PT. Pupuk XYZ is one of the urea producers in Indonesia, operating Urea Plants with high production performance demands. The purpose of this study is to evaluate the efficiency of the equipment and pinpoint the reasons for the deterioration in performance at Urea Plant Pabrik 2. The methods used include Overall Equipment Effectiveness (OEE), Six Big Losses, fishbone diagram, and Failure Mode and Effect Analysis (FMEA). The average OEE value, according to the data, is 88.32%, which is higher than the world-class benchmark of 85%. Performance (92.49%) stays below the benchmark and becomes the primary limiting factor, whereas Availability (95.10%) and Quality (100%) satisfy the requirements. The primary source of the effectiveness loss is unscheduled downtime, which reached 181.44 hours as a result of key equipment failures. Therefore, the implementation of Total Productive Maintenance (TPM) is proposed to improve equipment reliability and reduce downtime
