Abstract

Adherence to medication in elderly patients is crucial to support their health, as many suffer from chronic non-communicable diseases requiring long-term treatment. The complexity and volume of medications can lead to patient fatigue. Identifying factors that affect adherence is key to addressing these challenges. This study aims to identify the factors influencing medication compliance among elderly patients at PKU Muhammadiyah Gamping Hospital’s outpatient clinic. This quantitative, cross-sectional study involved 80 elderly patients at the internal medicine clinic, selected through accidental sampling. Data was collected using the MMAS-8 (Modified Morisky Adherence Scale-8) and additional questionnaires to determine the factors influencing therapy adherence, analyzed through multiple linear regression. Results showed that compliance was low in 21.3% of patients, moderate in 43.8%, and high in 35.0%. Statistical analysis found significant influences from education level (p<0.05), illness duration (p<0.05), knowledge level (p<0.05), health service access (p<0.05), family support (p<0.05), healthcare workers’ role (p=0.029), and treatment motivation (p<0.05). Compliance among elderly patients at PKU Muhammadiyah Gamping Hospital falls in the moderate category, with these factors explaining 75.6% of adherence levels.

Abstract

The Ministry of Industry of the Republic of Indonesia has emphasized the importance of cosmetics as an essential need due to the increasing awareness of the general population. Various values, including functional, conditional, emotional, social, epistemic, and religious, influence individuals in selecting cosmetics, particularly halal cosmetics, given Indonesia's predominantly Muslim population. This study aims to identify the factors influencing the adoption of halal cosmetics among students of the Faculty of Engineering at Muhammadiyah University of Yogyakarta. This research employs a non-experimental quantitative design using a survey method with a cross-sectional approach and a questionnaire as the primary instrument. The sample consists of 137 students who use halal cosmetics, selected through purposive sampling based on inclusion criteria, namely active students who have used or are currently using halal cosmetics. Data analysis was conducted using the Partial Least Squares Structural Equation Modeling (PLS-SEM) approach. The PLS-SEM analysis revealed that functional, conditional, emotional, and epistemic values have a positive and statistically significant influence on the usage of halal cosmetics, with p-values of 0.089, 0.001, 0.001, and 0.004, respectively. In contrast, social and religious values showed no significant relationship with halal cosmetics usage, with p-values of 0.706 and 0.374, respectively. Therefore, this study concludes that the adoption of halal cosmetics among students of the Faculty of Engineering at Muhammadiyah University of Yogyakarta is primarily driven by functional, conditional, emotional, and epistemic values rather than social or religious values.

Abstract

The American Society of Health-System Pharmacists (ASHP) 1999 listed guidelines for providing stress ulcer prophylaxis (SUP) therapy as the standard of care for critical patients in the ICU. However, numerous studies have shown that most patients admitted to non-intensive care units continue to receive acid suppressant prescriptions, without appropriate indications or risk factors. This can lead to increased morbidity and increased patient care costs. This study aimed to determine the utilization profile of stress ulcer prophylaxis, its appropriateness, and the costs incurred by patients for SUP. This was an observational study with a cross-sectional design based on medical records. The study participants were inpatients from the Internal Medicine Polyclinic at Wates Hospital who met the inclusion criteria from January to December 2021. The study revealed that the H2 receptor antagonist (H2RA), proton pump inhibitor (PPI), sucralfate, and antacid groups received 57.9 %, 35.7%, 6.1%, and 0.3% of preventive drugs, respectively. Of the 215 patients, 76 received appropriate indications, whereas 139 received inappropriate indications based on the guidelines. During the study period, the cost of using stress ulcer prophylaxis for patients with appropriate indications was IDR 15,344,812 (76%), whereas the cost of using inappropriate stress ulcer prophylaxis was IDR 5,965,572 (28%) of the total cost. Factors such as the length of stay (LOS), type and cost of each medication, and frequency of medication administration can influence the high cost of inappropriately indicated patients. Abstract

