Abstract

ABSTRACT The OEIS complex—comprising Omphalocele, Exstrophy of the cloaca, Imperforate anus, and Spinal defects—is one of the rarest and most severe congenital malformation syndromes, with an estimated incidence of 1 in 200,000–400,000 live births. Early prenatal detection plays a critical role in facilitating appropriate counseling and multidisciplinary perinatal planning. We report a prenatally diagnosed case of severe OEIS complex in a 21-year-old primiparous woman referred for suspected abdominal wall defect and fetal tachycardia. Ultrasound revealed a single live fetus at 32 weeks' gestation with a large omphalocele, protruding bowel loops, and features suggestive of cloacal exstrophy. Postnatal examination confirmed omphalocele, cloacal exstrophy, imperforate anus, ambiguous genitalia, and right-sided congenital talipes equinovarus. These findings were consistent with the classical presentation of severe OEIS. This case underscores the importance of early prenatal diagnosis and coordinated multidisciplinary management in severe OEIS complex. Timely recognition through imaging and delivery planning at a tertiary care center are crucial for optimizing immediate neonatal outcomes. Long-term prognosis remains dependent on the extent of anomalies and access to comprehensive surgical and supportive care. Keywords: OEIS, Omphalocele, Exstrophy of the Cloaca, Imperforate Anus, and Spinal Defects, Ccongenital Anomaly.

Abstract

Introduction: In Utero Genetic Editing (IUGE) is an emerging technology intended to modify the fetal genome to prevent or treat genetic disorders before birth. While it presents significant medical potential, it raises complex ethical, legal, and regulatory challenges. Current interventions remain largely animal-based, with human applications at theoretical or preclinical stages.Methods: This review examines ethical guidelines, legal frameworks, and regulatory perspectives globally and in Indonesia. Sources included WHO, the Indonesian Obstetrics and Gynecology Association (POGI), Indonesian health laws, and journals such as the American Journal of Obstetrics and Gynecology (AJOG). Literature was drawn from PubMed, WHO databases, Indonesian archives, and AJOG publications (2015–2025).Results: WHO emphasized precaution and transparency, while POGI stressed maternal–fetal safety under strict oversight. Indonesia lacks specific regulation, requiring interventions to respect safety and human dignity. The United States and European Union maintain restrictive regulations, including embryo research bans.Conclusion: IUGE is promising for reproductive health but remains ethically and legally unresolved, requiring clearer frameworks before clinical translation. AbstrakPendahuluan: In Utero Genetic Editing (IUGE) merupakan teknologi baru untuk memodifikasi genom janin guna mencegah atau mengobati kelainan genetik sejak sebelum lahir. Teknologi ini menjanjikan manfaat medis, namun menimbulkan tantangan etik, hukum, dan regulasi. Hingga kini, sebagian besar intervensi masih berbasis penelitian hewan, sementara aplikasi pada manusia berada pada tahap teoritis atau pra-klinis.Metode: Kajian ini menelaah pedoman etik, kerangka hukum, dan regulasi dari perspektif global dan Indonesia. Sumber mencakup WHO, Perkumpulan Obstetri dan Ginekologi Indonesia (POGI), peraturan kesehatan nasional, serta jurnal internasional seperti American Journal of Obstetrics and Gynecology (AJOG). Literatur diperoleh dari PubMed, basis data WHO, arsip hukum Indonesia, dan publikasi AJOG (2015–2025).Hasil: WHO menekankan prinsip kehati-hatian dan transparansi, sementara POGI menegaskan keselamatan ibu–janin dengan pengawasan ketat. Indonesia belum memiliki regulasi khusus, tetapi mensyaratkan intervensi genetik menjaga keselamatan dan martabat manusia. Negara seperti Amerika Serikat dan Uni Eropa menetapkan aturan restriktif, termasuk larangan riset embrio.Kesimpulan: IUGE berpotensi meningkatkan kesehatan reproduksi, namun memerlukan regulasi yang lebih jelas sebelum penerapan klinis.Kata kunci: In Utero Genetic Editing, etika, regulasi, terapi janin, bioetika

