Abstract

Penyakit ginjal kronik (PGK) merupakan masalah kesehatan masyarakat yang terus meningkat di Indonesia seiring dengan meningkatnya prevalensi hipertensi. Data Riset Kesehatan Dasar (Riskesdas) tahun 2018 menunjukkan bahwa prevalensi gagal ginjal kronik di Indonesia masih menjadi perhatian serius, dan di Rumah Sakit X Tangerang, PGK termasuk dalam penyakit dengan angka kejadian tinggi dengan hipertensi sebagai komorbiditas utama. Penelitian ini bertujuan untuk mengevaluasi pola penggunaan obat antihipertensi pada pasien PGK yang dirawat inap, meliputi ketepatan pemilihan pasien, indikasi, pemilihan obat, dan dosis. Penelitian ini menggunakan desain deskriptif dengan pengambilan data retrospektif dari rekam medis pasien PGK yang dirawat di ruang rawat inap Rumah Sakit X Tangerang. Data yang dikumpulkan meliputi karakteristik pasien, jenis obat antihipertensi yang digunakan, kadar kreatinin serum, tekanan darah, serta kondisi klinis pasien. Hasil penelitian menunjukkan bahwa sebanyak 41,6% pasien menerima terapi monoterapi dan 58,3% menerima terapi kombinasi. Obat antihipertensi yang paling banyak digunakan meliputi captopril, ramipril, valsartan, telmisartan, furosemid, bisoprolol, amlodipin, nifedipin, dan klonidin. Evaluasi penggunaan obat antihipertensi menunjukkan ketepatan indikasi sebesar 100%, ketepatan pemilihan obat sebesar 98,33%, dan ketepatan dosis sebesar 96,66%. Kesimpulan penelitian ini menunjukkan bahwa penggunaan obat antihipertensi pada pasien PGK yang dirawat inap di Rumah Sakit X Tangerang sebagian besar telah rasional dan sesuai dengan pedoman terapi, namun evaluasi berkelanjutan tetap diperlukan untuk meningkatkan kualitas terapi.

Abstract

Chronic kidney disease (CKD) has become an increasingly important public health problem in Indonesia, in parallel with the rising prevalence of hypertension. Data from the 2018 Basic Health Research (Riskesdas) indicated that the prevalence of chronic kidney failure in Indonesia remained a serious concern, and at RSU Kab. Tangerang, CKD was among the diseases with a high incidence, with hypertension as the main comorbidity. This study aimed to evaluate patterns of antihypertensive drug use in hospitalized CKD patients, including the appropriateness of patient selection, indications, drug selection, and dosing. A descriptive study design was applied using retrospective data obtained from the medical records of CKD patients treated in the inpatient ward of RSU Kab. Tangerang during the most recent study period. The data collected included patient characteristics, types of antihypertensive drugs prescribed, serum creatinine levels, blood pressure measurements, and patients’ clinical conditions. The results showed that 41.6% of patients received monotherapy, while 58.3% received combination therapy. The most commonly prescribed antihypertensive drugs were captopril, ramipril, valsartan, telmisartan, furosemide, bisoprolol, amlodipine, nifedipine, and clonidine. Evaluation of antihypertensive drug use revealed 100% appropriateness in indications, 98.33% appropriateness in drug selection, and 96.66% appropriateness in dosing. In conclusion, the use of antihypertensive drugs in hospitalized CKD patients RSU Kab. Tangerang was largely rational and consistent with therapeutic guidelines; however, continuous evaluation is still required to further improve the quality of therapy.

Abstract

Background: Gastritis is a health disorder in digestion, especially the stomach. Treatment of gastritis can be cured by using medicinal plants which are thought to have compounds that are efficacious as gastroprotective. The karamunting plant is one of the biodiversity that must be developed because the karamunting plant has one of the properties as a gastroprotective. Karamunting, among others, has various kinds of ingredients such as flavonoids, saponins, tannins, alkaloids, and triterpenoids which are useful as herbal medicines. Objective: This study aims to determine the content of secondary metabolites, determine the effectiveness of healing gastritis in rats and determine what dose of 70% ethanol extract of karamunting leaves has the effectiveness of gastroprotective healing in rats. Method: Test animals were divided into 6 groups consisting of group I negative control CMC 0.5%; group II positive control of 500mg/5ml sucralfate; group III induction control 1000mg/kgBB; groups IV, V, VI 70% ethanol extract of karamunting leaves at a dose of 20mg/200g BW of rats, a dose of 40mg/200g BW of rats and a dose of 60mg/200g BW of rats. After the treatment, the animals were dissected and the stomach was observed macroscopically. Result: Data were analyzed with the Kruskal Wallis test, and followed by the Mann-Whitney test. The results of this study showed that 70% ethanol extract of karamunting leaves has effectiveness as a gastroprotective. A dose of 20mg/200gBW rats has a % protection value of 76%, a dose of 40mg/200gBW rats has a % protection value of 75%, and a dose of 60mg/200gBW rats has a % protection value of 100%. Statistical test results showed that there was a significant difference in each group on gastroprotective effectiveness (p <0.05). Conclusion: The conclusion of this study was that the 70% ethanol extract of caramunting leaves had the most optimal gastroprotective effectiveness at dose III, namely 60mg/200gBW rats.
