Abstract

This study aims to analyze the effectiveness of organizational design and its impact on the performance of the Emergency Department at Luwuk Banggai Regional General Hospital. A quantitative descriptive-analytic method was used, involving all 75 health and non-health workers in the ED selected through total sampling. Data were collected using a Likert-scale questionnaire and analyzed using Pearson correlation and simple linear regression tests. The findings indicated that the effectiveness of organizational design and ED performance were both in the high category, with means of 4.12 and 4.18 respectively. The correlation test showed a positive and significant relationship between organizational design effectiveness and ED performance (r = 0.713, p = 0.000). Regression analysis revealed that organizational design contributed 50.8 percent to ED performance (R² = 0.508). In conclusion, effective implementation of organizational design through clear structure, task distribution, communication, and interprofessional coordination leads to higher performance in the emergency department. The study emphasizes the importance of adaptive and collaborative organizational design in enhancing hospital service performance.

Abstract

Dengan penerapan strategi tersebut, diharapkan partisipasi peserta Prolanis di Klinik SMC Banyusari dapat meningkat secara berkelanjutan Kata Kunci: prolanis, analisis SWOT, Strategi Manajerial     ABSTRACT Low participation in the Chronic Disease Management Program (Prolanis) remains a major challenge for primary healthcare facilities, including SMC Banyusari Clinic in Denpasar. Clinic records show that the average monthly attendance rate of Prolanis participants is approximately 21.2%, which is below the minimum target of 50% set by the National Health Insurance Agency (BPJS Kesehatan). This study aims to analyze internal and external factors using a SWOT analysis to determine appropriate managerial strategies for improving the implementation of the Prolanis program at SMC Banyusari Clinic. This study employed a qualitative approach with a case study design. Data were collected through in-depth interviews, observations, and document reviews involving six informants, consisting of the clinic head, Prolanis officer, and Prolanis participants. The data were analyzed using the Internal Factors Analysis Summary (IFAS) and External Factors Analysis Summary (EFAS) matrices. The findings indicate that the clinic’s internal strengths outweigh its weaknesses, reflected by an IFAS score of 1.31, while external opportunities exceed existing threats, with an EFAS score of 1.12. These results place the clinic in a stable growth strategic position, enabling program improvement through the optimization of internal resources and the utilization of external opportunities. Recommended managerial strategies include strengthening participant-centered health education, enhancing the role of Prolanis officers, utilizing information technology, and developing collaboration with local communities and village authorities. Implementing these strategies is expected to sustainably increase Prolanis participant attendance at SMC Banyusari Clinic. Keywords: Prolanis, SWOT Analysis, Managerial Strategy

Abstract

This study examines the roles of professionalism and organizational commitment as predictors of human resource (HR) productivity in a military hospital (Hospital X) in the Special Region of Yogyakarta, Indonesia. A descriptive qualitative single case study was employed. Data were collected through semi-structured interviews with ten purposively selected informants representing multiple organizational levels, supported by participant observation and document review. Data were analyzed using Braun and Clarke's (2006) six-phase thematic analysis with source and method triangulation. Data saturation was achieved at the eighth informant. The findings reveal that HR professionalism was generally adequate but constrained by cross-assignment practices involving 60% of nursing staff, nurse-to-patient ratios exceeding recommended standards (1:10 versus 1:4–1:6), and administrative tasks consuming approximately 40% of nurses' working time. Affective commitment emerged as the strongest dimension of organizational commitment, while continuance commitment remained the weakest, particularly among younger and civilian personnel. The study also identified a synergistic interaction between professionalism and organizational commitment in enhancing HR productivity, rather than a simple additive effect. Furthermore, military-specific contextual factors, including command hierarchy, dual workforce composition, and dual operational demands, moderated these relationships. Based on these findings, the study proposes a conceptual model in which affective commitment mediates the relationship between professionalism and HR productivity, while hierarchical structure and role duality function as contextual moderators. These findings extend conventional human resource management models and provide practical implications for developing integrated managerial strategies to improve workforce productivity in military healthcare organizations.