Abstract

Abstrak Placenta Accreta Spectrum Disorder (PASD) merupakan komplikasi obstetri berat dengan risiko morbiditas dan mortalitas maternal yang tinggi. Selain dampak fisik, diagnosis PASD berpotensi menimbulkan reaksi stres akut dan meningkatkan risiko depresi pada ibu hamil. Penelitian ini bertujuan untuk membandingkan risiko stres dan depresi antara ibu hamil dengan dan tanpa PASD, serta menganalisis hubungan antara risiko stres dan depresi pada pasien PASD. Penelitian ini menggunakan desain deskriptif analitik dengan pendekatan potong lintang. Subjek penelitian terdiri dari 68 ibu hamil (34 PASD dan 34 non-PASD) yang menjalani antenatal care di RSUD Arifin Achmad Kota Pekanbaru periode Agustus–Oktober 2025, dengan teknik consecutive sampling. Risiko stres dinilai menggunakan Impact of Event Scale–Revised (IES-R), sedangkan risiko depresi dinilai menggunakan Edinburgh Postnatal Depression Scale (EPDS). Hasil penelitian menunjukkan bahwa risiko stres akut secara signifikan lebih tinggi pada kelompok PASD dibandingkan non-PASD (23,53% vs 0%; p = 0,001). Risiko depresi juga secara signifikan lebih tinggi pada kelompok PASD dibandingkan kontrol (p = 0,006). Terdapat hubungan yang sangat bermakna antara stres akut dan risiko depresi pada pasien PASD (p < 0,001; Cramér’s V = 1,00). Analisis multivariat menunjukkan bahwa pekerjaan dan penghasilan merupakan prediktor independen risiko stres (p = 0,009 dan p = 0,007), sedangkan suku, agama, dan pekerjaan berhubungan dengan risiko depresi pada pasien PASD. Kesimpulannya, diagnosis PASD berhubungan erat dengan peningkatan risiko stres akut dan depresi pada ibu hamil, sehingga skrining kesehatan mental perlu diintegrasikan dalam penatalaksanaan PASD. Abstract

Tetanus Toxoid (TT) immunization remains a critical strategy in preventing maternal and neonatal deaths caused by tetanus. Despite its proven effectiveness, TT immunization coverage in Indonesia including in Pekanbaru City remains below target. Among the contributing factors are pregnant women's levels of knowledge, attitudes, and behaviors, which are believed to significantly influence immunization uptake. Objective: This study aimed to assess the relationship between pregnant women’s knowledge, attitudes, and behaviors and their participation in TT immunization programs across three health center areas in Pekanbaru: Simpang Tiga, Rejosari, and Okura. Methods: A descriptive analytic quantitative study with a cross-sectional design was conducted, involving 111 pregnant women who attended antenatal care (ANC) visits in May 2025. Data were analyzed using the Chi-Square test, Spearman Rank correlation, Cramer’s V, and logistic regression, with a significance level set at p < 0.05. Results: The study found statistically significant differences in knowledge, attitudes, and behaviors among the three health center areas (p < 0.05). Notably, good knowledge did not always correspond to positive attitudes—many well-informed respondents still exhibited negative attitudes. However, a strong correlation was observed between attitude and behavior (Cramer’s V = 0.552). Logistic regression revealed that knowledge (OR = 0.201; p = 0.005), attitude (OR = 2.068; p = 0.036), and behavior (OR = 5.660; p < 0.001) significantly influenced TT immunization uptake, with behavior emerging as the most dominant factor. Conclusion: Efforts to improve TT immunization coverage must go beyond knowledge-based education and focus on fostering positive attitudes and reinforcing supportive behaviors. Behavior-centered interventions and effective communication by health workers are essential to strengthen immunization outcomes.

Abstract

Abstrak: Remaja merupakan kelompok yang rentan terhadap risiko penularan HIV akibat keterbatasan pengetahuan dan masih adanya sikap serta perilaku berisiko. Kegiatan pengabdian ini bertujuan meningkatkan pengetahuan, sikap, dan perilaku pencegahan HIV pada remaja melalui edukasi berbasis sekolah. Metode yang digunakan berupa penyuluhan dan diskusi interaktif tentang HIV yang dilaksanakan kepada 240 siswa sebagai mitra kegiatan. Evaluasi dilakukan menggunakan kuesioner pengetahuan, sikap, dan perilaku pencegahan HIV setelah kegiatan, yang terdiri dari 61 pertanyaan. Hasil menunjukkan bahwa 52,5% peserta memiliki tingkat pengetahuan baik, 58,8% menunjukkan sikap positif terhadap pencegahan HIV, dan 84,6% telah menerapkan perilaku pencegahan yang baik. Capaian ini menunjukkan bahwa edukasi HIV berbasis sekolah berkontribusi positif dalam memperkuat literasi kesehatan dan mendorong pencegahan perilaku berisiko pada remaja. Program ini berpotensi menjadi model intervensi promotif dan preventif di lingkungan sekolah.Abstract: Adolescents are a vulnerable group for HIV transmission due to limited knowledge and the persistence of risky attitudes and behaviors. This community service program aimed to enhance knowledge, attitudes, and HIV preventive behaviors among adolescents through school-based education. The program was conducted through health education sessions and interactive discussions involving 240 students as program partners. Evaluation was carried out using a questionnaire assessing knowledge, attitudes, and HIV-related preventive behaviors after the activity, consisting of 61 questions. The results indicated that 52.5% of participants had good knowledge, 58.8% demonstrated positive attitudes toward HIV prevention, and 84.6% reported good preventive behaviors. These findings suggest that school-based HIV education contributes to improved health literacy and supports the reduction of risky behaviors among adolescents. This program has the potential to serve as a promotive and preventive model for HIV education in school settings.
