A fracture is a disruption of bone continuity that may occur either totally or partially, caused by direct trauma, indirect trauma, or pathological conditions of the bone. A radial fracture itself is a break that occurs in the radius bone (the lateral bone of the forearm, aligned with the thumb). This type of fracture generally occurs due to a fall on an outstretched hand (FOOSH), traffic accidents, or occupational trauma." This study used a descriptive observational case study design with a nursing process approach, including assessment, diagnosis, planning, implementation, and evaluation. The sample consisted of one preoperative patient diagnosed with radius fracture, selected using purposive sampling according to the case criteria. The study was conducted in the Operating Room of Dr. Tajuddin Chalid General Hospital, Makassar, in 2025. Data were collected through observation, interviews, and measurement of anxiety levels using the Hamilton Anxiety Rating Scale (HARS) before and after the implementation of the deep breathing relaxation techniq.. The results of the study showed a decrease in anxiety level from moderate to mild after the implementation of the deep breathing relaxation technique, as measured using the Hamilton Anxiety Rating Scale (HARS).The deep breathing relaxation technique was effective in reducing preoperative anxiety. This intervention can be used as a non-pharmacological nursing approach to help patients feel calmer and better prepared for surgical procedures.
