<![CDATA[ABSTRACT Hypertension is a chronic disease with a high prevalence in the community and often requires long-term therapy. Inappropriate use of antihypertensive medications can lead to Drug-Related Problems (DRPs), which can potentially reduce the effectiveness of therapy and increase the risk of side effects in patients. This study aims to analyze the incidence of DRPs in hypertensive patients in outpatient settings. This study used a descriptive, observational method with a retrospective approach through a review of the medical records of hypertensive patients in outpatient settings. Data collected included patient characteristics, the type of antihypertensive medication used, and the incidence of DRPs, including inappropriate medication selection, inappropriate dosage, drug interactions, and side effects. The results showed that several DRPs were still found in hypertensive patients, including inappropriate medication dosages, potential drug interactions, and inappropriate medication use based on the patient's clinical condition. These DRPs have the potential to impact the success of hypertension therapy. The conclusion of this study indicates that DRPs still occur in hypertensive patients in outpatient settings. Therefore, the role of healthcare professionals, particularly pharmacists, in monitoring drug therapy is needed to improve the safety and effectiveness of patient treatment. Keywords: Hypertension, Drug-Related Problems, Antihypertensive, Outpatient ABSTRAK Hipertensi merupakan salah satu penyakit kronis yang memiliki prevalensi tinggi di masyarakat dan sering memerlukan terapi jangka panjang. Penggunaan obat antihipertensi yang tidak tepat dapat menyebabkan terjadinya Drug Related Problems (DRPs) yang berpotensi menurunkan efektivitas terapi serta meningkatkan risiko efek samping pada pasien. Penelitian ini bertujuan untuk menganalisis kejadian DRPs pada pasien hipertensi di instalasi rawat jalan. Penelitian ini menggunakan metode deskriptif observasional dengan pendekatan retrospektif melalui penelaahan rekam medis pasien hipertensi di instalasi rawat jalan. Data yang dikumpulkan meliputi karakteristik pasien, jenis obat antihipertensi yang digunakan, serta kejadian DRPs yang meliputi pemilihan obat yang tidak tepat, dosis yang tidak sesuai, interaksi obat, dan efek samping obat. Hasil penelitian menunjukkan bahwa masih ditemukan beberapa kejadian DRPs pada pasien hipertensi, di antaranya dosis obat yang tidak sesuai, potensi interaksi obat, serta penggunaan obat yang tidak tepat berdasarkan kondisi klinis pasien. Kejadian DRPs tersebut berpotensi mempengaruhi keberhasilan terapi hipertensi. Kesimpulan dari penelitian ini menunjukkan bahwa DRPs masih terjadi pada pasien hipertensi di instalasi rawat jalan, sehingga diperlukan peran tenaga kesehatan, khususnya apoteker, dalam melakukan pemantauan terapi obat untuk meningkatkan keamanan dan efektivitas pengobatan pasien.
