Abstract

Emergency ambulance is an emergency service carried out outside the hospital, both trauma and non-trauma, which is carried out to prevent death and further disability. This service is carried out by increasing the ambulance response time. Emergency ambulance response time in pre-hospital emergency services can impact the assessment of patient triage levels in the development of modern triage, one of which is the Australasian Triage Scale (ATS). This study aims to determine the relationship between emergency ambulance response time and the Australasian Triage Scale (ATS) triage level in non-trauma case patients in Samarinda. The design of this study used a quantitative method, namely descriptive correlation, using a cross-sectional research design. The population in this study was 78 patients. Sampling used the total sampling technique. The instruments used in this study were observation sheets, stopwatches (response time), and the Australasian Triage Scale (ATS). This study was conducted for 3 months from March to May in Public Hospital Samarinda in 2024. Meanwhile, the person correlation test results found a p-value of 0.000<0.05, which means there is a relationship between the emergency ambulance response time to ATS Triage and an r-value of 0.767 where the strength of the relationship is high/strong. With a positive correlation result, the direction of the relationship is in the same direction. This means that the faster the emergency ambulance response time, the better (lower) the emergency condition of non-trauma patients based on the ATS triage level.

Abstract

Hipertensi telah menjadi masalah kesehatan global yang prevalensinya terus meningkat, terutama pada populasi lansia. Faktor psikologis seperti kecemasan sering kali kurang mendapat perhatian dalam penatalaksanaan hipertensi, padahal berbagai penelitian menunjukkan adanya keterkaitan antara kondisi psikologis dengan peningkatan tekanan darah. Di Kelurahan Rawa Makmur, wilayah kerja Puskesmas Palaran Samarinda, kasus hipertensi menempati posisi tertinggi dengan 5.638 penderita pada tahun 2023. Studi pendahuluan menunjukkan bahwa 40% lansia hipertensi di wilayah tersebut mengalami gejala kecemasan, yang diduga berkontribusi terhadap peningkatan derajat hipertensi. Penelitian ini bertujuan untuk menganalisis hubungan antara tingkat kecemasan dengan derajat hipertensi pada lansia di Kelurahan Rawa Makmur wilayah Puskesmas Palaran Samarinda. Metode penelitian yang digunakan adalah deskriptif korelasional dengan pendekatan cross-sectional. Dari 135 responden, ditemukan bahwa mayoritas lansia mengalami hipertensi derajat 1 dan memiliki tingkat kecemasan yang bervariasi. Hasil analisis menunjukkan adanya hubungan positif yang signifikan antara tingkat kecemasan dan derajat hipertensi (p = 0,003; r = 0,253). Temuan ini menegaskan pentingnya pendekatan holistik dalam manajemen hipertensi lansia yang tidak hanya berfokus pada aspek fisik, tetapi juga memperhatikan kondisi psikologis seperti kecemasan. Intervensi yang mengintegrasikan skrining kesehatan mental dan terapi psikososial direkomendasikan untuk mengoptimalkan pengendalian tekanan darah pada populasi ini.

Abstract

Cedera pada pelajar SMA sering terjadi akibat aktivitas fisik seperti olahraga dan kegiatan ekstrakurikuler. Penanganan yang tepat sangat penting untuk mencegah komplikasi. Penelitian ini bertujuan untuk mengevaluasi efektivitas pendidikan kesehatan dalam meningkatkan pengetahuan pelajar tentang pencegahan dan pertolongan pertama pada cedera. Metode yang digunakan adalah ceramah, diskusi, pretest, posttest, dan praktik. Peserta terdiri dari 40 siswa kelas 10 hingga 12 pada salah satu SMA Negeri di Samarinda. Program penyuluhan mengusung materi "Pencegahan dan Pertolongan Pertama Pada Cedera Kecelakaan", dengan menggunakan media powerpoint, video edukasi, booklet, dan simulasi. Hasil evaluasi menunjukkan peningkatan pengetahuan yang signifikan, tercermin dari peningkatan nilai rata-rata pretest sebesar 53 menjadi 84,75 pada posttest. Berdasarkan hasil ini, dapat disimpulkan bahwa pendidikan kesehatan efektif dalam meningkatkan pengetahuan pelajar mengenai pertolongan pertama pada cedera. Program ini diharapkan dapat meningkatkan kesadaran dan keterampilan pelajar dalam menangani cedera di lingkungan sekolah.

