Abstract

Work is often the reason for mothers not to have the opportunity to give exclusively breastfeeding for 6 months to their babies. This reason is motivated by the lack of sufficient time to breastfeed or express milk. This is also experienced by breastfeeding mothers in the Klaten region who already have regulations on exclusive breastfeeding. This study aims to determine the large percentage of exclusive breastfeeding for working mothers in Klaten district. This study uses a categorical descriptive method involving 117 respondents. Primary data were collected from the midwife clinics of Ni'ma and Warsiti, the Belang Wetan posyandu, and several TKs in Klaten, which were included in the inclusion criteria. A total of 63 mothers (54%) who worked successfully gave exclusive breastfeeding to their children, while unsuccessful were 54 mothers (46%). The success rate of exclusive breastfeeding in the age group of 17-25 years was 14 subjects (11.97%), at the age of 26-35 years was 44 (37.61%) and at the age of 36-45 was 5 (4.27%). The educational background of successful working mothers is D4 / S1 of 21 subjects (17.95%). The occupational groups that succeeded in providing exclusive breastfeeding were Teachers (54.5%), Entrepreneurs (75%), Civil Servants (80%), BUMN Employees (66.7%), and Health Workers (90.9%). The percentage of success of exclusive breastfeeding for mothers who worked for ≤ 8 hours amounted to 57.1%, while mothers who worked> 8 hours amounted to 36.8%. The higher the level of mother's education, the higher the success rate of exclusive breastfeeding. The longer the mother works, the lower the success rate of exclusive breastfeeding.

Abstract

Background : The use of electronic medical records (EMR), including cloud computing-based EMR, is widely increasing. However, data regarding the perception of physicians using cloud computing-based EMR in Indonesia is still limited. Objective: This study aims to determine the perception of clinical doctors in using EMR based on cloud computing. Methods: This is a cross sectional study with a mixed-methods explanatory sequential design approach to determine the perception of clinicians in using cloud computing-based EMR. Qualitative analysis is used to support the depth of quantitative data analysis. Result: There are 10 private clinic doctors who participate. The average value of respondents' satisfaction after using cloud computing-based EMR is 7.4 from a scale of 1-10, with a median value of 8. In the analysis of interest in using cloud computing-based EMR in the long term, it was found that 30% of respondents said "yes", and 70% said "maybe". Qualitative data show that the problems in using EMR are the high price of the EMR system, security and privacy, the display is still limited, and the need for a stable internet network. Conclusion: The use of cloud computing-based EMR by clinicians still meet a number of obstacles. Abstract

Introduction: Chronic energy deficiency (CED) significantly impacts maternal health and child development. Socioeconomic conditions and access to health services cause young mothers in rural areas to have different health problems from those in urban areas. The study aimed to analyze the factors associated with CED among young pregnant women in rural areas in Indonesia. Methods: Secondary data for this cross-sectional study were obtained from the 2018 Basic Health Research Survey. One thousand one hundred and thirty-nine pregnant women aged <25 years were involved as subjects. Age, number of family members, education, occupation, gravida, abortion, supplementary feeding, and iron tablets during pregnancy were included as independent variables. Binomial logistic regression analysis was performed to measure the association between the independent and dependent variables. The strength of the association was expressed by p-value <0.05. Results: The three factors associated with CED were age (p=0.001), education (p=0.013), and supplementary feeding (p<0.001). Conclusions: Equitable education, increasing the age at which moms can become pregnant, and efforts to fulfill the nutrition needs of pregnant women by providing supplemental food based on community empowerment are significant. Family and community involvement will ensure the sustainability of this CED prevention program.

Abstract

Indonesia has the second-highest prevalence of low birth weight (LBW) babies in ASEAN countries. Various factors contribute to this high incidence. This study examined the association between gestational weight gain (GWG), pre-pregnancy body mass index (BMI), and antenatal care (ANC) visits frequency towards LBW in a region of Indonesia with a high prevalence of LBW. This is an analytical observational study using cross-sectional design with utilizing medical records data, conducted in June-August 2022. We analyzed 78 medical records of pregnant women who gave term-singleton birth. Main independent variables were gestational weight gain, pre-pregnancy BMI, and ANC frequency. Data analysis was performed using statistical package program, including descriptive statistics, bivariate and multivariate analyses to explore associations between those factors and LBW occurrence. The mean of infant birth weight was 3115.4± 465.2 grams. Low birth weight proportion was 11.5%. More than 90% of subjects follow frequency of ANC visits for K1 and K4, but only a third of participants meet K6 ANC visit recommendation. Approximately half of the subjects (48.7%) had inadequate GWG. Bivariate analysis revealed a strong association between insufficient GWG and LBW (OR = 10.4, 95% CI = 1.233–87.74, p=0.013). However, pre-pregnancy BMI and ANC frequency showed no association with LBW. Inadequate gestational weight gain is significantly linked to an elevated risk of LBW. Emphasizing the appropriate quantity of ANC visits and regular monitoring of maternal weight gain throughout pregnancy is crucial to mitigate the risk of LBW.

Abstract

Anak sebagai salah satu populasi beresiko tinggi terinfeksi COVID-19. Kasus rawat inap COVID-19 pada anak di Indonesia terus meningkat dan lebih besar persentasenya dibanding negara lain. Anak-anak dengan penyakit komorbid dapat memperparah gejala COVID-19 hingga menyebabkan kematian. Tujuan penulisan artikel ini adalah untuk melaporkan kasus Pneumonia COVID-19 disertai dengan Penyakit Jantung Bawaan (PJB) asianotik untuk mendiskusikan kemungkinan diagnosis banding lain sebagai pemahaman dalam melakukan pengelolaan pasien dengan lebih optimal. Anak perempuan 13 tahun 6 bulan dirujuk ke Rumah Sakit Akademik Universitas Gadjah Mada dengan keluhan utama sesak napas sejak 3 hari sebelum masuk rumah sakit, disertai nyeri perut, mual, muntah, dan berat badan susah naik beberapa bulan ini. Pasien memiliki gangguan jantung PJB asianotik ASD secundum dan VSD subaortic berdasarkan hasil echocardiography. Pemeriksaan fisik ditemukan penurunan saturasi, murmur holosistolik, dan nyeri tekan abdomen regio kanan dan kiri. Selama perawatan sesak semakin bertambah. Selama perawatan ditemukan leukositosis, neutrofilia, peningkatan NLR test, peningkatan PPT, penurunan APTT, hiponatremia, hipoksemia, hipoksia, alkalosis respiratorik, dan tes PCR positif Covid-19. Gambaran rontgen dada menunjukkan pneumonia dextra dan kardiomegali. Pasien dipasang ventilator mekanik selama perawatan di ruang isolasi PICU. Setelah 2 hari perawatan pasien mengalami henti jantung hingga meninggal. Anak dengan komorbid dan peningkatan CRP yang terinfeksi COVID-19 menyebabkan gejala yang berat dan serius. Telah dilaporkan seorang anak perempuan 13 tahun 6 bulan yang meninggal karena COVID-19. Meskipun pandemik COVID-19 sudah mereda, perlu meningkatkan kewaspadaan yang tinggi terhadap virus maupun bakteri lain terutama bagi populasi yang rentan terinfeksi.
