November 2023.RESULT : This research involved 58 patients consisting of 42 (72.4%) male and 16 (27.6%) female patients. The most common cause of death was brain injury (63%), followed by spinal cord injury (13.7%), multi-organ failure and sepsis (8.6%), cardiac and respiratory arrest (6.8%), and comorbidities patients (6.8%).CONCLUSION : The majority of trauma patients who died at the UGM Academic Hospital are male and aged 60-80 years with the main cause of death of brain injury. The patient dies at the late death stage based on trimodal distribution of death time.

Abstract

Anak sebagai salah satu populasi beresiko tinggi terinfeksi COVID-19. Kasus rawat inap COVID-19 pada anak di Indonesia terus meningkat dan lebih besar persentasenya dibanding negara lain. Anak-anak dengan penyakit komorbid dapat memperparah gejala COVID-19 hingga menyebabkan kematian. Tujuan penulisan artikel ini adalah untuk melaporkan kasus Pneumonia COVID-19 disertai dengan Penyakit Jantung Bawaan (PJB) asianotik untuk mendiskusikan kemungkinan diagnosis banding lain sebagai pemahaman dalam melakukan pengelolaan pasien dengan lebih optimal. Anak perempuan 13 tahun 6 bulan dirujuk ke Rumah Sakit Akademik Universitas Gadjah Mada dengan keluhan utama sesak napas sejak 3 hari sebelum masuk rumah sakit, disertai nyeri perut, mual, muntah, dan berat badan susah naik beberapa bulan ini. Pasien memiliki gangguan jantung PJB asianotik ASD secundum dan VSD subaortic berdasarkan hasil echocardiography. Pemeriksaan fisik ditemukan penurunan saturasi, murmur holosistolik, dan nyeri tekan abdomen regio kanan dan kiri. Selama perawatan sesak semakin bertambah. Selama perawatan ditemukan leukositosis, neutrofilia, peningkatan NLR test, peningkatan PPT, penurunan APTT, hiponatremia, hipoksemia, hipoksia, alkalosis respiratorik, dan tes PCR positif Covid-19. Gambaran rontgen dada menunjukkan pneumonia dextra dan kardiomegali. Pasien dipasang ventilator mekanik selama perawatan di ruang isolasi PICU. Setelah 2 hari perawatan pasien mengalami henti jantung hingga meninggal. Anak dengan komorbid dan peningkatan CRP yang terinfeksi COVID-19 menyebabkan gejala yang berat dan serius. Telah dilaporkan seorang anak perempuan 13 tahun 6 bulan yang meninggal karena COVID-19. Meskipun pandemik COVID-19 sudah mereda, perlu meningkatkan kewaspadaan yang tinggi terhadap virus maupun bakteri lain terutama bagi populasi yang rentan terinfeksi.