Abstract

Anxiety is one of the most common psychological responses experienced by patients with ST-Elevation Myocardial Infarction (STEMI) during hospitalization in the Intensive Cardiac Care Unit (ICCU), which may affect physiological stability and the recovery process. Therefore, supportive non-pharmacological nursing interventions are needed to help manage anxiety. This study aimed to analyze nursing practice through the application of the Spiritual Emotional Freedom Technique (SEFT) and PC6/Neiguan acupressure on anxiety levels in patients with STEMI. A case analysis was conducted on two patients with STEMI in the ICCU using a nursing care approach. The first patient received standard care combined with SEFT and PC6/Neiguan acupressure interventions, while the second patient received standard care only. Anxiety levels were measured using the State–Trait Anxiety Inventory (STAI). The patient who received SEFT and PC6/Neiguan acupressure experienced a reduction in anxiety level from moderate anxiety to no anxiety, while the patient who received standard care only did not show any reduction in anxiety level and remained in the moderate anxiety category. The application of SEFT and PC6/Neiguan acupressure in nursing practice was associated with reduced anxiety levels in patients with STEMI and may serve as supportive nursing therapies within a holistic nursing care approach for cardiovascular patients

Abstract

ST-Elevation Myocardial Infarction (STEMI) is part of Acute Coronary Syndrome (ACS), a cardiovascular condition in which a coronary artery is completely occluded, preventing oxygen from reaching the heart muscle. This study aimed to determine the effect of Swedish massage therapy combined with warm water on hemodynamic stability. This study employed a case study design comparing an intervention patient and a control patient diagnosed with STEMI who had unstable hemodynamic status in the ICCU. After the implementation of Swedish massage and warm water therapy in the intervention patient for three consecutive days, the patient’s hemodynamic parameters such as blood pressure, heart rate, mean arterial pressure, and respiratory rate remained within stable limits, showing no significant increases or decreases. This outcome differed from the control patient, who received only pharmacological therapy. The analysis deminstated a difference in hemodynamic outcomes between patients who received the combined intervention and those in the control group.

Abstract

Non-ST Elevation Myocardial Infarction (NSTEMI) is often accompanied by increased blood pressure due to pain and anxiety, which trigger activation of sympathetic nervous system, thereby increasing cardiac workload and worsening myocardial ischemia. In addition to pharmacological therapy, non- pharmacological interventions are needed to help stabilize patients' hemodynamic status. This study aimed to analyze the implementation of autogenic relaxation combined with nature sound music therapy on blood pressure in patients with NSTEMI. A case study was conducted on one NSTEMI patient in the Intensive Cardiac Care Unit (ICCU), where blood pressure was measured before and after intervention. The findings showed that the patient who received autogenic relaxation combined with nature sound music therapy experienced a decrease in both systolic and diastolic blood pressure and a more relaxed condition. In conclusion, autogenic relaxation combined with nature sound music therapy is effective as a supportive non-pharmacological intervention in stabilizing blood pressure in patients with NSTEMI.

Abstract

Ambulance response time is a key performance indicator of Emergency Medical Services (EMS) and is essential for early patient stabilization. However, evidence on factors associated with pre-hospital clinical improvement, particularly changes in the Australasian Triage Scale (ATS), remains limited in developing countries. This study aimed to identify factors associated with pre-hospital clinical improvement among non-trauma emergency patients. A cross-sectional analytical study was conducted involving 113 non-trauma emergency patients transported by ambulance to a tertiary referral hospital in Samarinda, East Kalimantan, Indonesia. ATS improvement was defined as a positive shift to a lower-acuity category between the initial assessment at the emergency scene and reassessment at the emergency department. Data were analyzed using multivariate logistic regression controlling for age, transport distance, diagnosis, and pre-hospital interventions. Ambulance response time of ≤15 minutes was the strongest factor associated with ATS improvement (p < 0.001; adjusted OR = 0.019; 95% CI = 0.003–0.122). Circulation management during pre-hospital care also significantly improved clinical status (p = 0.039), whereas age (p = 0.298) and transport distance (p = 0.114) showed no significant association. These findings indicate that rapid ambulance response and appropriate pre-hospital interventions are strongly associated with improved clinical outcomes in non-trauma emergencies. Although the cross-sectional, single-center design limits causal inference and generalizability, strengthening EMS dispatch systems and timely pre-hospital care remains essential for optimizing patient outcomes.
