Abstract

Patients with breast cancer at the terminal stage often experience a decrease in quality of life and problems in psychological well-being. Management of patients with terminal illnesses requires end-of-life care, which is comprehensive care for chronic diseases that are nearing death, which involves multidisciplinary collaboration of health workers with the main focus of care being a peaceful death. The aim of this research is to develop empowerment-based palliative care to improve psychological wellbeing and quality of life of breast cancer patients. Explanatory research with a cross sectional approach to develop a model. The sample of this study were 125 breast cancer patients. The variables in this study consisted of self- management, delivery systems, decision support, clinical information, community resources, palliative care, empowerment, quality of life and spiritual wellbeing which were measured using a modified questionnaire whose validity and reliability were measured. The data were then analyzed using Smart PLS and used in formulating strategic issues for focus group discussions and expert consultations. The output of this study was an empowerment-based palliative care module on the quality of life and psychological well-being of breast cancer patients as well as scientific publications in the Scopus International journal, a module that already prepared will be arranged in the ISBN and Intellectual Property Rights

Abstract

Compliance with hand hygiene activities is an important aspect in maintaining patient health, preventing the spread of nosocomial infections, and improving nursing clinical practice standards. This study aims to analyze the level of compliance with the implementation of hand hygiene activities by nurses in several inpatient service units. This research method uses a quantitative descriptive approach involving a sample of nurses from several inpatient rooms. Data was collected through direct observation of the practice of 5 moments of hand hygiene as well as a questionnaire regarding compliance procedures for implementing hand hygiene activities. The results of the study showed that the level of compliance with the implementation of the 5 moments hand hygiene activities among inpatient room nurses was 94% for the moments before carrying out aseptic procedures, with almost all service units achieving a high level of compliance. Meanwhile, the moment before contact with the patient was 87%. Factors that influence compliance include knowledge, attitudes, accessibility of hand hygiene facilities, available time, and organizational culture. This research provides deeper insight into the obstacles faced by nurses in maintaining compliance with the implementation of hand hygiene activities. The results of this research can be used as a basis for developing interventions aimed at increasing and maintaining compliance with the implementation of hand hygiene activities, including training, supervision and facility improvements

Abstract

Diabetic ulcer is lower extremity illness in diabetic patients with sensory, motoric, autonomic neuropathy, and or limbs blood vessel disorder as the characteristics. Patients with diabetic ulcer often experience psychological stress as the result of the infection of the ulcer itself or the operation procedure, thus negatively affects patients’ self-concept, self-worth, quality of life, physiological and psychological health, hope to recover, and their spirituality. This research aimed to recognize the effect of family empowerment method modern dressing based in inflating diabetic ulcer patients’ quality of life. This research utilized pre-experimental with one group pretest-posttest design. The population and sample in this research are diabetic ulcer patients. The sampling technique used was purposive sampling with the total of 53 respondents. The instrument used was WHOQOL (The World Health Organization of Quality of Life). The research found that patient’s QoL before family empowerment was given, the moderate QoL (Quality of Life) was 41 respondents (77.4%) and the good QoL was 12 respondents (22.6%). After the treatment using the aforementioned method, the good QoL increased to 28 respondents (52.8%), the moderate QoL became 7 respondents (13.2%) and the very good QoL increased as much as 18 respondents (34%). Wilcoxon analysis test resulted ρ 0,000, meaning that there was an effect of family empowerment method modern dressing based to patients with diabetic ulcer

Abstract

Background: Hemodynamic instability is a major predictor of organ failure and mortality in ICU patients. Conventional monitoring often fails to detect early deterioration, which has encouraged the use of artificial intelligence (AI) to improve the detection and prediction of hemodynamic instability. Methods: This systematic review followed the PRISMA 2020 guidelines and analyzed studies using machine learning or deep learning to predict hypotension, vasopressor requirements, or hemodynamic instability in adult ICU patients. Six major databases were screened, and 16 studies met the inclusion criteria. Due to heterogeneity in model design and outcomes, the findings were synthesized narratively. Results: The included studies comprised retrospective model development, multicenter validation, prospective evaluation, and two randomized clinical trials. Multivariable models such as the hemodynamic stability index (HSI) demonstrated strong predictive performance (AUROC 0.76–0.90). Dynamic models such as TvHEWS consistently provided stable predictions with reduced false alarms. Waveform-based predictors, including the hypotension prediction index (HPI), were able to anticipate hypotension 5–15 minutes before onset, even in patients with sepsis. Personalized approaches, such as DynaCEL and HM-TARGET, generated patient-specific hemodynamic targets. Prospective studies showed a reduction in the duration of hypotension, although evidence regarding effects on mortality and organ failure remains limited. Conclusion: Artificial intelligence has the potential to improve the accuracy of hemodynamic monitoring and enable earlier intervention in the ICU. However, large-scale clinical trials are still needed to confirm its benefits on meaningful clinical outcomes.
