The high number of workers at PT. XYZ's central distribution warehouse, without a systematically structured HSE system based on risk analysis, increases the vulnerability to workplace accidents. This aligns with national data from BPJS TK, which recorded 162,327 work accident cases from January to May 2024. This study aims to identify potential hazards, analyze risk levels using the HIRADC method, trace the root causes through Fault Tree Analysis (FTA), and design an HSE Plan tailored to actual warehouse conditions. The initial assessment showed 50% of risks were classified as high. After implementing risk controls based on the hierarchy of controls, high-risk levels dropped to 8.3%, while low-risk levels increased to 79.2%, indicating successful risk minimization. These findings emphasize the importance of a systematic HSE Plan to reduce workplace risks effectively.
