Abstract

Posyandu is one form of community empowerment aimed at improving public health, particularly for mothers and children. It serves as one of the spearheads of public health services in Karawang. However, challenges still exist in Posyandu services, including the level of participation of toddlers and the capacity of cadres in carrying out Posyandu activities. Gombongsari Village is a village where most of the residents are farmers. This village has been designated as a model village in Karawang Regency to compete in the National-level P2WKWS program.P2WKWS is an integrated assessment activity aimed at evaluating the success of villages or urban wards in empowering women’s roles in family development, particularly in health aspects, in this case, Posyandu. In response to this, a strategic step was needed to reactivate the “Activate Posyandu” movement as an effort to increase the capacity of Posyandu cadres to accelerate the achievements of Gombongsari Village Posyandu. The cadres were provided with training and education on Exclusive Breastfeeding (ASI Eksklusif), complementary feeding (MP-ASI) and animal protein-rich foods, as well as nutritional status and its management. The results of this community service activity showed a significant increase before and after the intervention in education on Exclusive Breastfeeding, complementary feeding and animal protein-rich foods, as well as nutritional status and its management (p-value = 0.001, p-value = 0.001, p-value = 0.001).The success of this intervention was supported by the delivery of materials relevant to the cadres’ field conditions, engaging media, and effective delivery methods. This proves that providing interventions effectively improves cadres’ knowledge capacity. With the increased understanding and capacity of the cadres, it is expected that they will be able to improve health services at the Posyandu.

Abstract

Background: Hypertension requires continuous self-management to achieve optimal blood pressure control. Telemonitoring can support home blood pressure monitoring and continuous self-management, but previous interventions have varied in their components and healthcare professional support. Purpose: To develop and validate a telemonitoring-based Self-Management Care Model and evaluate its effectiveness in improving blood pressure among patients with hypertension. Method: This developmental study included platform design, validation by three experts, performance testing, usability testing involving 30 potential users, evaluation, and refinement. The implementation stage used a two-group pretest-posttest design involving 128 patients with hypertension, comprising 64 intervention and 64 control participants. The intervention group used the platform for two months, while the control group received standard hypertension lifestyle education through brochures. Blood pressure was assessed at T0, T1, and T2. Effectiveness was analyzed using Difference-in-Differences with an intention-to-treat approach. Results: In the intervention group, systolic blood pressure decreased from 145.60 ± 11.32 mmHg to 140.60 ± 10.11 mmHg, with a mean reduction of 5.01 mmHg (p < 0.001), while diastolic blood pressure decreased from 95.21 ± 10.35 mmHg to 86.35 ± 9.32 mmHg, with a mean reduction of 8.45 mmHg (p < 0.001). Control-group changes were not significant. DID showed differences in systolic blood pressure at T1 (2.57, 95% CI: 1.32–5.52) and T2 (4.34, 95% CI: 2.44–7.36), and diastolic blood pressure at T1 (2.13, 95% CI: 1.57–4.68) and T2 (3.57, 95% CI: 1.89–6.60). Conclusion: The model demonstrated potential effectiveness in improving blood pressure control and provides a structured approach to long-term hypertension management.
