Abstract

Introduction : In the modern era, dental esthetics has become an important consideration, particularly in cases of gummy smile, which may significantly reduce a patient’s self-confidence. A gummy smile is defined as excessive gingival display of more than 3 mm in the vertical dimension of the maxillary anterior region during smiling. The etiology of gummy smile is multifactorial and may include hyperactivity of the upper lip elevator muscles, vertical maxillary excess, and delayed passive eruption. Case Report: A 32-year-old woman presented with the chief complaint of excessive gingival display during smiling and dissatisfaction with poorly fabricated veneers, which negatively affected her confidence. She also reported halitosis following veneer placement performed at a non-clinical setting. Intraoral examination of the maxillary and mandibular anterior regions revealed poor-quality veneers, hyperemic and edematous gingival margins, and abnormal gingival texture. Discussion: A pleasant smile requires harmony among tooth shape, color, size, gingival health, and an appropriate amount of gingival display during smiling. This case report describes a surgical approach combining periodontal soft tissue reconstruction with smile elevator tissue containment for the correction of gummy smile. The technique aims to reduce excessive gingival display by limiting the excessive retraction of the smile elevator muscles. Conclusion: Periodontal soft tissue reconstruction in combination with smile elevator tissue containment represents an innovative surgical technique for the correction of gummy smile.

Abstract

Ketuban Pecah Dini (KPD) meningkatkan risiko infeksi neonatal yang mengancam jiwa, sehingga deteksi dini proses inflamasi sangat krusial. Penelitian ini menganalisis nilai Laju Endap Darah (LED) dan kadar C-Reaktif Protein (CRP) menggunakan metode Immunofluorescence Assay (IFA) pada neonatus dengan riwayat KPD. Metode penelitian yang digunakan adalah observasional deskriptif analitik dengan rancangan cross-sectional terhadap 50 neonatus (25 laki-laki dan 25 perempuan) di Rumkit Tingkat IV Gubeng. Hasil penelitian menunjukkan bahwa median nilai LED pada neonatus perempuan (17 mm/jam) dan laki-laki (16 mm/jam) tidak memiliki perbedaan signifikan (p = 0,558). Demikian pula, kadar CRP tidak menunjukkan perbedaan signifikan berdasarkan jenis kelamin (p = 0,207). Temuan krusial menunjukkan hubungan antara LED dan CRP bersifat sangat lemah dan tidak signifikan secara statistik (rho = 0,19; p = 0,178). Simpulan dari penelitian ini adalah meskipun KPD memicu peningkatan biomarker inflamasi di atas ambang batas normal, profil tersebut tidak dipengaruhi oleh gender dan kedua biomarker tidak bergerak secara linier. Hal ini menegaskan pentingnya penggunaan kedua parameter secara komplementer dalam skrining infeksi neonatal.

Abstract

Background: A ‘gummy’ smile, also known as excessive gingival display, is a dental condition characterized by the excessive visibility of the gums when a person smiles. This condition typically occurs when more than 3–4 mm of gum tissue is exposed, making the teeth appear shorter and affecting the smile’s aesthetic appeal. Purpose: This case report aims to explain the minimally invasive surgery technique for correcting a gummy smile using diamond frenotomy. Case: A 24-year-old woman with the chief complaint that her gingiva was visible excessively when smiling. Case management: Intraoral examination on the maxillary anterior region showed a gingival margin at the normal position, normal texture, standard color of gingiva, and normal crown size. Diamond frenotomy with a double mosquito hemostat was inserted parallel to the attached gingiva and vestibule, then incised until the two mosquito clamps met, producing the remaining frenum tissue to be excised. Following evaluation over six months, the gummy smile was observed to have been significantly reduced. Conclusion: Diamond frenotomy is an innovative surgical technique for gummy smile correction.
