Abstract

Pemberdayaan staf keperawatan merupakan komponen penting dalam peningkatan mutu layanan rumah sakit, namun masih menghadapi berbagai tantangan seperti lemahnya infrastruktur organisasi, kurangnya dukungan institusi, dan rendahnya keterlibatan perawat dalam pengambilan keputusan. Kondisi tersebut berdampak pada motivasi, kinerja, serta keselamatan pasien, sehingga kajian ilmiah mengenai efektivitas pemberdayaan menjadi semakin mendesak. Penelitian ini bertujuan menganalisis secara sistematis pengaruh pemberdayaan struktural dan psikologis terhadap kinerja pelayanan keperawatan, serta mengidentifikasi dimensi pemberdayaan dan intervensi yang terbukti efektif dalam meningkatkan kualitas layanan. Metode yang digunakan adalah Systematic Literature Review (SLR) berbasis pedoman PRISMA 2020 dengan menelaah 30 artikel terbitan 2021–2025 terkait pemberdayaan struktural, pemberdayaan psikologis, dan kinerja keperawatan. Hasil analisis menunjukkan bahwa pemberdayaan struktural berpengaruh signifikan terhadap motivasi kerja (p=0,000) dan kinerja perawat (p=0,016), sementara pemberdayaan psikologis meningkatkan keterlibatan kerja dengan koefisien 0,487 (p<0,05). Employee Empowerment dan Knowledge Management menjelaskan 41,9% variasi kinerja pegawai. Intervensi coaching meningkatkan skor kinerja dari 1,68 menjadi 2,94 (p=0,005) dan pelatihan Patient Centered Care meningkatkan pengetahuan perawat hingga 95,7%. Secara keseluruhan, temuan menegaskan bahwa pemberdayaan struktural dan psikologis berperan penting dalam meningkatkan motivasi, keterlibatan, dan kinerja keperawatan, dengan pelatihan terstruktur sebagai strategi pendukung yang efektif.   Nurse empowerment is a critical component in improving hospital service quality; however, it continues to face challenges such as weak organizational infrastructure, limited institutional support, and low nurse involvement in decision-making. These conditions affect motivation, performance, and patient safety, making research on effective empowerment strategies increasingly urgent. This study aims to systematically analyze the influence of structural and psychological empowerment on nursing service performance and to identify empowerment dimensions and interventions proven effective in enhancing service quality. A Systematic Literature Review (SLR) guided by PRISMA 2020 was conducted by reviewing 30 articles published between 2021 and 2025 addressing structural empowerment, psychological empowerment, and nursing performance. Findings indicate that structural empowerment significantly improves work motivation (p=0.000) and nurse performance (p=0.016), while psychological empowerment enhances work engagement with a coefficient of 0.487 (p<0.05). Employee Empowerment and Knowledge Management explain 41.9% of performance variation. Coaching interventions increased performance scores from 1.68 to 2.94 (p=0.005), and Patient-Centered Care training improved nurses’ knowledge by 95.7%. Overall, the review confirms that structural and psychological empowerment substantially improve motivation, engagement, and nursing performance, with structured training serving as an effective supporting strategy.

Abstract

Bronchial asthma is a chronic inflammatory disease of the respiratory tract characterized by recurrent episodes of dyspnea, wheezing, and coughing. Asthma management requires a comprehensive approach including pharmacological and non-pharmacological therapy. Pursed lip breathing as part of mind-body therapy can help improve breathing control and reduce anxiety in asthma patients. This case study aims to evaluate the effectiveness of pursed lip breathing application in improving breathing patterns and reducing anxiety levels in bronchial asthma patients. This research design is a descriptive case study of a 64-year-old female patient with bronchial asthma. Pursed lip breathing intervention was performed for 3 x 24 hours with evaluation of respiratory rate (RR), oxygen saturation (SpO₂), wheezing, anxiety level using Hamilton Anxiety Rating Scale (HARS), and sleep quality. Data were collected through physical assessment, observation, and interviews. After 3 days of intervention, there were significant improvements in all measured parameters. Respiratory rate decreased from 28 breaths/minute to 20 breaths/minute, oxygen saturation increased from 93% to 97%, wheezing disappeared, anxiety level decreased from moderate category (score 18) to mild (score 8), and sleep quality improved from 4-5 hours/night to 7-8 hours/night. The patient was able to perform pursed lip breathing technique independently on the third day. Pursed lip breathing is effective as an independent nursing intervention in improving breathing patterns and reducing anxiety in bronchial asthma patients. This technique can be integrated into holistic nursing care as part of asthma patient self-care management.
