Noncommunicable diseases, particularly hypertension and diabetes mellitus, remain a public health issue in Kepuharjo Village. This situation highlights the need for promotive and preventive efforts through increased knowledge and early detection of disease risk factors. This community service activity aims to improve the community’s knowledge of hypertension, diabetes mellitus, and the CERDIK behavior as a means of preventing noncommunicable diseases, as well as to raise awareness about the importance of regular health checkups. The activity was conducted in Jambu Hamlet, Kepuharjo Village, Cangkringan Subdistrict, Sleman Regency, involving 30 participants consisting of women of childbearing age and the elderly. The activity methods included participant registration; measurement of body weight, height, waist circumference, blood pressure, and random blood glucose levels; completion of a pre-test; health education; discussions; distribution of leaflets; and completion of a post-test. A descriptive analysis was conducted by comparing knowledge scores before and after the education session. The average knowledge score increased from 68.33 on the pre-test to 99.33 on the post-test. A total of 28 participants scored 100 and two participants scored 90 on the post-test. Descriptively, the activity demonstrated an increase in participants’ knowledge following the health education and health screenings. Similar activities need to be implemented on an ongoing basis, involving community health workers and the Non-Communicable Disease Screening Posts (Posbindu), to strengthen prevention and early detection efforts in the community
