Abstract

Acute respiratory failure merupakan komplikasi dari pneumonia karena kegagalan atau ketidakmampuan sistem respirasi dalam mempertahankan keadaan pertukaran udara dari luar tubuh dengan sel-sel tubuh yang sesuai dengan kebutuhan tubuh normal. Studi kasus ini bertujuan untuk menjelaskan asuhan keperawatan pada pasien Acute Respiratory Failure et causa Pneumonia di Unit Perawatan Intensif Rumah Sakit Umum Daerah dr. Zainoel Abidin Banda Aceh. Diagnosis keperawatan yang muncul pada Tn. A adalah gangguan pertukaran gas dengan intervensi keperawatan manajemen ventilasi mekanik, hipervolemia dengan intervensi keperawatan manajemen hipervolemia, bersihan jalan napas tidak efektif dengan intervensi keperawatan manajemen jalan napas, risiko aspirasi dengan intervensi keperawatan pencegahan aspirasi. Hasil evaluasi belum terdapat perbaikan pada kondisi pasien, ditandai dengan tekanan darah: 123/87 mmHg, Mean Arterial Pressure (MAP): 92 mmHg, heart rate: 132x/menit, SpO2: 89%, pH 7.344 mmHg, pCO2 65.30 mmHg, pO2 57 mmHg, HCO3 35.9 mEq/L, pasien terpasang ventilator mode CMV dan terdapat edema perifer. Rekomendasi bagi perawat agar memberikan terapi antibiotik dan mempertahankan oksigenasi yang adekuat untuk mencegah terjadinya hipoksemia.Kata kunci: Acute Respiratory Failure, Pneumonia, Unit Perawatan IntensifAcute respiratory failure is complications of pneumonia due to failure or inability of the respiratory system to maintain air exchange from outside the body with body cells in accordance with normal body needs. In this study, it aimed at describing the nursing care for patients with Acute Respiratory Failure et causa Pneumonia in the Intensive Care Unit At dr. Zainoel Abidin General Hospital Banda Aceh Municipality. The nursing diagnoses that emerged in Mr. A were gas exchange disorders with nursing interventions of mechanical ventilation management, hypervolemia with nursing interventions for hypervolemia management, ineffective airway clearance with airway management nursing interventions, risk of aspiration with aspiration prevention nursing interventions. The evaluation results showed no improvement in the patient's condition, indicated by blood pressure: 123/87 mmHg, Mean Arterial Pressure (MAP): 92 mmHg, heart rate: 132x/minute, SpO2: 89%, pH 7,344 mmHg, pCO2 65.30 mmHg, pO2 57 mmHg, HCO3 35.9 mEq/L, the patient is on a CMV mode ventilator and there is peripheral edema. Recommendations for nurses to provide antibiotic therapy and maintain adequate oxygenation to prevent hypoxemia.Keywords: Acute Respiratory Failure, Intensive Care Unit, Pneumonia
