The implementation of Islamic management in healthcare facilities, particularly government-owned hospitals, is becoming increasingly relevant as the public demand for services that are not only professional but also align with religious values. The purpose of this study is to explore the implementation of Islamic management in regional public hospitals in Indonesia and assess its impact on service quality, patient satisfaction, and implementation challenges. This study is a systematic literature review based on the PRISMA 2020 guidelines and the Cochrane Handbook. A literature search was conducted in Google Scholar, PubMed, Scopus, and Garuda using the keywords "sharia management," "Islamic service," and "RSUD." Inclusion criteria included original research articles discussing the implementation of Islamic management in regional public hospitals in Indonesia. Of the 119 articles identified, 8 met the criteria for analysis. The study results indicate that the implementation of Islamic management in regional public hospitals has been shown to improve patient satisfaction, strengthen motivation for healing through spiritual guidance, and support the strengthening of sharia-based hospital governance. Key success factors include the readiness of healthcare workers, the integration of spiritual aspects into standard operating procedures, and support for hospital policies. The existing obstacles include limited trained human resources, inconsistent operational standards, and limited Islamic facilities. In conclusion, Islamic management has proven relevant in improving service quality and patient satisfaction.Keywords: Islamic management; regional general hospitals; Islamic healthcare; patient satisfaction ABSTRAK Implementasi manajemen Islami di fasilitas kesehatan, khususnya rumah sakit milik pemerintah semakin relevan seiring meningkatnya kebutuhan masyarakat akan pelayanan yang tidak hanya professional tetapi juga sesuai dengan nilai-nilai religius. Tujuan studi ini adalah mengeksplorasi penerapan manajemen Islami pada rumah sakit umum daerah di Indonesia dan menilai dampaknya terhadap mutu pelayanan, kepuasan pasien, serta tantangan implementasi. Studi ini merupakan systematic literature review berdasarkan pedoman PRISMA 2020 dan Cochrane Handbook. Pencarian literatur dilakukan di Google Scholar, PubMed, Scopus, dan Garuda dengan kata kunci “manajemen syariah”, “pelayanan Islami”, dan “RSUD”. Kriteria inklusi mencakup artikel penelitian asli yang membahas implementasi manajemen Islami di rumah sakit umum daerah di Indonesia. Dari 119 artikel yang diidentifikasi, 8 di antaranya memenuhi kriteria untuk dianalisis. Hasil studi menunjukkan bahwa implementasi manajemen Islami di rumah sakit umum daerah terbukti meningkatkan kepuasan pasien, memperkuat motivasi kesembuhan melalui bimbingan rohani, dan mendukung penguatan tata kelola rumah sakit berbasis syariah. Faktor keberhasilan utama meliputi kesiapan tenaga kesehatan, integrasi aspek spiritual dalam standard operating procedures, dan dukungan kebijakan rumah sakit. Kendala yang ada adalah keterbatasan sumberdaya manusia terlatih, standar operasional yang tidak konsisten, dan keterbatasan fasilitas Islami.
