Abstract

Kehamilan pada usia >35 tahun membawa risiko medis dan psikologis, seperti kecemasan akibat ketidaksiapan emosional dan komplikasi medis. Kecemasan berlebih pada ibu hamil dengan asma bisa memperparah kondisi dan membahayakan janin. Bidan berperan penting dalam perawatan medis dan psikososial untuk mengelola risiko tersebut. Case report ini bertujuan menggambarkan manajemen kehamilan risiko tinggi pada Ny. M, 41 tahun, dengan kehamilan tidak diinginkan yang disertai eksaserbasi asma. Penelitian ini menggunakan metode studi kasus dengan pendekatan deskriptif, sampel seorang IRT berusia 41 tahun dengan G3P2A0 dan menggunakan data primer dan sekunder. Ny. M, 41 tahun UK 28-29 minggu, datang ke IGD VK RSUD dengan keluhan sesak napas. Kehamilan tidak direncanakan, namum didukung keluarga, dengan riwayat asma, hipertensi (142/90 mmHg), dan anemia ringan (Hb 10,8 g/dL). Pengelolaan meliputi terapi nebulizer, pemantauan tekanan darah, serta suplementasi kalsium dan zat besi. Persalinan direncanakan di rumah sakit rujukan, dengan diskusi tentang kontrasepsi pasca-salin dan dukungan keluarga juga diintegrasikan untuk mengurangi kecemasan pasien. Kehamilan tidak direncanakan di atas usia 35 tahun membutuhkan manajemen komprehensif multidisiplin untuk meminimalkan risiko medis dan psikologis bagi ibu dan janin.

Abstract

Background: Maternal mortality rates in Indonesia remain high, with the main causes of maternal death being hemorrhage, infection, and hypertension. One effort to reduce maternal mortality rates is to increase the coverage of Antenatal Care (ANC) visits. The first ANC visit (K1) in the first trimester is very important for detecting pregnancy risks and providing early treatment. The purpose of this study was to analyze the factors that influence the first ANC visit (K1) at the Baruharjo Community Health Center in Trenggalek Regency. Methods: This study used a cross-sectional design with a sample of 81 pregnant women selected using purposive sampling. Data were collected using a questionnaire and analyzed using the chi-square test. Results: The chi-square test showed that the variables associated with the first ANC visit were age (p=0.018), knowledge (p=0.025), occupation (p=0.021), parity (p=0.049), husband's support (p=0.020), and family support (p=0.08). Meanwhile, education (p=0.740) and income (p=0.251) were not related to the first ANC visit. Conclusion: Factors that influence the first ANC visit (K1) are maternal age, maternal knowledge, parity, occupation, family support, and husband support. The 20–35 age group had the best K1 coverage. Education and family income were not associated with K1 attendance.

Abstract

Background: Early marriage remains a significant challenge in developing countries, including Indonesia, which ranks fourth globally in child marriage cases. According to UNICEF (2023), there are 25.53 million child marriage cases in Indonesia. In 2022, the rate of early marriage in Bangkalan, Madura, continued to show an increasing trend, with an annual rise of 1.71%. The Marriage Age Maturity Program (PUP) aims to increase the minimum age of marriage to 21 years for women and 25 years for men. Objective: This study aims to examine the relationship between knowledge of marriage maturity and adolescent attitudes toward early marriage. Methods: This research uses a quantitative method with a cross-sectional design. The population consisted of 10th- and 11th-grade students at SMAN 1 Kamal, with a sample size of 183 respondents selected through cluster random sampling. The independent variable is knowledge of marriage maturity, while the dependent variable is adolescents' attitudes toward early marriage. A questionnaire was used as the instrument, and data analysis was performed using the Spearman rank test. Results: The study shows that most respondents (72.7%) have good knowledge about marriage maturity. Respondents with good knowledge tend to have a positive attitude or do not support early marriage (90%). The Spearman rank test revealed a p-value of <0.001 (<0.005), indicating a significant relationship between knowledge of marriage age maturity and adolescents' attitudes toward early marriage, with a low correlation level (r = 0.312). Recommendations: It is recommended that schools, particularly counseling services, conduct educational sessions on the importance of marriage age maturity to enhance adolescents' knowledge and prevent early marriage.

Abstract

Preeclampsia is one of the causes of maternal and perinatal deaths in Indonesia, especially in East Java. This condition can increase the risk of long-term complications and potentially cause death. Preeclampsia can worsen quickly, so Antenatal Care (ANC) is needed to monitor the condition and reduce the risk of morbidity and mortality. Objective: to analyze the relationship between ANC quality and maternal risk factors with preeclampsia morbidity and mortality. Method: This research is retrospective analytical, data collection uses secondary data obtained from patient medical records. A total of 105 patients met the inclusion criteria, namely a diagnosis of preeclampsia and a gestational age of 2023 weeks and the exclusion criteria were incomplete medical records. Research data was analyzed using the chi-square test with a level of significance (α=0.05). Results: Preeclampsia morbidity and mortality were significantly associated with BMI (P=0.006), while morbidity alone was associated with health services (P=0.001), parity (P=0.045), education (P=0.006), and type of preeclampsia (P= 0.007). Factors that were not associated with mortality were age (P=0.704), occupation (P=0.639), proteinuria levels (P=0.411), and ANC frequency (P=0.565). Conclusion: the quality of ANC services influences the incidence of preeclampsia morbidity, along with the risk factors of BMI, parity, education, and type of preeclampsia. Early identification of risk factors and improving the quality of ANC services are important for appropriate management and prevention of mortality.
