Diabetes Mellitus (DM) is a chronic disease that is one of the main health problems in the world. The complications that often occur in people with DM are peripheral neuropathy and diabetic foot ulcers, which can reduce the quality of life of sufferers. One effort to prevent these complications is through health education and regular implementation of diabetic foot exercises. The purpose of community service is to increase the knowledge of people with DM regarding disease management and improve their ability to perform diabetic foot exercises to prevent diabetic foot complications. The community service activity was carried out at the Karang Tengah Community Health Center, Garut Regency, involving 50 participants of the Chronic Disease Management Program (PROLANIS). The activity methods included health education about diabetes mellitus, demonstration of diabetic foot exercises, direct practice by participants, and evaluation using pre-test and post-test. Evaluation was carried out on the level of knowledge and complaints of foot pain of participants. Participants were predominantly women (88%) and elderly (64%). The evaluation results showed an increase in knowledge in the good category from 36% before the intervention to 72% after the intervention. Furthermore, the proportion of participants with severe pain decreased from 36% to 22% after participating in diabetic foot exercise training. Health education and diabetic foot exercise training can increase knowledge and reduce foot pain complaints in people with diabetes. This activity can be used as a sustainable promotional and preventive program within the PROLANIS program at community health centers
