Abstract

Intracranial Atherosclerotic Stenosis (ICAS) adalah penyebab umum Transient Ischemic Attack (TIA) dan Stroke Iskemik. Stenosis paling sering diderita oleh orang Asia dan predileksinya umumnya menyerang arteri vertebrobasiler. Terapi ICAS hingga kini masih mengandalkan manajemen konservatif, namun kini dengan pedekatan endovaskular, metode balloon angioplasty, morbiditas dan mortalitas ICAS kini dapat dikendalikan dan diturunkan. Laki-laki, 49 tahun, datang dengan keluhan utama pusing berputar yang dialami secara tiba – tiba saat bangun tidur. Keluhan disertai dengan mual muntah dan gangguan berjalan pada ekstremitas inferior. Pemeriksaan fisik : nystagmus (+), dysmetria (+) gait ataxia (+) Romberg (+) refleks patologis Babinsky pada plantar pedis sinistra (+), dan penurunan kekuatan motorik (-). Pemeriksaan menggunakan DSA (Digital Substraction Angiography) menunjukkan adanya stenosis 90% arteri vertebralis kiri segmen V1. Ballon angioplasty untuk pasien ICAS simptomatik mungkin memiliki potensi hasil klinis yang lebih baik jika pasien juga ditunjang dengan manajemen konservatif, hal tersebut dibuktikan dengan 48 jam pasca prosedur pada pasien ini.

Abstract

Terdapat banyak laporan yang menunjukkan bahwa COVID-19 berdampak pada perubahan fungsi neurologi penderita COVID-19. Menganalisis mengenai gejala terkait neurologis yang muncul pada pasien COVID-19. Metode penelitian yang digunakan adalah penelitian sekunder yang termasuk dalam jenis literature review, penelitian menggunakan 10 literatur yang dipublikasi pada periode tahun 2019 hingga tahun 2022, literatur mengenai gejala neurologis pada COVID-19, dan literatur berupa hasil penelitian dengan metode kohort. Gejala neurologis yang sangat sering muncul pada pasien COVID-19 yaitu nyeri kepala, pusing, stroke, anosmia, dysgeusia, dan kejang. Gejala neurologis yang kadang ditemukan pasien COVID-19 yaitu myalgia, gangguan gerak, perubahan status mental, ensefalopati, penurunan kesadaran, dan plegia dan atau paresis. Gejala neurologis yang jarang terjadi pada pasien COVID-19 yaitu disautonomia, kesemutan, sindrom sistem saraf pusat, meningitis, afasia, penglihatan buram, rabdomiolisis, neuropati, gangguan tidur, ensefalitis, trombosis vena cerebral, meningoensefalitis, perdarahan intraserebral, hipotensi ortostatik, ketidakseimbangan postural, abnormalitas tonus otot, mielopati, gangguan ingatan, somnolen, koma, dan kecemasan.

Abstract

Stroke merupakan manifestasi klinis yang muncul secara tiba-tiba yang berkembang pesat dari gangguan fungsi otak fokal (atau global), dengan gejala yang berlangsung 24 jam atau lebih, atau menyebabkan kematian, tanpa penyebab yang jelas selain dari asal vaskular. Stroke secara luas diklasifikasikan menjadi stroke hemoragik dan stroke non hemoragik. Pada penelitian ini, metode yang digunakan adalah penelitian deskriptif yang dilakukan di RSUD Dr. Chasbullah Abdulmajid Kota Bekasi dengan pengumpulan data sekunder yang diperoleh dari rekam medik pasien rawat inap periode tanggal 15 – 22 September 2021. Sampel penelitian diambil dari seluruh populasi pasien dengan diagnosis stroke hemoragik dan non hemoragik di Gedung E ruangan Sakura lantai 5 dan Cattleya lantai 4. Variabel yang digunakan adalah pengukuran tensi, riwayat merokok, dan hasil lab darah (gula darah puasa dan profil lipid). Dari hasil penelitian yang dilakukan, jumlah pasien stroke non hemoragik terdapat 20 pasien sedangkan stroke hemoragik terdapat 5 pasien. Dapat disimpulkan bahwa pada stroke hemoragik, faktor risiko yang paling dominan adalah hipertensi, merokok dan hiperkolesterol. Sedangkan pada stroke non hemoragik, faktor risiko yang paling dominan adalah hipertensi dan merokok.

