The implementation of the home visit health service program in Medan, particularly at the Padang Bulan Selayang II Community Health Center (Puskesmas Padang Bulan Selayang II), was motivated by ongoing public complaints about the quality of health services received, necessitating an evaluation of its implementation. This study aims to evaluate the implementation of the home visit health service program at the Padang Bulan Selayang II Community Health Center. This study employed a qualitative descriptive approach with in-depth interviews, and data collection techniques included interviews, observation, and documentation. Data analysis was conducted qualitatively using the program evaluation theory of Peter Bridgman and Glyn Davis, covering aspects of input, process, outcome, and impact. Data validity was tested through triangulation between interview and observation results. The results indicate that the home visit program has generally been successful, particularly in achieving targets, outputs, and outcomes. However, it is not yet optimal due to ongoing obstacles in the implementation process and the low level of community mindset shift from a sick to a healthy paradigm. Therefore, it is necessary to increase the number of health workers, strengthen program outreach, and intensify communication with the community to optimize the effectiveness of home visit health services.
