A safe and sustainable blood supply relies on regular voluntary blood donors, yet adolescent participation remains low. This community service program aimed to improve adolescents' knowledge through Game Fe Card-based education and support the establishment of sustainable blood donor cadres. The program was conducted at SMA Negeri 1 Kasihan and involved 36 students. The implementation consisted of coordination with the partner school, preparation of educational materials, blood donation education using the interactive Game Fe Card, reflective discussions, and the selection of blood donor cadres. Participants' knowledge was evaluated using pre-test and post-test questionnaires. The educational intervention significantly improved participants' knowledge of blood donation. The proportion of participants with good knowledge increased from 11.11% before the intervention to 69.44% after the intervention, while the moderate and low knowledge categories decreased from 50.00% to 27.78% and from 38.89% to 2.78%, respectively. Furthermore, eight participants were selected as sustainable blood donor cadres based on their active participation and understanding of the educational materials. The interactive learning approach using Game Fe Card encouraged active engagement and strengthened participants' understanding of blood donation concepts. Game Fe Card is an effective educational medium for improving adolescents' knowledge of blood donation and supporting the establishment of sustainable blood donor cadres. This approach has the potential to promote voluntary blood donation awareness among adolescents and strengthen the sustainability of blood donation programs through peer.
