Abstract

Hypertension is a non-communicable disease characterized by a persistent increase in blood pressure with a systolic value ≥140 mmHg and diastolic ≥90 mmHg, and represents one of the leading causes of premature death worldwide. According to WHO (2023), 33% of the world's population suffers from hypertension, while the 2023 Indonesian Health Survey (SKI) recorded hypertension as the highest risk factor for mortality at 10.2%. At Puskesmas Perbaungan, 1,327 hypertensive patients were recorded in 2025. Non-pharmacological interventions such as Moringa oleifera leaf tea decoction—rich in antioxidants, potassium, magnesium, and anti-inflammatory compounds—combined with lifestyle education are considered an affordable and accessible complementary approach in blood pressure management. This study aimed to determine the effect of Moringa leaf tea decoction and lifestyle education on blood pressure reduction in hypertensive patients at Puskesmas Perbaungan, Serdang Bedagai Regency. This was a quantitative quasi-experimental study with a pre-test and post-test with control group design. Sampling used purposive sampling with a total of 60 respondents divided into an intervention group (n=30) and a control group (n=30). The intervention group received Moringa leaf tea decoction combined with lifestyle education, while the control group received no intervention. Blood pressure was measured using a sphygmomanometer before and after intervention. Data were analyzed using the Wilcoxon Signed Rank Test at a 95% confidence level. Results showed that in the intervention group, the mean systolic blood pressure decreased from 160.53 mmHg to 147.23 mmHg (a reduction of 13.30 mmHg), and the mean diastolic blood pressure decreased from 96.33 mmHg to 87.90 mmHg (a reduction of 8.43 mmHg). The Wilcoxon test showed p-value = 0.02 for systolic (Z = -3.051) and p-value = 0.00 for diastolic (Z = -3.501), both below the significance level of 0.05. The control group showed no significant change (p = 0.46). It is concluded that the combined administration of Moringa leaf tea decoction and lifestyle education significantly reduced both systolic and diastolic blood pressure in hypertensive patients. This intervention is recommended as a safe, affordable, and evidence-based complementary therapy in hypertension management at the primary health care level.
