<![CDATA[Pregnancy with anemia is a condition in which a pregnant woman experiences low hemoglobin levels (generally <11 g/dL), reducing the blood’s ability to carry oxygen and potentially affecting maternal and fetal health. Moreover, maternal age >35 years is classified as a high risk group, making appropriate care essential to prevent complications and maintain health during pregnancy. This case report aims to describe midwifery care for a multigravida woman in the second trimester, aged over 35 years, with moderate anemia, conducted at the Independent Midwifery Practice U Kamal, Bangkalan, between March–June 2026, and at the home of Mrs. M. At the first visit, the mother presented with moderate anemia symptoms including pallor, weakness, easy fatigue, and shortness of breath. Risk factors included advanced maternal age, noncompliance with iron tablet intake, and limited nutritional intake. At the second visit, the mother reported back pain, which is a physiological condition in pregnancy. By the third visit, the mother was no longer anemia, supported by general examination findings of pink conjunctiva. Thus, the remaining risk factor was maternal age, as anemia was no longer present. Management consisted of nutritional counseling, administration of iron tablets, and recommendations to consume iron rich foods along with vitamin C. Comprehensive midwifery care combining supplementation, education, and family support is crucial to reduce the risk of complications and to minimize moderate anemia in multigravida pregnant women aged >35 years.]]>
