Abstract

ABSTRACT Self efficacy is a person’s belief in his ability to regulate himself, and take action to achieve certain skills. Stress is the body’s response caused by demands from outside the individual that exceed the ability to solve problems. This study aims to determine the relationship between self efficacy and stress levels in breast cancer patients at Waled Hospital, Cirebon Regency.  This type of research is quantitative with cross-sectional approach. Sampling using purposive sampling technique amounted to 31 respondents. The instrument used were standard self efficacay questionnaires and DASS 42 (Depression, Anxiety and Stres). Data analysis using chi-square. The research location was at Waled Hospital, Cirebon Regency. The results of the study found that most of the respondents experienced less self efficacy with moderate stress levels dominating. 31 respondents experienced less self efficacy (35,5%) and some respondents with moderate stress levels (41,9%). The results of the chi-square test obtained a p-value of 0.000 a=0.05, so H0=rejected, which means that there is a relationship between self efficacy and the stress levels of breast cancer patients at Waled General Hospital.  For breast cancer patients who have low self efficacy, it is recommended to add more insight and knowledge so that their self efficacy increases so that stress levels are also reduced in patients at Waled Hospital, Cirebon Regency.  Keywords: Breast Cancer, Self Efficacy, Stress Levels  ABSTRAK Self Efficacy merupakan keyakinan sesorang terhadap kemampuannya untuk mengatur diri sendiri,dan mengambil tindakan untuk mencapai keterampilan tertentu. Stress adalah respon tubuh yang diakibatkan karena adanya tuntutan dari luar diri individu yang melebihi kemampuan dalam menyelesaikan masalah. Penelitian ini bertujuan untuk mengetahui hubungan self efficacy dengan tingkat stress pada pasien kanker payudara di RSUD Waled Kabupaten Cirebon. Jenis penelitian ini adalah kuantitatif dengan pendekatan cros sectional. Pengambilan sampel menggunakan Teknik purposive sampling berjumlah 31 responden. Instrumen yang digunakan berupa kuesioner baku Self-Efficcay dan DASS 42 (Depression, Anxiety and Stres). Analisa data menggunakan Chi-Square. Tempat penelitian di RSUD Waled Kabupaten Cirebon. Hasil penelitian didapatkan Sebagian besar responden mengalami Self Efficcay kurang dengan di dominasi tingkat stress sedang. 31 responden mengalami self efficacy kurang sebanyak (35,5%) dan Sebagian responden dengan tingkat stress sedang sebanyak (41,9%). Hasil uji chi-square didapatkan p-value 0,000 a=0,05 maka H0=ditolak yang artinya ada hubungan antara self efficacy dengan tingkat stress pasien kanker payudara di RSUD Waled Kabupaten Cirebon. Bagi pasien kanker payudara yang memiliki Self Efficcay rendah disarankan agar menambah wawasan dan ilmu yang lebih banyak agar self efficacy nya meningkat sehingga tingkat stress juga berkurang pada pasien di RSUD Waled Kabupaten Cirebon. Kata Kunci: Efikasi Diri, Tingkat Stres, Kanker Payudara.

Abstract

ABSTRACT Indonesian adolescents are increasingly dependent on smartphones, often using them during bedtime, which may affect sleep quality. To examine the relationship between smartphone usage intensity and sleep quality among adolescents at SMP Negeri 1 Susukan Lebak, Cirebon Regency. A cross-sectional correlational study involving 220 students selected through stratified random sampling. Data were collected using the Smartphone Usage Intensity Questionnaire and the Adolescent Sleep-Wake Scale, and analyzed using Spearman’s rank correlation. The majority of respondents had moderate smartphone usage (53.2%) and poor sleep quality (50.9%). There was a strong negative correlation between smartphone usage intensity and sleep quality (p = 0.000; r = -0.645). Higher smartphone usage intensity is associated with poorer sleep quality. Health promotion efforts targeting adolescents and parents are needed to address this issue. Keywords: Adolescent, Sleep Quality, Smartphone Usage Intensity.  ABSTRAK Remaja di Indonesia semakin bergantung pada smartphone, sering menggunakannya saat waktu tidur, yang dapat mempengaruhi kualitas tidur mereka. Meneliti hubungan antara intensitas penggunaan smartphone dengan kualitas tidur pada remaja di SMP Negeri 1 Susukan Lebak, Kabupaten Cirebon. Penelitian korelasional dengan desain potong lintang yang melibatkan 220 siswa yang dipilih melalui stratified random sampling. Data dikumpulkan menggunakan Smartphone Usage Intensity Questionnaire dan Adolescent Sleep-Wake Scale, lalu dianalisis menggunakan uji korelasi Spearman’s rank. Mayoritas responden memiliki intensitas penggunaan smartphone dalam kategori sedang (53,2%) dan kualitas tidur yang buruk (50,9%). Terdapat korelasi negatif yang kuat antara intensitas penggunaan smartphone dengan kualitas tidur (p = 0,000; r = -0,645).Semakin tinggi intensitas penggunaan smartphone, semakin buruk kualitas tidur remaja. Upaya promosi kesehatan yang menargetkan remaja dan orang tua diperlukan untuk mengatasi masalah ini. Kata Kunci: Remaja, Kualitas Tidur, Intensitas Penggunaan Smartphone.

