Abstract

Based on observations in the medical record filling unit of the Cikalong Wetan Regional General Hospital, the authors found that the missfiled medical record document occurred due to the negligence of the filling officer in the process of storing medical record documents and the hospital information system was still in the process of being developed and the facilities and infrastructure in the filling had not yet been completed. complete so that it hampers the distribution process of medical record documents. This study aims to determine the incidence of missfiles in the medical record filling unit of the Cikalong Wetan Regional General Hospital. Based on the research that has been carried out, there are 26 medical record documents from 631 files that were examined, caused by the numbering system not according to SOPs, filing racks, document folders, expedition books that were still filled manually without using a tracer in the filling storage system, and human resources for filling officers who are inexperienced and lack discipline and responsibility in carrying out their duties as medical recorders.

Abstract

This concerns the return of claim files from the Social Security Administering Body (BPJS) to the hospital. Problems were found in the Participant Eligibility Letter (SEP) for inpatient care, which was not issued by the BPJS. There was a delay in the issuance or claiming of the SEP by the BPJS from January to April 2023. The research method used was a descriptive method with a quantitative approach. The population consisted of all BPJS inpatient claim files from the last three months at Cikalongwetan Regional General Hospital, totaling 251 files. The sampling technique used was total sampling, with 251 samples. Data collection techniques included observation, interviews, and documentation studies. Data processing techniques were editing and tabulation. Research instruments included stationery, a stopwatch, interview guidelines, a calculator, and a checklist sheet. The research results indicate that Cikalongwetan Regional General Hospital does not yet have written Standard Operating Procedures (SOPs) regarding the implementation of BPJS claims. From the sample data of returned inpatient BPJS claims, totaling 251 files from May to July 2024, in May, the criterion for illegible doctor's handwriting was 98 (36.6%). Therefore, the author suggests regular training and education for medical staff and routine supervision to ensure compliance with Standard Operating Procedures (SOPs).

Abstract

This study is entitled Review of Medical Information Release Services and Filling in Raharja Service Claim Files for Traffic Accident Victims at Al Ihsan Hospital, Bandung Regency, West Java Province. The purpose of this study is to determine the Medical Information Release Service and Filling in Jasa Raharja Claim Files for Traffic Accident Victims at Al Ihsan Hospital, Bandung Regency, West Java Province. The method used in this study is: using a descriptive method with a qualitative approach. The subject of this study is the Medical Information Release Service and Filling in the Raharja Service Claim File at Al-Ihsan Hospital, Bandung Regency, West Java Province. The object of this study is the standard operating procedure for releasing medical information and Jasa Raharja claim forms for traffic accident victims who claim Jasa Raharja insurance at RSUD AlIhsan Bandung Regency. Data collection techniques by observation, interview and documentation. Research results: medical information release services at Al-Ihsan Hospital are in accordance with SOPs. There is no written procedure for filling Jasa Raharja's claim file but the implementation is in accordance with the SOP.

Abstract

The duplication that occurs is only one patient with more than one medical record number but there is also duplication in the form of one medical record number. Based on observations in the medical record unit of the Cikalong Wetan Regional General Hospital in the filling and registration section, duplicate medical record numbering was found, this occurred because the patient did not carry a medical card, the hospital system was disrupted, filling and discontinuity of the contents of the patient’s medical record. The purpose of this study was to determine the cause of duplication of medical record numbering at the Cikalong Wetan Regional General Hospital. The research method used is a descriptive method with a qualitative approach. Data collection techniques are interview, observation and documentation studies. Non-probability sampling is used. The results of the research using the triangulation method were, the patient did not carry a medical card, the hospital system was disrupted, officer discipline, education background, the expedition book was still manual, the filling section was one of the factor causing duplication of medical numbering. The author suggests conducting discussion, education ang training and providing Computerized Patient Record (CPR) facilities in the filling section.