Gastritis is a common gastrointestinal disorder that affects global population, including Indonesia. Many individuals choose self-medication as a primary approach to manage gastritis symptoms. Inappropriate self-medication practices can lead to worsening symptoms and complications. Understanding the knowledge level is essential to improve self-medication safety. This study aims to assess the level of community knowledge regarding gastritis self-medication in Indonesia. An observational study with cross-sectional design conducted among Indonesia community. Data were collected through an e-questionnaire, which was distributed across 34 provinces. The instrument in this study has been tested for validity and reliability using Content Validity, Face Validity, and Cronbach's Alpha tests. Descriptive statistical analysis was performed using SPSS to summarize the characteristics of the respondents and to categorize knowledge levels into low, moderate, and high. The results of this study indicated that 921 respondents (78.1%) had a high knowledge level, 247 respondents (21%) had a moderate knowledge level, and 11 respondents (0.9%) had a low knowledge level of gastritis self-medication. In conclusion, knowledge level of Indonesian community regarding gastritis self-medication is categorized as high. Abstract

Chronic diseases are a major global health challenge and remain a leading cause of morbidity and mortality in Indonesia. Sustained adherence to long-term pharmacological therapy is essential but is often suboptimal. This study aimed to analyze the determinants of medication adherence among patients with chronic diseases at RSUD Kota Yogyakarta using structural equation modeling-partial least squares (SEM-PLS). A cross-sectional survey was conducted between January and February 2025 with 120 purposively selected outpatients. Medication adherence was assessed using the Adherence to Refills and Medications Scale (ARMS) and additional questionnaires measuring therapeutic regimens, patient perceptions, socioeconomic factors, and healthcare system attributes. Data were analyzed using SmartPLS 4. The results showed that 74.2% of patients had low adherence and 25.8% had moderate adherence, with none achieving high adherence. SEM-PLS analysis indicated that therapy complexity (?=0.262, p<0.01), patient perception (?=0.381, p<0.01), socioeconomic status (?=0.265, p<0.01), and healthcare system support (?=0.199, p<0.01) were significant predictors, explaining 66% of adherence variance (R²=0.660). In conclusion, medication adherence among patients with chronic diseases in Yogyakarta was low and was strongly influenced by regimen burden, perception, socioeconomic status, and healthcare support. Targeted interventions addressing these factors are essential for improving adherence and long-term health outcomes.  Keywords: chronic disease, therapy adherence, ARMS, SEM-PLS, Yogyakarta

Abstract

Guidelines from the American Society of Health-System Pharmacists (ASHP) 1999 prohibit acid-suppressing therapy for stress ulcer prophylaxis (SUP) in patients who are not critically ill. Stress ulcer prophylaxis is not recommended in non-ICU patients with <2 risk factors. Inappropriate use of SUP can increase costs for patients. This study aims to evaluate the use and the cost of SUP. This research was a non-experimental observational study with a cross-sectional approach. Data was collected retrospectively using the consecutive sampling method with a random sampling technique on the medical records of inpatients in the internal medicine ward of Sleman Regional Public Hospital from January to December 2020, totaling 340 samples. The results showed that proton pump inhibitors were the most widely used acid-suppressing drugs, with 45.8%. Furthermore, the histamine-2 receptor antagonist was 42.6%, the sucralfate group was 7.4%, and the antacid group was 4.2%. Of 340 patients, 57 (16.8%) were in the proper indication based on the guidelines, and 283 (83.2%) were under the wrong indication for SUP. They were using SUP with the proper indication so that the therapy could save treatment costs by Rp. 19,933,582. There was a high prevalence of inappropriate SUP prescriptions among inpatients in the internal medicine department; if these drugs were given with the appropriate indications, they could save more on the prophylaxis cost. Clinician pharmacists should develop an effective intervention strategy to reduce inappropriate SUP drugs.