Abstract

Chronic Kidney Disease is a condition when kidney function begins to decline gradually. Chronic kidney failure disease called kidney damage can be tissue abnormalities, blood composition, and urine or kidney imaging tests, experienced for more than three months. This scientific paper aims to analyze nursing care with the application of slimber ice therapy  to chronic kidney disease  clients with hypervolemia nursing problems. The method applied in the preparation of this scientific paper is the case study method. The subject of this scientific paper is involving 1 client with a case of CKD and having hypervolemia nursing problems who are undergoing treatment in the Rosella Room of Kardinah Hospital, Tegal City. Data collection uses interview, observation, and physical examination techniques. The nursing care process for the client was carried out for 3 days, and the results of the assessment were obtained, namely edema in both lower extremities. The intervention provided is in the form of fluid restriction with the application  of slimber ice  therapy to reduce thirst which is implemented for 3 days, with slimber ice therapy  for 5-10 minutes during dialysis sessions can reduce thirst and can moisturize the esophagus so as to reduce thirst and minimize fluid input in the body. Based on the results of the evaluation, it was found that the application  of slimber ice therapy  on the first to third day was able to reduce thirst in CKD patients undergoing hemodialysis.

Abstract

Pleural effusion is a condition characterized by the accumulation of excess fluid in the pleural cavity, which may cause impaired lung expansion, decreased ventilation, dyspnea, and reduced oxygen saturation. This condition often leads to the nursing problem of ineffective breathing pattern, requiring prompt management to maintain adequate oxygenation. One of the non-pharmacological interventions that can be applied is the Pursed Lip Breathing (PLB) technique, a breathing exercise aimed at improving lung ventilation, reducing shortness of breath, and increasing oxygen saturation. The purpose of this case study was to determine the application of Pursed Lip Breathing in improving oxygen saturation in a patient with pleural effusion experiencing an ineffective breathing pattern at Rosella Ward, Kardinah Regional General Hospital, Tegal City. The method used was a case study approach based on the nursing process, including assessment, nursing diagnosis, intervention, implementation, and evaluation. Data were collected through interviews, observation, physical examination, and documentation. The subject of this study was one patient diagnosed with pleural effusion who experienced an ineffective breathing pattern. The Pursed Lip Breathing intervention was administered for three consecutive days, with a duration of 5–10 minutes per session according to the patient's tolerance. The results showed an improvement in the patient's respiratory status after the implementation of Pursed Lip Breathing. The patient experienced a reduction in dyspnea, respiratory rate approached the normal range, the use of accessory respiratory muscles decreased, and oxygen saturation increased compared to the pre-intervention condition. In addition, the patient appeared more relaxed and comfortable while breathing. In conclusion, the application of Pursed Lip Breathing was effective in improving oxygen saturation and breathing patterns in a patient with pleural effusion and an ineffective breathing pattern. This intervention can be considered a simple, safe, and effective non-pharmacological nursing intervention to support patients' oxygenation needs.