Abstract

Pendahuluan: Meningioma merupakan tumor primer intrakranial yang paling sering ditemui, ekstra-aksial, dan berasal dari arachnoid cap cell yang menempel pada duramater. Tumor yang tumbuh membesar secara signifikan dan terdapat gejala maka dapat dilakukan terapi pembedahan. Embolisasi pre-operatif menjadi pilihan terapi tambahan pada pasien meningioma. Embolisasi ini ditujukan untuk melakukan devaskularisasi dari lesi atau mengurangi aliran darah tumor melalui eliminasi satu atau lebih arteri yang mensuplai nutrisi tumor. Metode: Tinjauan Pustaka dilakukan berdasarkan issue, metodologi, persamaan dan proposal penelitian lanjutan. Dari 5 penelitian yang digunakan secara keseluruhan metode yang digunakan adalah analisis deskriptif dan pengumpulan data secara retrospektif selama tahun periode penelitian. Pembahasan:  Berdasarkan 5 penelitian menunjukkan bahwa embolisasi meningioma pre-operatif bertujuan untuk menurunkan vaskularisasi tumor. Tidak ada perbedaan yang signifikan dalam komplikasi, recurrence-free survival, perkiraan kehilangan darah, waktu pembedahan, serta volume transfusi darah antara kelompok embolisasi dan non-embolisasi. Simpulan: Embolisasi pre-operatif tidak memberikan hasil yang menguntungkan secara signifikan. Embolisasi pre-operatif lebih direkomendasikan pada pasien dengan kondisi komorbiditas dan indeks keparahan penyakit yang lebih tinggi.

Abstract

Chronic rhinosinusitis with nasal polyps is a chronic inflammatory condition of the nasal mucosa and paranasal sinuses, characterized by bilateral polypoid tissue growth. This disease can significantly reduce quality of life due to persistent and recurrent symptoms. This report aims to describe a case of chronic rhinosinusitis with right and left nasal polyps, including risk factors, clinical features, supporting examination results, and patient management and follow-up. This study is a descriptive case report involving one patient with bilateral chronic nasal congestion, anosmia, and rhinorrhea that lasted more than 12 weeks. Data were collected through anamnesis, physical examination, nasoendoscopy, CT scan imaging, and patient medical records. Data analysis was performed qualitatively by describing the clinical course, supporting examination results, therapeutic interventions, and patient follow-up evaluation. The patient experienced significant improvement post-operatively with decreased symptoms, increased quality of life, and no polyp recurrence was found within 3 months after the procedure. Appropriate diagnosis and management of chronic rhinosinusitis with bilateral nasal polyps are essential to prevent complications and improve patient prognosis.

Abstract

Stroke is a leading cause of death and disability worldwide, including in Indonesia. While vascular factors are the primary cause, metabolic components such as serum electrolytes and blood lipids also play a significant role in the onset and progression of stroke. Imbalances in sodium, potassium, and chloride, as well as abnormalities in cholesterol and triglyceride levels, can influence both stroke risk and prognosis. Understanding these biochemical profiles in stroke patients is essential for improving clinical outcomes. This study aims to analyze the profile of serum electrolytes and blood lipids in stroke patients treated at Dr. Chasbullah Abdulmajid Hospital, Bekasi City, during the period December 2024 to March 2025. Stroke is one of the leading causes of morbidity and mortality worldwide, which is known to occur due to vascular factors. Metabolic factors such as serum electrolytes and blood lipids can play an important role in the pathogenesis and prognosis of stroke. Using a descriptive design, patient medical record data were collected and analyzed, which were then processed univariately, with a focus on serum electrolyte profiles, including sodium, potassium, and chloride, as well as blood lipid profiles, such as total cholesterol, LDL, and HDL. The results showed that the characteristics of stroke patients treated at Dr. Chasbullah Abdulmajid Hospital, Bekasi City, during the period from December 2024 to March 2025, differed only by gender, with 46 patients (50.5%) being male and 45 patients (49.5%) being female. The most common comorbid history in stroke patients was hypertension, totaling 53 people (58.3%), followed by diabetes mellitus, totaling 26 (28.6%). The largest electrolyte imbalance disorder was hypokalemia in 43 people (47.3%), followed by hyponatremia in 35 people (38.5%) and hyperchloremia in 23 people (25.3%). The majority of patients had low HDL levels, with 46 people (50.5%), 27 people (29.7%) having increased triglyceride levels, and 20 people (22.0%) having quite high cholesterol levels. These findings are expected to provide important insights for clinical management and prevention of stroke in the future.

Abstract

Background. Rapid technological advancements have made gadgets an essential part of students' daily lives. However, excessive gadget use can have negative impacts, particularly on sleep quality. This study aims to investigate the relationship between gadget use behavior and sleep quality among students from the Faculty of Medicine at Universitas Kristen Indonesia (UKI), specifically those in the 2023 class. Research Method. The study employed a cross-sectional design with a quantitative approach, involving 138 university students as a sample collected through a total sampling technique. Data were collected using the SAS-SV (Smartphone Addiction Scale Short Version) questionnaire to measure gadget use behavior and the PSQI (Pittsburgh Sleep Quality Index) to assess sleep quality. Data analysis was performed using the chi-square test with the help of SPSS. Findings. The study's results showed that 84.1% of respondents exhibited negative gadget usage behavior, and 86.2% experienced poor sleep quality. A chi-square test showed a significant relationship between gadget usage behavior and sleep quality (p = 0.000). This study suggests that excessive gadget usage negatively impacts students' sleep quality. Conclusion. Managing gadget use wisely is crucial for improving the quality of sleep and overall health of students.
