Abstract

Background and Purpose: Hypertension remains a leading non-communicable disease burden globally, with self-care management identified as a critical determinant of blood pressure control and complication prevention. Despite efforts to increase treatment coverage, adherence to self-care behaviors among hypertensive patients in primary health center settings in Indonesia remains suboptimal. This study aimed to evaluate the effectiveness of structured health education interventions on improving self-care management among hypertension patients registered at Medan Johor Primary Health Center. Methods: A quasi-experimental study with a one-group pre-test–post-test design was conducted from September to December 2025. A purposive sample of 60 hypertension patients meeting the inclusion criteria was recruited. The intervention comprised four structured health education sessions delivered over four consecutive weeks. Self-care management was measured using the adapted Hypertension Self-Care Activity Level Effects (H-SCALE) instrument across six domains. Ethical clearance was obtained from the Research Ethics Committee of Universitas Bunda Thamrin (No. 042/EC/UBT/2025). Data were analyzed using the Wilcoxon Signed-Rank Test (α = 0.05). Results: The mean total self-care score increased significantly from 50.4 ± 8.5 (pre-test) to 74.4 ± 6.3 (post-test). All six self-care domains demonstrated statistically significant improvements (all p < 0.001). The overall Wilcoxon Signed-Rank Test yielded Z = −6.842, p < 0.001, with a large effect size (r = 0.88). Blood pressure monitoring showed the greatest improvement (Δ = 25.2), followed by dietary management (Δ = 25.0). Conclusion: Structured health education is highly effective in improving self-care management across all domains among hypertensive patients at the primary health center level. Integration of routine health education programs within hypertension management protocols at Puskesmas is strongly recommended to optimize patient outcomes and reduce long-term cardiovascular complications.

Abstract

Background: Tuberculosis (TB) remains a major public health challenge in Indonesia, with approximately 1,090,000 cases reported annually. Achieving the national TB elimination target by 2030 requires optimal treatment adherence; however, medication non-adherence continues to hinder treatment success and disease control. This study aimed to identify factors influencing medication adherence among pulmonary TB patients at Puskesmas Medan Area Selatan. Methods: A cross-sectional analytic study was conducted from January to March 2025 involving pulmonary TB patients registered under the Directly Observed Treatment Short-course (DOTS) program. A total of 87 respondents were recruited using consecutive sampling based on the following inclusion criteria: confirmed pulmonary TB diagnosis, undergoing at least one month of treatment, and willingness to participate. Data were collected using structured questionnaires assessing six independent variables: patient knowledge, family support, healthcare service quality, drug side effects, the role of treatment supervisors (PMO), and socioeconomic status. Medication adherence was measured using the Morisky Medication Adherence Scale-8 (MMAS-8). Binary logistic regression analysis was performed using SPSS version 26.0. Results: Among the respondents, 62.1% demonstrated good medication adherence. Bivariate analysis revealed significant associations between adherence and patient knowledge (p = 0.001; OR = 4.87; 95% CI: 1.89–12.53), family support (p = 0.003; OR = 3.94; 95% CI: 1.56–9.94), healthcare service quality (p = 0.021; OR = 2.87; 95% CI: 1.17–7.03), and drug side effects (p = 0.038; OR = 2.41; 95% CI: 1.05–5.51). Multivariate logistic regression identified patient knowledge as the most dominant factor influencing adherence (Adjusted OR = 5.12; 95% CI: 1.91–13.70; p = 0.001), followed by family support (AOR = 3.78; 95% CI: 1.43–9.98; p = 0.007). Conclusion: Patient knowledge is the strongest determinant of medication adherence among TB patients at Puskesmas Medan Area Selatan. Strengthening structured health education interventions and promoting family-centered support strategies are essential to enhance treatment adherence and accelerate progress toward Indonesia’s TB elimination target by 2030.

Abstract

Background. The postpartum period represents a critical phase of physical and psychological recovery for mothers following childbirth. In many rural communities in North Sumatra, Indonesia, local cultural practices continue to shape the manner in which postpartum care is implemented, yet the extent to which these practices are consistent with evidence-based midwifery standards remains inadequately examined. Objective. This study aimed to describe the behavior and attitudes of postpartum mothers regarding locally culturally-based self-care practices in Bandar Baru Village, Sibolangit Sub-District, Deli Serdang Regency, North Sumatra. Methods. A descriptive quantitative study with a cross-sectional approach was conducted from October 18, 2024 to January 31, 2025. The total sampling technique was used, encompassing 52 postpartum mothers who met the inclusion criteria. Data were collected using a validated structured questionnaire measuring self-care behavior and attitude. Univariate analysis was performed to generate frequency distributions and percentages. Ethical clearance was obtained from the Research Ethics Committee of STIKes Darmo, with reference number 012/KEPK-STIKESDARMO/X/2024. Results. The majority of respondents (71.2%) were aged 20–35 years, with secondary education (53.8%) being most prevalent. In terms of self-care behavior, 55.8% of postpartum mothers demonstrated adequate behavior, while 44.2% showed inadequate behavior. Regarding attitude, 61.5% of respondents held a positive attitude toward self-care, while 38.5% demonstrated a negative attitude. Locally culturally-based practices identified included consumption of traditional herbal beverages (jamu) (80.8%), application of traditional abdominal binders (gurita) (75.0%), dietary taboos (67.3%), traditional massage (57.7%), and restricted physical activity (65.4%). Conclusion. There remains a substantial proportion of postpartum mothers with inadequate self-care behavior, despite the prevalence of positive attitudes. Culturally-rooted practices continue to exert a strong influence on postpartum care. Healthcare providers, particularly midwives, should integrate cultural competence into postpartum counseling to harmonize local wisdom with clinical evidence.

Abstract

Latar Belakang: Stroke merupakan penyebab kematian dan disabilitas utama di dunia, termasuk Indonesia. Pasien stroke sering mengalami gangguan menelan dan defisit neurologis yang berdampak pada pemenuhan kebutuhan nutrisi. Selfcare berbasis keluarga diyakini mampu meningkatkan kemandirian pasien dalam memenuhi kebutuhan nutrisinya. Tujuan: Menganalisis pengaruh pelatihan selfcare berbasis keluarga terhadap pola makan (nutrisi) pada pasien stroke di Desa Nogo Rejo Kecamatan Galang. Metode: Penelitian ini menggunakan desain quasi experimental dengan pendekatan one group pretest-posttest design. Populasi adalah seluruh pasien stroke di Desa Nogo Rejo Kecamatan Galang sebanyak 42 orang. Sampel berjumlah 36 responden yang dipilih menggunakan teknik purposive sampling. Intervensi berupa pelatihan selfcare berbasis keluarga selama 4 minggu (8 sesi). Instrumen yang digunakan adalah kuesioner pola makan tervalidasi. Analisis data menggunakan uji Wilcoxon Signed-Rank Test. Hasil: Sebelum intervensi, mayoritas responden memiliki pola makan tidak baik (77,8%). Setelah intervensi, mayoritas responden memiliki pola makan baik (83,3%). Hasil uji Wilcoxon menunjukkan nilai p = 0,000 (p < 0,05), yang berarti terdapat pengaruh yang signifikan pelatihan selfcare berbasis keluarga terhadap pola makan pasien stroke. Kesimpulan: Pelatihan selfcare berbasis keluarga secara signifikan meningkatkan pola makan pada pasien stroke. Program ini direkomendasikan untuk diintegrasikan dalam layanan kesehatan komunitas dan program rehabilitasi stroke.