Abstract

Non-hemorrhagic stroke is a neurological disorder caused by obstruction of blood flow to the brain, resulting in reduced oxygen and nutrient supply to cerebral tissue. This condition can lead to impaired cerebral perfusion, decreased oxygen saturation, and neurological dysfunction. One of the non-pharmacological interventions that can be implemented to improve oxygenation in non-hemorrhagic stroke patients is the application of a 30-degree head-up position. This intervention aims to improve cerebral venous return, optimize cerebral perfusion, and enhance oxygen saturation.This study aimed to analyze the effectiveness of the 30-degree head-up position in improving oxygen saturation (SpO₂) among non-hemorrhagic stroke patients in the Emergency Department of Gunungjati Regional Hospital, Cirebon City. The study employed a descriptive case study approach by implementing nursing interventions in non-hemorrhagic stroke patients experiencing impaired cerebral perfusion and decreased oxygen saturation. The intervention involved positioning patients in a 30-degree head-up posture while observing oxygen saturation levels before and after treatment.The findings demonstrated an increase in oxygen saturation levels following the implementation of the 30-degree head-up position. This intervention was shown to improve cerebral tissue oxygenation, stabilize hemodynamic conditions, and enhance patient comfort. Therefoe, the 30-degree head-up position can be recommended as a non-pharmacological nursing intervention for non-hemorrhagic stroke patients in emergency settings.

Abstract

Background: Pulmonary tuberculosis is a contagious infectious disease caused by Mycobacterium tuberculosis. Patients with pulmonary tuberculosis frequently experience respiratory problems, such as dyspnea and decreased oxygen saturation, due to impaired gas exchange in the lungs. Objective: This study aimed to determine the effectiveness of the implementation of pursed lips breathing and the semi-Fowler position in improving oxygen saturation among patients with pulmonary tuberculosis in the Emergency Department. Methods: This study employed a descriptive case study design using a nursing care approach for pulmonary tuberculosis patients experiencing ineffective breathing patterns. The intervention consisted of pursed lips breathing exercises and positioning patients in the semi-Fowler position throughout the nursing care process. Results: The findings demonstrated an increase in oxygen saturation from 93% and 94% to 96% and 97%, respectively. In addition, respiratory rate decreased from 28 breaths per minute to 24 breaths per minute, accompanied by improvements in the patients’ breathing patterns following the nursing interventions. Conclusion: The implementation of pursed lips breathing and the semi-Fowler position was effective in improving oxygen saturation among patients with pulmonary tuberculosis in the Emergency Department.

Abstract

The crowd of visitors in the inpatient room is a problem that is often encountered in Lily Room 1 RSU Prof. Dr. Soerojo Magelang has the potential to reduce the quality of service and patient safety (patient safety). The low family compliance with SPO hours visit an obstacle in creating a therapeutic care environment. This innovation project aims to improve SPO jam besuk compliance through the implementation of a visual instrument-based access system in the form of a visitor's ID Card. The method used is the PDCA (Plan, Do, Check, Action) quality management approach with intervention in the form of visitor access procedure education to nursing staff and patient's family, as well as the implementation of ID Card as a visual control media. Evaluation is carried out through field observation of the patient's family compliance level in the pre (5 days) and post (4 days) phases of the intervention. The observation results showed a significant increase in the level of visitor compliance; the previously high level of visitor capacity violations in the pre-intervention phase was drastically suppressed after the implementation of the ID Card. The conclusion of this project is that the implementation of the visitor ID Card is proven to be effective as an objective access management system in improving the discipline of besuk hours, thus creating a more orderly, quiet, and safe care environment in accordance with the hospital's Standard Operational Procedure (SPO).

Abstract

Osteoarthritis is a degenerative joint disease that frequently causes pain, stiffness, and limited physical mobility. This study aimed to describe the implementation of simple knee exercises for an osteoarthritis patient experiencing impaired physical mobility. A descriptive case study method was employed, involving a 61-year-old female patient. The intervention was conducted over three consecutive days. The results showed a reduction in pain scale from 6 to 3, as well as improvements in range of motion (ROM) and mobility. Conclusion: Simple knee exercises are effective as a non-pharmacological intervention for reducing pain and improving physical mobility in patients with osteoarthritis. Nevertheless, the results of this study should be interpreted with caution, as it involved only a single patient and lacked a comparison group. Research involving larger sample sizes and standardized anxiety measurement instruments is needed to strengthen the evidence regarding the effectiveness of SEFT among hemodialysis patients across various renal care settings.
